3 Getting/Using our Catalog 1 Our new Catalog provides all the specifications and details concerning every prescriptive incentive measure we offer. 5 About our Application There is only one Application: It is used for reserving funds (Reservation Application) and It is used for requesting incentive payments (Final Application). Funds must be reserved for all custom projects and lighting projects that involve de-lamping. We encourage you to reserve funds for all prescriptive projects. 5

6 About our Application The following items are required to successfully complete your application and receive your incentive funds: Application Checklist Customer Information Sheet Prescriptive and/or Custom Incentive Worksheets Final Application Agreement Supporting Information, including invoices and product specifications We have highlighted the necessary fields in the following pages to assist you in identifying required information. 6

7 Application Checklist 2a Use our Checklist to ensure you complete all relevant portions of both the Reservation* and Final Application *A Reservation Application is required for some projects and encouraged for all projects Complete this side to reserve funds before starting your project Complete this side to request payment once your project is completed 7

8 Customer/Project Information 2b Use this sheet to enter all detailed customer, contact and general project information Fill out the project and customer information completely 8

9 Contractor Information/ Third-party payments 2c Fill out the contractor information completely If no contractor was involved, enter self installed Complete this section upon submission of Final Application ONLY if payment is to go to someone other than the customer 9

10 Prescriptive Incentive Worksheets 2d We have worksheets for every type of measure offered in the program. Simply complete all relevant areas. In the hardcopy printout, you will have to manually enter and calculate your incentives In the interactive version, the incentives will be calculated for you 10

11 Custom Incentive Worksheets 2e For your custom projects, use these sheets to describe your work in detail. Be as complete as possible when calculating your custom measures and estimated savings. 11

13 Final Application The Final Application must be signed by the customer the DTE account holder not the contractor 13

15 Submitting your Final Application A Reminder: Here are items to include when submitting your Final Application: 1. Project number (also include on any fax cover sheet or ) Page 2 2. Properly signed Final Application Page 5 3. Actual project completion date Page 2 4. Dated, itemized invoices 5. Manufacturers spec sheets 6. Highlight changes between Reservation and Final applications 7. DTE account number (electric or natural gas) Page 3 8. Payment release authorization signed by customer, not contractor, if you want the payment to go to a third party Page 4 15

16 About invoices and spec sheets 2g We must have complete, itemized invoices for all equipment installed as part of your project. This is an example of an easy-to-read, itemized invoice with recognizable part numbers 16

17 About invoices and spec sheets Include manufacturer Spec Sheets for all installed equipment. Manufacturer specifications NOTE: Send only relevant sheets, not a complete catalog 17

20 Being Complete and Accurate 2h Be complete and accurate: More than 25% of all Applications are missing required information and these are the most common omissions: 1. Estimated or Actual Completion Date Page 2, 5 2. DTE Energy Account Number Page 3 3. Product Specification Sheets 4. Incentive Worksheets 5. Total Project Cost Page 5 6. Correct tax ID and status* -- Page 3 Remember: Submitting a complete and accurate Application speeds up the processing time which means we can send out your incentive check faster! *If you are applying for incentives for the first time, a copy of your W9 will ensure payment is made quickly and accurately 20
