1 Dear Potential Sponsor/Exhibitor: The Organization, Caribbean lnstitute for Strategic Planning and Research in Oral Health (CISPROH) invites you to become a potential sponsor/exhibitor at our 7 th International Symposium from October 25-27, 2018 at Jamaica Pegasus Hotel, Kingston. The organization is a collaborative effort between the Ministry of Health, University of Technology, Jamaica and Jamaica Association of Public Dental Surgeons that provides continuing education opportunities for the dental profession. Our 2017 symposium was a huge success. We set record high numbers in attendance. Total registration in 2017 was over 300 dentists, auxiliaries and students. Included were over 100 dentists from both public and private sector from across the island over 70 auxiliaries and 7 international speakers. Our exhibit floor contained over 15 exhibits. With the success of last year s conference and the excellent caliber of speakers, it continues to solidify the position of CISPROH as one of the largest dental symposiums in the Caribbean and as the premier organizations that offer Continuing Professional Education credits for members in the dental community. All forms must be completed and returned to us before July 31, Please note that a deposit of $50,000 must accompany your signed Application. Applications received without a deposit will NOT be processed and spnsors/exhibitor assignment will not be made. Sponsors/Exhibitors from the 2017 Symposium will be given preference providing their applications are received prior to July 1, After this date, exhibitors will be allocated on a first-come, firstserve basis and your priority status will no longer be considered. 5 EXHIBITOR Trestle Table 6 x 3 Booth Space 10 x10 $500 USD $850 USD *Please note company s doing trestle table or booth only are allowed to bring up to two (2) persons and will NOT receive any lunch. In addition, company's name will be placed on website as an exhibitor for the symposium EXHIBIT SPACE: A 50% non-refundable deposit, per exhibition, MUST accompany the application. PROGRAM ADVERTISING SPACE Half page- $20,000 Full page--$30,000 Applications submitted without a deposit will not be processed until the deposit is received. Reservations for exhibition space will not be accepted over the telephone. Balance of payment is due not later than July 31, Non-payment for space by this date may be interpreted by the Committee to mean that the exhibiting company no longer wishes to reserve space, and the booth reservation will be moved to the waiting list. For applications submitted after July 31, 2018, full payment must accompany the application. Full payment of exhibitor fees must be received before the space can be occupied. CANCELLATION/REDUCTION POLICY All cancellations/reductions must be received in writing on company letterhead. Sponsorship/exhibition space cancelled/reduced by July 15, 2017 are subject to a US $ administrative/processing fee. No refunds will be made for cancellation/reductions after September 15, No refunds and credits will be given on-site they will be disbursed within 30 days after the symposium.

6 Sponsors/exhibitors are responsible for the safety of their exhibits. CISPROH does not assume any liability for lost or damaged materials. We are inviting a limited number of sponsors/exhibitors and tables will be filled on a first come, first served basis with preference being given to exhibitors from last year s symposium.
