2 APPLIED RESEARCH WORKS - COZEVA WELCOME COZEVA Cozeva utilizes dashboards and registries to display performance on quality measures alongside rich interfaces of clinically integrated data. Log in to COZEVA To log in to COZEVA visit Cozeva is best experienced using Google Chrome or Mozilla Firefox web-browsers. All other browsers are not supported or recommended. Be sure to keep your selected browser updated as recommended by its software developer. Upon initial log in, you can change your password, setup Security Questions and provide other relevant information in the event you forget your password. Be sure to follow all password requirements and select SAVE when completed. Troubleshooting access The login portal password field is case sensitive. Be sure your CAPS lock key is not enabled when entering your password at the login screen. If you have forgotten your username, password, or for any other reason are unable to access Cozeva, please contact our Support Team toll free at 1 (877) Monday-Friday**, 8 am 5 pm HST. **Except for the following observed holidays: New Year's Day, Dr. Martin Luther King Jr. Day, Presidents Day, Memorial Day, Independence Day, Labor Day, Veterans Day, Thanksgiving Day, and Christmas Day Connectivity limitations Some COZEVA builds integrate with different connectivity technologies. Requirements differ by Organization. *Note this guide does not contain any real patient or provider information.

3 ACCESS Connecting COZEVA uses a two step verification method of authentication.* 1. Navigate your web-browser to 2. Enter your username and password 3. A code will be automatically generated and sent to your work address. 4. Cozeva will display a 2-Step Verification Screen (below) 5. Input your verification code. ** Select Remember this computer for 30 days to have COZEVA skip the verification process for trusted computers for this time period. The 2-Step authentication process adds an extra layer of security when connecting to COZEVA. Choose or phone text delivery of authentication codes My Settings. Once Connected Upon successful login and acceptance of the Terms of Service, you will have the opportunity to change your password and input a date of birth. You must also select 3 Security Questions and answers. Be sure to hit the Save button in the top right corner when finished. *Some builds of COZEVA utilize a Single Sign-on authentication method, bypassing the Two-Step Authentication process.

4 OVERVIEW *Selecting the Lotus Flower toggles the left navigation menu. There are 3 main pages in Payment Transformation: PCP Dashboard Performance Measure Summary Page Help? Help bubbles provide information and/or hyperlinks to resources. Not all bubbles are available in all views. My Settings Change password Security questions Profile information Avatar selection Auto log-out option Contact information Tools Expand current pane Print/export current pane Refresh current pane View short video tutorial Total Cost of Care Dashboard Landing Page The Landing Page in Cozeva defaults to the Provider Dashboard. The components on the Dashboard include: Current patient count by LOB Dollars Earned Bar* PCP Performance Measure Summary Graph/Bar* Top 10 List TCOC Breakdown Bar TCOC PMPM Clinical Panel Risk Score Hospitalization Activity Chart *Data reflects care gaps and earned/potential payment in the Payment Transformation program.

5 PERFORMANCE MEASURE SUMMARY PAGE 3/12 Toggle Yes- Show Payments Associated with 3/12 attribution rule. Reflects in Performance, Payment and Denominator columns. Performance Column Clicking on performance bars give break out by LOB as well as overall performance in the measure Orange Triangle represents provider s baseline Black triangle to the left represents Minimum Threshold Dollars Earned Shows how much money the individual PCP has earned and how much money is remaining to earn across all LOBs. Clicking on payment bars provides a break out by LOB as well as overall dollars earned for the measure Only visible when 3/12 is toggled to Yes No- Show performance in measures for all current patients on providers panel. Performance Measure Summary Page The Measure Summary Page in COZEVA displays Provider performance across all measures. Selecting a measure from the PCP Performance Measures will open member-level registries on the right hand side of the screen. The following measure sets can appear on the Performance Measure Summary page: PCP Performance Measures (up to 21), PO Performance Measures (6), PCP Engagement Measures (5), PO Engagement Measures (6). Cozeva will not display a performance measure with a zero denominator on the Performance Measure Summary Page.

6 TOTAL COST OF CARE DASHBOARD With Social Comparison Clicking on any patient name will direct you to their members care ops page Without Social Comparison Overview The TCOC dashboard provides details on the cost of each PCP s panel. TCOC data only includes Commercial members, and does not include drug or behavioral health costs. The following components can be found on the TCOC Dashboard: TCOC PMPM Social Comparison Graph* Clinical Panel Risk Score Social Comparison Graph* Your TCOC PMPM Top 10 Charts PO Median TCOC PMPM Cost Utilization, Hospitalization, ER, Office, Imaging Network Median TCOC PMPM Total Cost of Care Breakdown Bar Your Clinical Panel Risk Score PO Median Clinical Panel Risk Score * When available Network Median Clinical Panel Risk Score

7 VIEWING MEMBER INFORMATION Care Opportunities Patient Demographic Info & Ribbon Red Dot = Open Care Gap Hollow Dot = Pending Care Gap No Dot = Closed Care Gap RAF # of Emergency Department Visits (<12 months) # of Hospital Admittances (<12 months) Cost vs. Risk graph quadrant Actual vs. Potential RAF Score Patient Panel Selecting a Member in Cozeva populates the Patient Profile, which displays Demographic Information, Care Opportunities and Encounters across the network. Red dots appear alongside open Care Gaps or actionable Active Conditions. Hollow dots appear alongside Gaps that are expected to close within three weeks. No dots appear alongside closed Care Gaps. Encounter Timeline Chronological timeline of encounters for a member across the network. Select an Encounter for detailed Information. Encounter Types Ambulatory InPatient Lab Outpatient Prescription Other Detailed Care Opportunity History Detailed Care Opportunity History information is displayed to the right of all Care Gaps. Cozeva displays all relevant Service or Diagnosis codes, descriptions, corresponding Provider or Specialist, results and service date(s).* Multiple service dates are viewable by expanding each service code type by selecting the + button. * When available

8 ACTIVE CONDITIONS/HCC S Working with Active Conditions (HCC s) COZEVA displays HCC s identified through past diagnoses and suspect HCC s that meet suspect model criteria in the Active Conditions measure and registry. (i.e. Review of Chronic Conditions, Review of Suspect Conditions measures) Legend Red Dot: Hollow Dot: No Dot: Non confirmed HCC Pending confirmed HCC Confirmed HCC HCC display HCC justification Service codes Provider history Service Date Patient display of HCC s Previously diagnosed HCC s needing evaluation/recapture by the provider are displayed alongside suspect HCC s. Both are confirmed (red dot removed,) when a supporting claim is received by COZEVA. HCC s and suspect HCC s may also be disconfirmed, or marked as pending using the Supplemental Data tool. Those marked as pending via this method must still be accompanied by a supporting claim. 9 SUPPLEMENTAL DATA Accessing Supplemental Data To access the supplemental data forms click the pencil icon. Working with Supplemental Data The upload of supporting documentation is accomplished within the Supplemental Data Attestation screen in COZEVA for Care Gaps or Active Conditions requiring it. COZEVA accepts attachments in the following file formats:.jpg.jpeg.gif.png.txt.doc.docx.xls.xlsx.pdf.ppt. pptx.pps.odt.ods.odp.zip.rar.tiff.tif. Viewing Attestations & Supplemental Data View submitted Supplemental Data entries in the Attestations submenu item within the Registries sidebar item. 10 Detailed Member Data Information Detailed Member Data is available in the list menu icon next to a member s name. Member Data is available for: Medications Lab Results Immunizations Claims Other misc data Medication Information Cozeva displays a comprehensive list of Medications for a member. Select a prescription to view dosage, prescription type, prescribing physician, prescription start and stop dates and other related information (Not all fields are populated for all prescription types). Labs A comprehensive list of Lab results are displayed in COZEVA including results and test dates. Other related information and resources are accessible when selecting a Lab from the Labs List (Not all fields are populated for all Lab types). Lab Results in Graphical View Medication Adherence Cozeva displays medication adherence via a bar chart in Medications. 11 WORKING WITH BATCHES Create batches to group members together for printing, export or other purposes. Creating a batch 1. Search for members to be added to batch 2. Select check box next to the member(s) to be added, or the top check box to select all members. 3. Select the list menu icon at the top of the check box listing 4. Select Add to Batch ** Repeat steps 1-4 for each member(s) to be added ** Viewing a batch 1. Select View Batch from the Sidebar 2. Select check boxes next to member(s) to export 3. Select Print Care ops Batch from the Print/Export tool to view Patient Detail PDF s. Deleting a member(s) from a batch 1. Follow steps 1 through 3 in Part A: Creating a batch 2. Select the list menu icon at the top of the check box listing 3. Select Delete from Batch.
