4 Incident Management The SpheraCloud core product for Incident Management provides a comprehensive reporting and workflow design based on best practice and regulatory reporting standards. The SpheraCloud core product also provides clients the ability to capture initial incident details, classify incident types and severities, conduct investigations and root cause analysis, allocate corrective and preventive actions, and report on standard key performance indicators and regulatory reports such as OSHA (300, 300A, and 301 log reports). 5 Rich Repository of Incident Reports Tabular Incident Report - a tabular report that will return a row for each incident created. Incident Trends Report - a chart & table report that will return the number of incidents grouped by a selected time period. View Incidents by Hour of the Day - a chart & table report that will return the number of incidents grouped by the hour the incident occurred. View Incidents by Day of Week - a chart & table report that will return the number of incidents grouped by the day of the week that the incident occurred. Time Taken to Record Incidents - a chart & table report that will return the number of incidents grouped by the number of days it took to capture the incident. OSHA Logs Days Since Last LTI/MD by location - a chart & table report that will return the number of days since a lost time injury (LTI) and/or modified duties (MD) incident occurred at a location. 5

6 Assessments and Audits The SpheraCloud core product offering for Audit & Inspection Management provides a framework for data capture and workflow, based on standard Audit process design. As content and event topics within Audits & Inspections vary, SpheraCloud uses a template that comprises multiple elements of an Audit Form, so that the questions can be plugged into this process. 8 Training The SpheraCloud core product offering for Training & Competencies enables organizations to track and manage certifications, and end-to-end training requirements for employees and contractors. One key aspect of the training module includes a configurable Training Matrix, which provides a framework for assigning users, by role, group and job function, as mandatory, recommended or optional training as required by identified training categories. Admins can also utilize the course management tool, which has the capability to build and schedule training courses, manage course users and evaluate the final results and performance of attendees. 10 Management of Change The Management of Change (MOC) process is used to define, approve, execute and communicate changes within the organization. A simple MOC business process can be used for things like organizational changes, document changes, etc. For facilities that are governed by process safety management regulations, the Advanced configuration allows for the typical OSHA-regulated MOC process required for higher-risk mechanical and equipment change.
