INTRODUCTION APPLICATION FOR REGISTRATION GENERAL INFORMATION AND QUESTIONNAIRE FOR CONTRACTOR S PREQUALIFICATION NADRA Regional Head Office, Multan intends to reintroduce the process of Pre-Qualification of all its Suppliers, Contractors, and Service Providers in order to streamline its Procurement procedures. All reputable and established firms/contractors are invited to apply for the participation in the Pre-qualification Process and submit the Pre-Qualification questionnaire. A transparent evaluation method for the purpose of Pre-Qualification of the applicant s capabilities in the respect of vital elements of applicant s organization and capacity to perform shall be established in order to participate in the NADRA, RHO Multan tenders. Each Supplier or Contractor performance in terms of quality, delivery, after sales service, credit terms and other contractual obligations will be closely monitored and periodically reviewed during the contractual period. Instead of issuing the press tender, NADRA RHO Multan will issues the tender documents in favor of Pre-Qualified Contractors, who have been registered in that particular category. Contractors who are not Pre-Qualified or fail to meet the Pre-Qualifying criteria will not be allowed to participate in the tender. NADRA Regional Head Office, Multan will process all the procurements in accordance with the policy defined and specified under PPRA Rules by the Government of Pakistan. 2. DOCUMENTS NEEDED Please submit the following documents as selection for prequalification will be based on information provided in these documents and questionnaire. a. Sales Tax Registration Certificate. b. NTN Certificate. c. Proof of being active taxpayer d. Certificate/affidavit for being not black listed ever on legal paper. e. Last six months bank statement of firm/contractor. f. Details of employees of the firm (Permanent, Temporary, Technical and non-technical) with designation and contact numbers. g. Firm/Company Profile. h. List of workshops & equipment. i. List of POs/WOs with available copies of these orders. j. Authorization/partner Certificates (if available). k. 8 5. TERMS AND CONDITIONS i. The Applicants must respond to all questions and provide complete information as advised in this document. Any lapses to provide essential information may result in disqualification of the Applicant. ii. iii. iv. Firm/Contractor must have at least one repair & maintenance workshop with complete equipment and Technical staff in Multan city. The competent authority reserves the right to reject any or all applications in the light of PPRA Rules. Pre-qualification will be valid till June 30 th SEALING AND MARKING OF BIDS: a. The application shall be submitted in sealed envelope marked as under: Deputy Director (Admin) NADRA RHO, 221-A, SRA Colony, Multan b. The envelope shall also bear the word CONFIDENTIAL and following identifications:- Tender No. NADRA/ /06 Dated Prequalification for Repair of Generators c. If the envelope is not marked, as instructed above, the NADRA will assume no responsibility for the misplacement or premature opening of the bid. d. The bids should reach this office before or by 16 th of Aug, 2017 at 1130 Hrs.
