1 Product Attributes - On-Premises 5.1 System Architecture - On-Premises Describe the overall architecture of the proposed solution including an architectural diagram of all System elements and redundancy Provide a list of the minimum hardware and software that TCPH must provide List the hardware, operating Systems, third-party software and database platforms that you support for the product Which versions of these operating Systems do you support? Which versions of these third-party software tools do you support? What is the preferred hardware and operating System platform? What is the preferred database platform? Which versions of the database platforms do you support? Describe how the system can integrate with third party enterprise content management systems such as Hyland's On Base solution Describe which virtualization products have been used with the System for database virtualization What is the preferred server? Provide technical specification for the best configuration of servers to support the requested number of user licenses Identify all network services provided by the System Describe the technical specifications for implementing secure access to the System via the Internet Describe the System's capability for allowing use of mobile devices, e.g., tablets or smart phones, for recording and viewing data Describe the System's capability for recording and viewing data on a mobile device when the mobile device is not connected to the System Describe the System's reporting environment and how ad-hoc reports can be created. 1 of 9

2 Scalability - On-Premises Describe the development and test environments you would recommend, in addition to a production environment, to allow TCPH to evaluate the performance impact of adding new workload (users, applications, and data) to the existing solution. What evidence can you provide to support this recommendation? Describe hardware and software requirements to effectively enable additional environments. 5.3 System Administration - On-Premises Identify all tools and third-party software necessary for proper administration of the System. These tools must cover management and administration of the System database, the user interface, and any auxiliary programs integrated into the System Describe the tools provided to manage the configuration, database storage and operations such as: Performance measurement and tuning Resource utilization Database archival processes Backup and recovery Configuration changes Upgrades and patches Rapid load and unload utilities Describe the tools available to monitor and tune the performance of queries in the System Describe the tools provided by your product to manage the workload on the database Describe the process and schedule for upgrading to a new release. How do you manage structural incompatibilities, if any, between releases? Describe the process for applying patches Describe the administrative staff required to manage a System that meets TCPH requirements described herein. The TCPH Selection Team will approve proprietary components prior to implementation in the System solution Describe your business continuity and disaster recovery processes to provide Continuity of Operations (COOP) including your Disaster Recovery (DR) Business Continuity Plan and your Disaster Management Business Continuity Plan Describe your policy for maintaining your operating System and application current with the most current integrated Microsoft applications. 5.4 Product Support - On-Premises Do you support multiple releases of your products? Discuss the process and timing of termination of support for older product releases. Respondents must document a history of seamless upgrades between i Discuss your product support offerings. Example requirements for remote access support include: Maintenance by remote access via VPN shall be to the Development Environment only. The Respondent shall not allow split tunneling during any VPN connection to the TCPH network or servers. 4 Training - On-Premises Include a detailed discussion of the training that will be provided to System Administration users at the time of initial installation. System Administration training must include System installation, configuration, user interface, instrument integration, data review, reporting, data backup, and maintenance, including: A description of whether classes are offered off-site (i.e., at the Respondent s location) and/or on-site (i.e., at a TCPH location.) For off-site classes: when the classes will be offered, the number of seats that will be made available to TCPH, and the total class size. For on-site classes: when the classes will be offered, the total class size allowed and what TCPH must provide Include a list of course titles, course abstracts, a description of the target audience, and typical class lengths for System Administrator training Include a detailed description of your concept for providing specifically tailored training to End Users in preparation for going into production. 5.6 Documentation - On-Premises Describe the electronic and printed documentation to be provided with the System. Documentation to be included must include the following: The System administration documentation must include a design specification detailing System functionality, as well as the design of the central database, to include entity relationship diagrams and table definitions. The user documentation must be in the form of a user manual that describes core functions of the System product. For each data entry field, the user manual must identify permitted data types, ranges of values, and relationships with other data Do you provide on-line access to a context-sensitive Help System that will provide specific information about each screen in the application? 4 of 9

5 Statement of Work - On-Premises 6.1 Project Approach - Implementation - On-Premises Describe your proposed project approach or methodology to accomplish these activities, broken down by tasks and subtasks in order to achieve a successful implementation, on time and within budget. Include as part of this discussion a project schedule indicating the major tasks (including significant milestones) necessary for an on-time, within budget, and fully functional implementation. The project plan should include the following: Tasks to be performed (within phase as applicable) Number of hours each task will require Deliverables created by each task Dates by which each task will be completed (dates should be indicated in terms of elapsed time from project inception) Resources assigned to each task Required TCPH support Task dependencies Training (if applicable) Provide the estimated number of resources (by phase or milestone) from Fill out Worksheet Tab "Resource Estimates" - Respondent Roles, On-Premises your company, their roles and the percent of allocation required to implement this project successfully while controlling costs and meeting timelines Provide resumes of potential candidates from your company who would fill these roles on the project based upon the projected start date. TCPH prefers a respondent implementation staff with functional experiences in laboratory operations and information technology tasks related to the System product being proposed. In lieu of established corporate experience, the Respondent personnel resumes must include references for previous experience in implementing System applications in a similar environment. Desired experience would include implementation in a public health laboratory. 5 of 9

Describe the technical specifications for implementing secure access to the System via the Internet Describe the technical specifications for implementing secure access to the database Describe the System's capability for allowing use of mobile devices, e.g., tablets or smart phones, for recording and viewing data. Does the System utilize responsive design for mobile device access? Describe the System's capability for recording and viewing data on a mobile device when the mobile device is not connected to the System Identify any risks or any limitations related to a SaaS implementation and mitigation plan, including but not limited to: bandwidth, server availability, multi tenancy, and number of users Describe the expected page response / refresh times and the requirements to achieve these times.
