5 Vesolock Polymer Ligation System Advanced Clip Design Polymer locking clips are designed to provide security on vessels and reduce lateral clip slippage. Smooth v-shape tips designed for easier closing/latching Well-defined teeth engineered to provide enhanced grip. Polymer clips are engineered to help eliminate thermal damage caused by energy-based ligation devices. More defined edges to push through connective tissue. Appliers are engineered for security, and help eliminate clip fallout. Fingers guide applier for easy clip loading. Cartridge system contains barium to improve detection. Adhesive tape on base allows adherence to any sterile surface. Large base with deep side grooves make handling easy. 8 Symmetry Vesolock Polymer Ligation Clip Applier Programs Symmetry Surgical is committed to providing expertly crafted appliers to customers who purchase its Vesolock Polymer Ligation. Symmetry offers warranty and repair programs on those appliers to ensure that they always perform to the highest standards. Applier Repair/Replace Program Symmetry Surgical s Vesolock polymer open and endoscopic appliers are available for purchase and come with very strong warranties. ENDOSCOPIC Appliers: Vesolock endoscopic appliers purchased from Symmetry Surgical come with a limited two-year warranty under which we will repair and replace endoscopic appliers for two years from the date of purchase at no charge. OPEN Appliers: Vesolock open appliers purchased from Symmetry Surgical come with a Limited Lifetime Warranty. Under this warranty Symmetry Surgical will repair or replace any open Vesolock appliers that fail to perform to specifications, at no charge, as long they have not been serviced or repaired by an unauthorized third party and have only been used to apply Vesolock Polymer Ligation Clips. ENDOSCOPIC & OPEN appliers already owned by the hospital: Symmetry Surgical offers a free repair service regardless of manufacturer. If the applier is deemed unrepairable, Symmetry Surgical will work with you to order a replacement applier suitable for Vesolock Polymer Ligation Clips. 8

10 Superior Clip Design Chevron shape designed to engulf tissue with precise tip-to-tip closure. Heart-shaped grooved wire designed to provide a firm, secure grip on vessels. Heart-shaped cross section designed to maximize surface-to-surface contact with applier jaw. Deeper transverse grooves designed to resist slipping across vessels for secure grip. Deeper center valley designed to provide more tissue space for added grip. Loading is easy with our patented push and click system that provides feedback when a clip is loaded. Cartridge system contains barium to improve detection. Applier fingers provide a guide for ease of loading each clip into the applier. After loading, fingers remain in a down position to indicate the clip has been loaded. Advanced Cartridge System Adhesive tape on base allows adherence to any sterile, dry surface. 13 Vesocclude Titanium Ligation Clip Applier Programs Symmetry Surgical offers two programs to provide appliers to customers using our Vesocclude Titanium Ligation Clips, demonstrating our commitment to providing expertly crafted appliers that are inspected and maintained to the highest standards. Applier Purchase Program Symmetry Surgical s Vesocclude open and endoscopic appliers are available for purchase and come with very strong warranties. OPEN Appliers: Purchased Open Appliers come with a Limited Lifetime Warranty. Symmetry Surgical will maintain, repair or replace your Open Vesocclude appliers, at no charge, as long as the Open appliers are only used to apply Vesocclude Titanium Ligation Clips and the appliers have not been serviced or repaired by a third-party vendor. ENDOSCOPIC Appliers: Purchased endoscopic appliers come with a limited two-year warranty under which we will repair, replace, and maintain endoscopic appliers for two years from the date of purchase. Applier Loaner Program Symmetry Surgical will loan a sufficient supply of open appliers to accounts that agree to purchase our Vesocclude Titanium Ligation Clips. The number of appliers loaned will be based on Vesocclude Titanium Ligation Clip volume. Loaned appliers, valued at $143 each, will be provided at no charge to the hospital and are only intended for use with Vesocclude Titanium Ligation Clips (loan agreement required). In addition, Symmetry Surgical will maintain, replace or repair loaner appliers at no charge to the hospital. Should a hospital decide to discontinue use of Vesocclude Titanium Ligation Clips, the appliers must be returned to Symmetry Surgical in good condition, normal wear and tear excepted, to avoid a charge. 13
