32 Reporting Project Updates Project Attestations CNYCC allows partners who are designated at Federally Assisted Drug and Alcohol Treatment Program to report de-identified actively engaged patient rosters by utilizing an attestation for Behavioral Health/Primary Care Integration and Behavioral Health Crisis Stabilization CNYCC has updated the attestation process and will be reaching out to the following organizations: Crouse Hospital Farnham Family Services Syracuse Behavioral Health Partner attestations will auto-populate based on information entered into the document CNYCC will provide explicit instructions regarding the new attestations New attestations need to be implemented for April 2017 Patients, May 2017 AEP Reporting

33 Actively Engaged Reporting - Updates Beginning with April 2017 Actively Engaged Patient Rosters, Partners will have to include a Unique Identifier that is assigned to them by CNYCC which will replace the Partner Name The Unique Identifier will be 6 alphanumeric characters used to ID files uploaded to sftp Site CNYCC will be contacting organizations directly with Unique Identifier No Rosters will be accepted without proper naming convention 3ai April 2017 CNY104 Actively Engaged Patient Roster.xlsx 3ai April 2017 CNY104 AEP Roster.xlsx In order to assist Partners with the new naming convention, Partners will be able to submit AEP Rosters 5 days before deadline to check file for correct naming conventions Adjusted timeline allows for partners to resubmit patient files if necessary

41 HIT PIC Notes Questions and Answers When have identified issues/questions in our implementation plan (both initial and update) but get no response from CNYCC. Why is that? o CNYCC will work to give more appropriate and timely responses to feedback from Implementation Plans Thank you very much for including today's slides in Handouts. Just wondering if these materials are also available on CNYCC's website? o Slides will be available on the calendar page within 2 business days I am having a little bit of a hard time following, could you please break down on a very basic level from the implementation plans we submitted, how it evolves? o Information within the implementation plans and HIT surveys have provided valuable information in planning process for the PHM system as it has helped prioritize integrations on bullets from slide #18 With the focus on the EMR, are you looking for agencies with an established EMR platform? o CNYCC will look to organizations beyond ones with EMR for insight into the build of all care management related modules is there additional information or resources available we can share with the staff about IBM Watson Care Manager so for those that are regularly participating in the HIT PIC understand the background and context before this virtual meeting on 3.10? o CNYCC will share documents on the project webpage Will roster recon be occurring monthly? o Yes My understanding from presentation to the board was that partners would receive $15,000 per organization. This has changed? o CNYCC has funds that carryover from the DY1 funds flow for PHM, actual distribution details and final amounts will be determined as part for DY2 funds flow policies. Just so I'm clear - is implementation/integration with this Watson software a requirement? o Data sharing is a requirement of the 2ai project, and it is a goal to integrate as many partners as possible into the PHM system Is there any advantage to being an early adopter either financially or in terms of assistance from CNYCC? o Having access to data in the Watson program is vital to the success of DSRIP and having organization level insight we feel will be valuable. Specific financial advantages have not been discussed. IPOS training isn't on the CNYCC calendar...? thanks o Will follow up with Tammy VanEpps Who would you recommend attend the demo webinar? Staff roles? o Any staff that may be interested and staff related to slide #19 Do you have an estimate of the amount of provider IT time that will be necessary to commit to implementation?
