Management and Development for Health (MDH) is a non for profit organization in Tanzania contributing to address public health priorities. MDH works with the Ministry of Health, Community Development, Gender, Elderly & Children, academic and non-academic institutions within the country and worldwide, to address the problems of tuberculosis, malaria, HIV/AIDS, nutrition, maternal and child health and to undertake initiatives to advance public health research, education, and services for improving the lives of people. 2. MDH now invites sealed bids from eligible bidders for the following tenders: TENDER NO. MDH/ /NC/11 Pre Qualification for the provision of general services and maintenance of Motor vehicles and motor cycles. MDH/ /C/01 Pre qualification for provision of IT services LOT No.1 System and Software Development. MDH/ /W/01 Pre-qualification for the renovation and minor repair of health facility buildings 3. Interested eligible bidders may obtain further information from and inspect the Bidding Documents at the office of the Secretary to the Tender Board,

4 Procurement Management Unit, CM Plaza along Mwai Kibaki Road Plot 802 Mikocheni Area B. from 9.00 am to 3.30pm on Mondays to Fridays except on weekends and public holidays. 4. A complete set of Bidding Document(s) in English may be purchased by interested Bidders on the submission of a written application to the address given under paragraph 3 above and upon payment of a non-refundable fee of Tsh. 200,000/- ( Two Hundred Thousand shillings only) per tender. Payment should be made by Cash, or Banker s Cheque, payable to Management and Development for Health. 5. All bids in one original plus one copy properly filled in, and enclosed in plain envelopes clearly marked the relevant Tender number and details of the tender must be delivered in the Tender Box located at MDH offices, Ground floor, CM Plaza along Mwai Kibaki Road Plot 802 Mikocheni Area B. on or before Wednesday 20 th December, 2017 at 5.00 pm. All bids will be opened in the presence of Bidders representatives who choose to attend the opening ceremony. The venue for Tender opening shall be MDH CM Plaza, Meeting Room 1 st floor, along Mwai Kibaki Road Plot number 802, Mikocheni Area B on Thursday 21 st December, 2017 at 10:00 am 6. Bidders wishing to submit more than one bid must ensure that each bid is 7. submitted in a separate envelope with a clearly marked the Tender number and the description of the Tender. 8. Instruction to bidders: The bid document must consist of, application letter, company profile/physical address and contacts, legal trading documents (TIN Certificate, VRN Certificate, complaint to the Governments Tax Regulations, current business license, certificate of registration/incorporation), Financial statements (Audited accounts for the past three years), Bank Details, bank statements for the last three (3) months, companies past experience in the related field, personnel capabilities in the relevant field, manufacturers authorization where applicable, anti-bribery policy and power of attorney. 9.
