4 Introduction MoHSW recognizes the Potential of ICT for healthcare delivery transformations. National ehealth Strategy was formulated to guide such a transformations. One of the key areas targeted for transformation is the use of ICT to make health facilities more effective and efficient. Thus, the need to have an integrated Hospital Management System (ihms) to make hospital become effective and efficient.

5 Current Status There some efforts to computerize various clinical and administrative functions in some health facilities in Tanzania While there is significant improvement in management of health resources, the degree of success on automation has been varying significantly, with some obtaining poor results.

6 ihms Implementation Challenges A Feasibility Study conducted on the existing ihms revealed as number of challenges such as: Lack of standards to support data sharing and interoperability Systems catering for diverse, fragmented functionalities There is no criterion for establishing functionalities of high priority or minimum set of requirements for automation Lack of proper governance

7 Vision of the ihms Initiative Definition: ihms is a single system or collection of integrated systems that automate both clinical and administrative processes in the hospital settings. Vision of the ihms: The MoHSW envisions an integrated national Health Information System (HIS) that has, as its components, the ihms that support provision of quality healthcare by making management of hospital more efficient and effective.

8 Vision of the ihms Central to the ihms vision is the need to have systems that can: Streamline the medical / administration work flow Provide seamless integration between functions for smooth patient movement within various services Prevent loss of medicines and other medical supplies Enable patient tracking to increase productivity of inpatient allocation Ensure instant Patient Billing Provide real-time management and accounting report. Provide effective administration and control

9 The Need for a Framework to Guide ihms Implementations & Management A well acknowledged approach to address the ihms challenges is to establish a country-wide ihms implementation framework that constitute: A minimum set of requirements that the ihms must meet A minimum set of standards that the ihms system must support A set of guidelines that must be followed for successful implementation of the ihms

11 Functional Requirements Functional requirements capture the intended behavior of the ihms expressed as services, tasks or functions the system is required to perform. MoSCoW (Must, Should, Could, Won t) technique has been employed To indicate the level of priority of each system requirement The minimum requirement that must be incorporated in each ihms is indicated with a Must priority.

12 Functional Requirements The hospital functional areas covered include: 1. Patient Care Management 2. Laboratory 3. Billing 4. Pharmacy and Inventory 5. Medical Record Management 6. Human Resource Management 7. Management Information System (Dashboard) 8. Financial Management.

13 Non-Functional Requirements Non-functional requirements define the overall qualities or attributes of the resulting system They place restrictions or conditions on the system being developed, and the development process They specify external constraints that the system must meet.

14 Non-Functional Requirements The following quality attributes must be considered for the ihms system: Usability Availability Access Security Efficiency Integrity Reliability Scalability

15 ihms Standards It is required that the ihms be able to share and exchange information with other systems in the health sector. This requires the creation, acceptance and implementation of clin The document defines the minimum set of standards required for implementing ihms as the core component of the national integrated Health Information System (HIS)

16 ihms Standards The following categories of standards Data Exchange (or Messaging) Standards HL7 standard for patient level data Coding Standards International Classification of Diseases (ICD)

17 Computing Infrastructure Requirements The computing infrastructure will support the hosting of applications and provide communication and system platforms for users to access and use the system. Computing infrastructure includes the following: Data center Local Area Network Internet connectivity, Workstations for staff, and Computer training laboratory

18 Human Resources Requirements The successful ihms implementation requires right people at the right time both within and outside the hospital. The minimum recommended competencies and roles for key players in the system implementation and operational process include the following: IT System Analyst System/Network Administrator ICT support technician Data/Medical Record Clerks

19 Implementing the ihms

20 ihms Implementation Planning The implementation of the ihms requires careful planning to ensure that the implementation proceeds in comprehensive, cost-effective and timely ways. This involves a range of activities including Establishment of the governance structure, Budgeting, Development of the implementation work plan, Acquiring the ihms software, Readiness assessment and Procurement of the required computing infrastructure.

22 ihms Deployment The ihms deployment include the following activities: Installation of Computing Infrastructure Software Configuration and Installation System Acceptance Testing Training Basic computer training ihms end user training Network and System Administration Training ICT governance training for managers Trial-run (hospitals should run a limited trial-run of the ihms to allow users to test the system and their knowledge)

23 ihms Support & Maintenance Maintenance and support of the ihms operations is an essential ingredient in making its adoption successful For users to efficiently and effectively use ihms properly plan should be devised and operationalized which should include: Maintenance of the computing infrastructure and the ihms system Continuous end user training and support Disaster preparedness and recovery plan Regular assessment of system usage Modifications and enhancements of the system and IT infrastructure

24 ihms Support & Maintenance The following should be included in the plan Initial maintenance and support should be provided by the supplier of the ihms for at least six months, and should be part of the supply and implementation contract There should be at least one year maintenance & support Service Level Agreement (SLA) with the supplier of the ihms after commissioning Mechanisms for identifying changes in the hospital functions & workflows that needs to be reflected in the ihms. Modification, fixing and updating the ihms software Ensuring security of ICT infrastructure and systems and patient and other business data and information

25 M&E of the ihms Implementation As part of ihms implementation, it is necessary to track and evaluate the implementation and functioning of the system. The objectives of the M&E includes: To determine whether implementation activities have been performed as intended To determine whether the system is functioning as expected To measure the effects of the ihms implementation including: Boosting hospital revenues Improving administration and performance of hospital operations Preventing loss of medicines and other medical supplies Improvement in patient tracking
