3 Overview 3 Today s webinar will briefly recap OneCity Health s approach to partner funding for projects included in Comprehensive Schedule B o The Comprehensive Schedule B includes eight projects, and the others are in distinct Schedules B o Each Comprehensive Schedule B has performance metrics with associated due dates and funds, if achieved o All partners should have received their Comprehensive Schedules B for signature o Contracts are due back to OneCity Health no later than July 31 We will provide an overview on next steps toward project implementation We will provide an overview of next steps for partner reporting o Available materials to assist you in meeting performance metrics o Registering for our newly established Partner Portal o Utilizing our new support desk for assistance Our hope is that by the end of the webinar, you will understand where to go for information, the reporting process, invoicing and next steps

4 Comprehensive Schedule B Webinars 4 Thank you for your participation in the Comprehensive Schedule B performance metrics and orientation webinars! There was strong turnout with diverse partner representation o Over 140 partners attended the live sessions o Since June 22nd, we ve had over 400 unique visits to the webinars on the OneCity Health website General partner questions and comments will be addressed today via the chat function. We ll stop and ask for questions when you see the following symbol: We will continue to provide ongoing support in meetings, additional webinars and through our newsletter. All resources are posted on our website as well

5 DSRIP Goal: Developing an Integrated Delivery System 5 Our goal is to care for patients in a communitybased setting inclusive of medical, behavioral, long-term care, community-based providers and social service organizations By putting patients in the center of that network, we can work together to keep them healthy Our 11 DSRIP projects align with our larger transformation efforts to ensure our patients receive high quality care in the right setting at the right time Comprehensive Schedule B begins the process of enhanced cooperation and accountability with our network, and strengthens our ability to provide coordinated, high-quality patient care CBOs Clinics Institutions Physicians Patient Home Care

6 How we Developed the Comprehensive Schedule B 6 The information requested in the April 2016 Master Partner Data Survey was utilized to produce individual Comprehensive Schedules B for each partner Each partner s Comprehensive Schedule B outlines: o The projects each partner selected o The performance metrics the partner must achieve to be eligible to earn related funds o Total eligible allocation o Payment and contract terms

8 Moving Forward with Project Implementation 8 The OneCity Health team is here to support your development within an integrated delivery system We ll schedule meetings with our partners to outline steps to move forward with the projects you have selected We ll provide a performance management framework to help monitor progress Additional support includes: o Project-specific toolkits, trainings and webinars o Clarification of expectations and long-term goals o Ongoing opportunities to review results and provide feedback o Learning collaboratives to discuss best practices

9 Next Steps for Partners 9 On August 9, we will dedicate our next webinar to project implementation support and performance management. In the meantime, our partners should: o Identify leads for performance discussions o Submit timely quarterly reports and responses to inquiries (we discuss reporting in the next section) o Commit to continuous performance improvement, which includes refining internal processes as needed and attending meetings to discuss performance

10 How and What do I Report? 10 OneCity Health has developed a variety of resources to help you with reporting, including o OneCity Health Reporting Manual o Reporting templates o Partner Portal o Support desk

11 What do I Report: Three Categories of Performance Metrics 11 Operations & Outcomes (Partner Performance) Partners will receive payments based on process or outcome goals against a set goal or benchmark (from the group or other benchmark) Data & Tracking (Partner Reporting) Partners will receive payments for reporting on key deliverables and progress towards meeting key milestones on time per protocol in terms of required content and use of template Participation (Partner Engagement) Partners will receive payments for participation in activities related to, but not limited to: PPS wide webinars and training, meetings, summits, surveys, and other opportunities as defined by OneCity Health The performance metrics that you are responsible for are included in your Comprehensive Schedule B and any distinct Schedules B. Performance metrics are also detailed in the OneCity Health Reporting Manual

12 OneCity Health Reporting Manual 12 By August 1, we will provide a OneCity Health Reporting Manual to all partners o The OneCity Health Reporting Manual will further describe all reporting requirements and help you complete all performance metrics this quarter (July 1, 2016 September 30, 2016) o We will continue to develop new versions of the OneCity Health Reporting Manual, with detail on additional performance metrics o Templates to assist you with reporting will be included in a future version

14 Where do I Report? OneCity Health Partner Portal 14 OneCity Health has launched a Partner Portal, a dedicated website that contains information unique to each partner. You will soon be able to: View your Schedules B (once submitted and approved) View your metrics View definitions and reporting expectations for each metric Access reporting templates Upload required reporting documents View and submit invoices

17 OneCity Health Partner Portal: How to Register 17 Your organization should designate a primary user responsible for reporting and invoicing The primary user will have to register with an , which is your ID, and password Your will be verified by OneCity Health Once verified, you will be granted access to the portal You will soon be able to add additional users

19 Introducing the OneCity Health Support Desk 19 OneCity Health has also launched a support desk for partners Contact us via phone and with questions and issues Our support desk will answer basic questions and/or direct you to appropriate resources All inquiries will be logged and tracked

21 How Do I Receive Payments? 21 Invoicing Process for Payment After you have completed a performance metric, first submit the report or deliverable through the Partner Portal Once a performance metric is approved for payment by OneCity Health, an automated notice will be sent to you requesting you to submit an invoice through the Partner Portal If a performance metric is not approved for payment, a OneCity Health team member will contact you If discrepancies arise regarding invoices, a resolution process will be available on the Partner Portal Each Performance Metric will have its own invoice The pre-filled invoices will be available on the Partner Portal for review and submission as the performance metrics are approved You will be able to monitor the status of all invoices in real time
