3 0B1. Guidelines Response to the pre-qualification questionnaire shall be completed in English by a senior member of your company. The requirements of each question shall be addressed comprehensively, with all supporting documentation duly provided, to enable Oman LNG evaluate thoroughly against the requirements for the required services. It should be noted that this document is solely for the pre-qualification evaluation of the company, for the purposes of inclusion on Oman LNG Tenderers Lists for the stipulated requirements. 4 1B2. Background Oman LNG LLC (hereinafter referred to as COMPANY ) intends to issue a tender for the Industrial Plant Cleaning Services and Maintenance Services for Catalyst Change Contract. COMPANY wishes to pre-qualify potential contractors capable of managing and executing such services, and hereby invites you to submit pre-qualification documentation as described in this document. 2B3. Brief Description of Scope The scope of work will include one or both of the following 2 Scopes/contracts. Capable contractors to tender for one or both scopes as detailed in section 4 of this document Objective of Pre-Qualification First Scope: Industrial Plant Cleaning Services First Scope includes the provision of the following industrial cleaning, in-situ and exsitu, services mainly during scheduled shutdowns and also for emergencies work and other day to day routine and scheduled maintenance of Plant equipments for a contract duration of four years or optionaly 4+4 years (total of 8 years). The Services to be provided shall include but not limited to the following: 1. Provide services of High Pressure Industrial Cleaning using pumps up to 10,000 psi with experienced and certified operators, for the cleaning of heat exchangers, pipelines, vessels, tanks, columns and reactors. 2. Provide services of hydraulic bundle pulling with the Peinemann bundle puller, including the operator certified to work with this equipment. 3. Provide services of hydraulic bolt tensioning and torquing for heat exchangers, pipelines, vessels, tanks, columns and reactors. 4. Provide services for chemical cleaning, including all equipment and experienced staff to operate the chemical cleaning units. 5. Provide services for cleaning of drain, pits, interceptors, structures, platforms within the plant areas using high pressure water jet cleaning. 6. Provide steam cleaning services for the internals of the heat exchangers, vessels, tanks, columns and reactors. 7. Provide services for ultra high-pressure equipment up to 30,000 psi and experienced personnel for the cutting of metal piping and equipment. Page 4 of 10

5 8. Provide services of spading, despading, opening covers, opening manholes, reassembly and testing of heat exchangers, pipelines, vessels, tanks, columns and reactors. This work has to be done with hydraulic tensioning/torquing according to COMPANY procedures and work-instructions. 9. Provide services of disposal of industrial cleaning waste materials from site to proper dumping ground within OLNG site. The detailed scope shall be provided in the tender document to be issued to those contractors that are invited to tender following this pre-qualification exercise. Second Scope: Maintenance Services for Catalyst Change Second scope includes the provision of general Catalyst handling services within the Qalhat Site Complex through professional and experienced supervision and labour, with proper equipment. The Contractor shall be be able to mobilise quickly to carry out on call-off request Catalyst Handling works. The CONTRACTOR shall assist the COMPANY in carrying out the work in a safe, professional and efficient way. The contract duration of this sscope is four years or optionaly 4+4 years (total of 8 years). The Services to be provided shall include but not limited to the following: 1. Spading and despading of vessels/columns and opening of the top by removing the elbow and opening of dump nozzles. On equipment in unit 1300 this work has to be done with hydraulic tensioning according COMPANY procedures and workinstructions. 2. Off-loading of catalyst, including working with a life support system used for inert condition, from process equipment as well as off-loading using wet method. 3. Loading of catalyst, including working with a life support system used for inert condition, into process equipment. 4. Packing of spent catalyst into designated containers or drums for transportation to spent catalyst storage area within COMPANY site. 5. Transfer of packed spent catalyst to designated storage site within COMPANY. 6. Sieving of catalyst, including under inert condition, for separation of fines, different sizes of catalyst and ceramic balls and/or contaminants. 7. Skimming of catalyst layer as specified by COMPANY, including the use of life support system. Page 5 of 10

6 8. Sampling of catalyst in catalyst vessel according to sampling pattern given by COMPANY, including the use of life support system for fresh and spent catalyst. 9. Cleaning of process equipment and piping which contains catalyst such as vessels, filters, columns, tanks, etc. including working with life support system. 10. Provision and proper usage of an intrinsically safe video color monitoring system that shows all catalyst activities internally in the process equipment and allows COMPANY representatives to make quality audits. 4. Objective of Pre-Qualification The objective of the pre-qualification exercise is to invite and identify potential capable contractors to tender for one or more of the following 4 options : Option 1: First Scope: Industrial Plant Cleaning Services for contract duration of 4 years. Option 2: Second Scope: Maintenance Services for Catalyst Change for contract duration of 4 years. Option 3: Both first and Second Scopes: Industrial Plant Cleaning Services and Maintenance Services for Catalyst Change for contract duration of 4 years. Option 4: Contract duration of 8 years for any of the 3 options above. Contractors shall be pre-qualified based on information submitted, in response to this prequalification, that demonstrate capability in the performance and management of the scope. Should you wish to be included (if pre-qualified) on the Tenderers list, kindly indicate that you have firm intension of tendering by providing the following: 4.1 General Commercial Information Registration: (a) Submit copies of commercial registration in Oman. (b) Give the name of the Owner(s) and their percentage of ownership. (c) Give the name(s) and position(s) of your authorized representatives. (d) Address and contacts numbers Current Structure of your organization listing: (a) Numbers of personnel directly employed by your company with beak up of personnel per trade/ discipline. Identify who has the HSE role / responsibility and briefly describe their HSE related job / works. - If no policy exists, explain how you intend to deal with HSE-related issues HSE TRAINING Submit list of training programmes and records of HSE training attended by your staff and clearly indicate if these staff(s) will be working on this project SAFETY RECORDS Submit 2 examples of your safety records for the last 5 years 4.4 QUALITY ASSURANCE Submit one copy of the following. If none issued, clearly state so: - Quality policy, endorsed by your Managing Director - Corporate quality manual and index of your procedures - Quality assurance system (e.g. ISO 9000) - Quality audit reports of your quality assurance system carried out by third parties in the last 5 years - Organisation chart detailing the functional responsibility of quality Page 9 of 10
