1 CUSTOMER INNOVATION STUDY Hackensack University Medical Center gets one step closer to precision medicine with cloud-based Infor Cloverleaf Integration Suite We are trying to understand human beings. So in order to do research, understand the data, and the information, we have to have access to the raw data. If researchers can look at all the data about a patient, and find the one unique piece of data, it can change a patient s entire treatment plan. For the patient, this means the right information for the right person at the right time, leading to the right care and best treatment plan. Infor Cloverleaf and the HL7 FHIR standard enable all of this data sharing and dialogue to happen. 4 Executive overview Situation analysis Uphold its pledge to the White House to provide individuals with access to their clinical data via open, standardized APIs, enabling connections with applications that allow patients to donate their data to support precision medicine research, as part of the Precision Medicine Initiative. Create a common Health Level Seven International (HL7) Fast Healthcare Interoperability Resources (FHIR) standard service layer, where any system can interact and share data among partners and clinical research teams, quickly and easily. Use mobile applications more broadly throughout the organization and empower patients to manage their own health data. Innovation strategy Support interoperability (the exchange of patient information between various systems) with the Infor Cloverleaf Integration Suite, enable the FHIR standard, and connect new patient data with Epic Interconnect, HackensackUMC s Electronic Health Record (EHR) system. Implement Infor Cloverleaf to build a web service interface layer that allows Epic Interconnect to query and update the primary EHR, bringing information from multiple sources through a common FHIR standard. Results Built a foundation to improve patient care, engagement, and empowerment by eliminating the one-size-fits-all approach to healthcare. Transformed how patients personal and health data is managed, while streamlining processes so that data is collected only once, creating more productive interactions between physicians and patients. Created a simple-to-use, mobile system for patients to schedule appointments and provide information via a single health questionnaire, which increased patient satisfaction, decreased waiting room time, and allowed for more face time between patients and physicians. High level impact 5X to 10X faster appointment scheduling process, avoiding the need for patients to research doctors, call the doctor s office within their office hours, and ensure that their insurance is accepted 1 minute to schedule an appointment via the mobile application, allowing real time access to physician availability and enhancing the patient experience 100 % increase in time patients can schedule an appointment from a traditional 12-hour day to 24-hour round the clock availability providing flexibility and getting patients in front of the right doctors as soon as possible Customer Innovation Study: Hackensack University Medical Center 4

5 SITUATION ANALYSIS Fulfilling the White House Precision Medicine Initiative Personalizing patient care Hackensack University Medical Center (HackensackUMC) is a 775-bed, nonprofit, teaching and research hospital located in Bergen County, NJ. HackensackUMC is the largest provider of inpatient and outpatient services in the state. Founded in 1888 as the county s first hospital, it is a member of Hackensack Meridian Health, one of the largest networks in the state comprised of 28,000 team members and more than 6,000 physicians. With 24 joint commission gold seals of approval, HackensackUMC has received numerous accolades for the quality of its care, including being listed as the number one hospital in New Jersey and one of the top four New York metro area hospitals. HackensackUMC is one of only five major academic medical centers in the nation to receive Healthgrades America s 50 Best Hospitals Award for five or more years in a row. It was the first hospital in New Jersey and second in the nation to become a Magnet recognized hospital for nursing excellence; receiving its fifth consecutive designation in HackensackUMC maintains its leadership, in part, by remaining at the forefront of the nation s larger healthcare conversation and initiatives. Under the guidance of Shafiq Rab M.D., Regional Senior Vice President and Co-Chief Information Officer at Hackensack Meridian Health, the medical center became one of the first to take the pledge to participate in the White House Precision Medicine Initiative. To ensure that HackensackUMC could deliver on its pledge, the organization chose the Infor Cloverleaf Integration Suite to improve patient care, eliminate the one-size-fits-all approach to healthcare, and find ways to tailor treatment and prevention strategies to each patient s unique characteristics. 6 INNOVATION STRATEGY Revolutionizing the healthcare experience HackensackUMC chose the Infor Cloverleaf Integration Suite to enhance the flow and exchange of patient information and make it more accessible and convenient for both patients and providers. With the Infor Cloverleaf Integration Suite, HackensackUMC is increasing patient satisfaction by giving time back to patients and providers, and decreasing waiting room time and anxiety for patients. The Infor team implemented the Infor Cloverleaf Integration Suite in just three weeks to meet an aggressive timeline. What excites me about the relationship with Infor is that they are people who are as passionate as I am, says Dr. Rab about HackensackUMC s relationship with Infor. Healthcare is a big challenge and Infor has that will and that capacity to fight the fight. It s great to have a partner that shares the passion, he adds. In regard to wait times and throughput often, you re losing out on the clinical care, whether it be in an ambulatory setting, an inpatient setting, or just a regular office visit. Prior to Infor Cloverleaf, doctors and nurses spent time filling out information with the patient. Today, we have information available before the patient arrives, giving care time back to the nurse and the doctor. 7 5 to 10 minutes to schedule an appointment reduced to 1 minute Streamlining doctor visits HackensackUMC continues to increase the quality of its patient care in many innovative ways. The Infor Cloverleaf Integration Suite helped the organization to reduce waiting room times and increase the amount of face time patients have with physicians. With the Infor Cloverleaf Integration Suite, HackensackUMC received access to a simple-to-use, mobile system for patients to schedule appointments and provide context for their doctors through a single health questionnaire. This efficient scheduling tool, not familiar in a healthcare setting, reduced the time it takes to schedule an appointment from 5 to 10 minutes down to 1 minute. This made appointment scheduling 5 to 10 times faster than before the tool was in place. Not only does this change greatly improve the patient experience, but it also allows a patient to manage their visit from an app, providing them with 24-hour scheduling options and value-added automation, like receiving text message alerts when earlier appointments are available. 8 RESULTS Saving time to provide better patient care Value realized With over 447,898 outpatient visits scheduled at HackensackUMC in 2015, the Infor Cloverleaf Integration Suite has already helped patients save time and it has made the appointment scheduling process 5 to 10 times faster than traditional telephone-based communication. The Infor Cloverleaf Integration Suite has reduced manual data entry for care providers and administrators. In addition, the Infor Cloverleaf Integration Suite has created a more efficient flow of information between patients and doctors by eliminating transaction costs associated with phone calls. This has resulted in more face time between patients and medical professionals, allowed for quicker and more efficient care delivery, and increased the number of patients that can be helped in a given timeframe for a given number of care workers. Improving patient care with better data With the Infor Cloverleaf Integration Suite, physicians at HackensackUMC can access more specific information about their patients, so treatments can be tailored to match the patient s condition. At a past American Association for Cancer Research (AACR) Precision Medicine Series conference, Intermountain Precision Genomics Medical Director Lincoln Nadauld, MD presented results from a study that compared the different outcomes between patients who received standard chemotherapy versus a precision medicine treatment. Patients receiving precision cancer medicine nearly doubled their survival rate (from 12 weeks to 23 weeks) with no increase in costs, according to DNAnexus. The findings from this study paint a picture of the untapped potential that exists in the future of precision medicine that can be enabled by the Infor Cloverleaf Integration Suite. We ve taken a stand with Infor as our partner to enable our environment and our EHR to help us meet the HL7 FHIR standard. Our main goal was to empower the patient so that they can take care of their personal health data. We believe that the HL7 FHIR will allow us to reach this goal faster than any other technology. HackensackUMC has made major strides toward achieving this vision of delivering precision medicine, and plans to continue to innovate to fulfill its commitment to the White House. We are an early adopter but there are other organizations that have also stepped up to the pledge.
