3 1. Purpose 1.1. This document sets out Northern Devon Healthcare NHS Trust s best practice guidelines for Agreed Surgical Blood Ordering Schedule The following general principles can be applied in order to improve: Inappropriate requests for blood to cover surgical procedures Stockholding of blood in particular at times of shortage in accordance with best practice guidance. This guideline applies to all surgical teams. Non compliance with this guideline should be for valid clinical reasons only. The reason for non-compliance must be documented clearly in the patient s notes. 2. Definitions XM Cross Match. i.e. Serological testing of patients blood against donor blood to ensure compatibility EXM Electronic Cross Match. i.e. provision of suitable blood without serological cross match G&S Group and Save. i.e. Perform group and antibody screen to facilitate cross match, or electronic cross match if subsequently required No requirement to test as the need for blood is considered most unlikely 3. Monitoring Compliance with and the Effectiveness of the Guideline Monitoring of implementation, effectiveness and compliance with these guidelines will be the responsibility of the Blood Transfusion Laboratory Manager.
