1 10TH ANNUAL HOPE FOR THE HOLIDAYS TELETHON Sunday, December 17, 2017 PRESENTING SPONSOR-$30,000 (SOLD) You will have an omnipresent logo displayed throughout the three-hour program on the lower third banner (logo will not appear during commercial breaks). You will receive six 30-second time slots during the telethon to showcase your organization with commercial(s) of your choice. All marketing (including Fox 13 commercial spots and online materials) will be identified as the Tenth Annual Hope for the Holidays Telethon Presented By... with your organization s name and/or logo. We will identify you on our two Interstate-15 LED electronic billboards as a proud HSU sponsor during November/December Additionally, our telethon billboard ads will feature your logo. Organization s logo will be featured on our Facebook cover photo for one month preceding the telethon. Business name tagged on all telethon-related social media-facebook posts, advertisements and events (not including Pre-Telethon Match Posts). Your executive will be recognized at the top of each hour with an introduction/interview and acknowledgement of your generous support. The opportunity for four of your organization s employees or guests, wearing your logo on shirts or hats, to staff the front row of the telethon phone bank for one hour. This will include one brief interview/spotlight by the hosts. A portion of your sponsorship donation will be recognized on air as a Matching Grant to garner additional recognition for your organization and to stimulate donations. The hosts will repeatedly mention your organization s name and your logo will be displayed in the designated area during the match period.

2 3-HOUR SPONSOR-$30,000 (SOLD) For all 3 hours of our program, your logo will be prominently displayed on the designated backdrop during all in-studio interviews, the running total board, and during all pet segments. You will receive six 30-second time slots to showcase your organization with a commercial of your choice (2 per hour). Your organization s name will be tagged on all telethon-related social media outreach (Facebook posts & ads-not including Pre-Telethon Match Gift Posts). The opportunity for six of your organization s employees or guests, wearing your logo on shirts or hats, to staff the front row of the telethon phone bank. (2 per hour) Opportunity for a company representative to present a check to HSU for the total amount given and talk about your support. (Pre-filmed check presentation; approximately 2 minutes in length).

3 PRE-TELETHON MATCH SPONSOR-$20,000 (SOLD) All pre-telethon match marketing materials (website lightbox, social media posts, and campaign) will exclusively feature your company logo and designation as brought to you by (Company Name). A company executive will be interviewed at the start of the broadcast to talk about your contribution. You will receive four 30-second time slots to showcase your organization with a commercial of your choice. Logo to appear on all City Weekly advertising for the telethon. Inclusion in Support our Partners campaign recognizing your contribution and encouraging HSU constituents to support your business. The opportunity for two of your organization s employees or guests, wearing your logo on shirts or hats, to staff the front row of the Telethon phone bank. This will include one brief interview/spotlight by the hosts.

Your sponsorship of the event is an excellent opportunity to make an end-of-year taxdeductible contribution and, best of all, to make a lasting impact for the thousands of animals that will continue to rely on us for love, and hope for many years to come. PRESENTING, PRE-MATCH, AND HOURLY MEDIA IMPRESSIONS: Logo inclusion on full-page back cover ad in Pets in the City Magazine (50,000/month). Logo on Fox 13 telethon landing page. Your choice of a 1-year dog kennel or cat condo sponsorship plaque with your logo. Logo with click-through to your website on HSU event webpage. Thank you in our mail/ newsletter to (60,000/ s). Use of HSU logo for one year in your marketing materials. Humane Society of Utah Facebook - 88,000 Likes; 120,000 Monthly Reach; 72,500 Monthly Engagement HSU Quarterly Newsletter (14,000/mail; 60,000/ s). Logo Inclusion on City Weekly half-page ad (50,000/week). HSU Event Notications (60,000/ s). Fox 13 Commercials (2,338,400/100 commercial spots). 5 SUPPORTING SPONSOR-$7,500 You will receive one 30-second time slot (or two 15-second time slots) during the telethon to showcase your organization with a commercial of your choice. Inclusion in Support our Partners campaign recognizing your contribution and encouraging HSU constituents to support your business. Two of your company representatives on camera phone bank for 1-hour with company logo on placard, including an interview with a Fox 13 News Anchor. MATCHING GIFT SPONSOR $3000 level - As the sponsor of a matching grant, your organization will challenge viewers to match your contribution during for 2 minutes during our broadcast. Your logo will appear next to the countdown during the matching grant window.

6 2017 Sponsor Form Individual/Company Name Contact Name Title Mailing Address Phone Company Representative Signature Proceeds will be used to: -Create an environment of respect, responsibility and compassion for all animals across our state. -Provide veterinary care and sterilization surgeries at our clinic for all homeless companion animals in our care (over 10,500 spays/neuters performed per year) as well as affordable sterilization and vaccination services for the public. -Facilitate foster care opportunities for special needs, sick or baby animals to ensure adoptability. -Manage transfer and rescue network of over 320 partner facilities and groups to place more animals. -Offer off-site adoption events and rapidly growing social media presence to expand the visibility of adoptable pets and engage the public in our efforts to control the pet population and increase adoptions. -Foster volunteer program with 700 active volunteers to keep all pets healthy and happy at our facility. -Further our multi-pronged approach to saving lives by constantly improving our shelter amenities and capacity to increase adoptions and placements of dogs and cats. The Humane Society of Utah holds high standards in executing contracts and sponsorship benefits. However, in some instances, we shall not be liable for any failure of or delay in the performance of this Agreement for the period such failure or delay is due to causes beyond its reasonable control, including but not limited to television scheduling conflicts, strikes of labor or disputes, acts of God, embargoes, government orders or any other occurrence beyond their respective control.
