2 Dear Iowa Community Health Center Supporter, The Iowa Primary Care Association (Iowa PCA) and its member health centers truly value our relationship with your organization. This year s conference, Empowering Change, will be held on October 17-18, 2017, at the Marriott in West Des Moines, Iowa. We will be offering four levels of sponsorships this year; Platinum - $10,000, Gold - $6,000, Silver - $3,500, and Bronze - $2,000. We will also have the option to be a vendor for $750 and $250 for each additional staff member that attends. 3 About Us The Iowa Primary Care Association (Iowa PCA) provides technical assistance and training to Iowa Community Health Centers (CHCs) to support their ongoing commitment to provide quality, affordable primary and preventive health care to the underserved in Iowa. Many of our member health centers provide access to medical, dental, and behavioral health services. Because of this, our member health centers provide a health home to more than 188,000 individuals. Internet Access Wireless Internet access is available at no additional cost. Prizes Sponsors and vendors are required to provide a prize for the participant drawing during Wednesday s lunch. Participants will be required to visit every booth to receive a sticker in order to be entered in to the prize drawing. No refunds will be honored after October 1, PROGRAM ADS For the first time, we will be offering the option to design your own full page advertisement for the conference program. 7 FAQs How do I register to be a sponsor? Please visit to access the online registration forms. You can access the form by selecting 2017 Iowa Primary Care Association - Sponsors or 2017 Iowa Primary Care Association - Vendors under the calendar section on the bottom right side of the home page. Is there an advantage to submitting our registration early? By submitting your registration early, you have a much better chance of securing an exhibitor table and be included in promotional materials. What is included at my exhibitor table? Sponsors/Vendors will be assigned an 8 x 8 table including a white table cloth, black skirting, and two chairs. Electrical outlets will be available upon request. Please indicate if you will need access to an electrical outlet during the registration process. What credit cards can I use to pay for my registration? Mastercard, Visa or Discover Can I receive an invoice for my registration fee? Yes, when registering select the Bill Me Later option at checkout. Where can I send my materials before the conference? Booths are assigned based on sponsorship level and a first-come, first-served basis and will be finalized by October 6, Space assignments will be ed before the conference and tables will include table tents with the assignments. Will there be internet in the exhibit area? Wireless internet will be available free of charge. What are your ad specifications? Color Mode CYMK; Resolution 300 dpi or higher; File format JPG or EPS preferred, may be PDF; may include full bleed. Sizes and orientation of full page ad: 8.5 wide by 11 high. Ads can be full color. If you purchase a print ad, the creative must be ed to Katie Owens no later than October 2. How do I submit my logo and company description or full page ad to be included in the program and promotional materials? All sponsors and vendors that have confirmed space before September 15 will be included in the conference program and website. All logos should be sent in high-quality JPG or EPS format so they can be scaled and used as needed. Logos and descriptions should be submitted to Katie Owens at kowens@iowapca.org.
