In recent years, the research in this field has grown significantly: Detection and/or characterization of ailments. Segmentation. Conventional radiography, computed tomography, magnetic resonance, ultrasound (breast, chest, colon...) and these lines of research are of most interest to the SERGAS. 3 APPLICATION AREA CT lung Lung RX DERMATOLOGÍA and ophthalmology CURRENT SITUATION Use of CAD in CT lung but only in some centers Use linked to the field of research Organizational difficulties in access to CAD tools Technique of clinical interest but with low development. Difficulties in access to CAD tools lung X-RAY Under use of CAD in dermatology, ophthalmology and other specialties. Difficulties in access to CAD tools in these modalities General Level: Few initiatives implemented and isolated Local Solutions Difficulties of updating Non-standard Use. Integration Difficulties. From the analisis carried out, arises the need to develop a platform that concentrate the management of CAD services (computer-aided diagnosis) in medical imaging for the access to proprietary and/or commercial algorithms with the aim of improving the productivity and the quality of the diagnostic of the optional tasks and facilitate implementation and scalability of these services. The functional requirements of this platform will be: Enable connectivity to any clinical workstation or computer on the network Scalability Adaptability: Use different systems according to the centre Homogeneity and single point of entry and exit in the CAD services Securing and controlling activity in the CAD services. 8 CAD MEDICAL IMAGE. DEFINITION CAD systems are computer applications that with advanced digital medical imaging processing helps the specialist in with diagnosis. It is also called computer-aided diagnosis systems. Item of research interest. In recent years research in this field has grown significantly. Detection and/or characterization of injury. Segmentation. Conventional radiography, CT, MRI, US (breast, chest, colon... )

9 CAD IMAGE. Concept. Basic Concept: The computer gives a response that is interpreted by a specialist as a helpful second opinion. Therefore research in CAD is : 1) The development and evaluation of algorithms 2) the study of the usefulness of the computer's response, For the radiologist or optional in other disciplines. Historical and formal Definition. The DAC is the diagnosis that performs a specialist that takes into account a quantitative analysis conducted by a computer on radiological images both historical and formal by Definition.

10 CAD MEDICAL IMAGE. DEFINITION Objective of the CAD systems by image: locate injury and to estimate the probability of it being a disease. Application. As a broad concept it has applications in almost all the imaging modalities and parts of the anatomy: X-rays, CT, MRI, US, nuclear medicine images of the skeleton, chest, heart, abdomen and extremities.

11 CAD MEDICAL IMAGE. DEFINITION It has been used in the detection of breast injuries in mammography (and RM), detection of lung nodules in X-ray and CT, detection of polyps of the colon in TC/colonoscopy... Advantage. Numerous studies, many recent, such as K. Doi and Q Li, have shown that their use improves the performance of radiologists, being greater improvement when it comes to general radiologists or residents.

Reduction in 5.3 years, the detection time. The detection of carcinoma has increased from 6.7 % to 36.8 % with the use of CAD in digital mammography in screening programs in relation to the conventional mammography.

17 CAD MEDICAL IMAGE DEFINITION CHEST X-RAY. 25% Of cases of lung cancer with previous X-RAY plate undiagnosed Detection and differential diagnosis of lung nodules and interstitial lung disease, pneumothorax.
