Completed Contractor Registration Form & Payment. o Unless already registered for the current year with the Village of Buffalo Grove. Two copies of the Plat of Survey for the property noting the following: o Location and dimensions of the existing as well as proposed driveway and/or apron. The width of the existing and proposed driveway and apron measured at the public sidewalk is required to be shown on the Plat of Survey A copy of the proposal detailing the materials to be used and the thickness of the materials. Please refer to the Driveway & Driveway Apron handout for size limitations and material requirements. All information on this checklist is required at the time of permit submittal. Incomplete or partial submittals will not be accepted. CALL THE ENGINEERING DEPARTMENT AT (847) TO SCHEDULE THE PRE- LAY/PRE-POUR INSPECTION A MINIMUM 24 HOURS IN ADVANCE. CONTRACTOR IS REQUIRED TO CALL THE BUILDING DEPARTMENT AT (847) TO SCHEDULE THE FINAL INSPECTION WHEN THE WORK IS COMPLETE. If granted the permit applied for shall comply with all requirements of the Village Ordinances relating thereto and pay the fees required by such ordinances. No error or omission in either the plans or application, whether said plans or application have been approved by the Building Official or not, shall permit or relieve the applicant from constructing the work in any other manner than that provided for in the ordinance of this Village relating thereto. 3 DRIVEWAY & DRIVEWAY APRONS A permit is required to construct, replace or extend a driveway The processing time for permits is a minimum of three days A permit is required when you are replacing your driveway and/or apron. If you are only seal coating your driveway, a permit is not required. The permit application requires the name, address, and telephone number of the contractor installing the driveway/apron and the valuation of the project. Provide two (2) copies of the plat of survey noting the following information: Location and dimensions of the existing as well as proposed apron or driveway (width at garage, width at both sides of the public sidewalk, width at the street/curb and the dimensions of any expansion to the existing driveway and the length of all portions). Location of any street light, fire hydrant, B-box and trees in the parkway. Indicate distance from the above to the edge of the new apron. Village Code requires the following for these projects: Aprons One and two car garages a minimum width of ten (10) feet and a maximum of twenty-four (24) feet in width as measured at the sidewalk and a maximum of twenty-one (21) feet at the curb. Three car garages - Not to exceed twenty-seven (27) feet in width as measured at the sidewalk and three (3) feet greater at the curb. In no case shall the driveway apron width at the curb be greater than six (6) feet wider than the driveway width at the sidewalk. Materials: a) A minimum of six (6) inches concrete and five (5) inches CA-6 stone; or, b) Concrete brick pavers (ASTM C-936) with a minimum of six (6) centimeters (2.36 inches) on a granular base of eight (8) inches minimum compacted thickness with one (1) inch setting sand. A concrete base may be used and installed per the concrete paving brick manufacturers specifications. Driveways -One and two car garages - Maximum of twenty-four (24) feet in width as measured at the property line. Three car garages - Not to exceed twenty-seven (27) feet in width as measured at the property line. Materials: a) A minimum of five (5) inches concrete and five (5) inches CA-6 stone; or, b) A minimum of ten (10) inches CA-6 stone and three (3) inches Class I bituminous asphalt; or, c) Concrete brick pavers (ASTM C-936) with a minimum of six (6) centimeters (2.36 inches) on a granular base of eight (8) inches minimum compacted thickness with one (1) inch setting sand. A concrete base may be used and installed per the concrete paving brick manufacturers specifications.

4 The alteration of the existing grade is not allowed in any of the following areas: 1. In any easement. 2. Within five (5) feet of any property line. 3. Within any drainage way. Driveways, from property lines, may taper at a rate of 1:1 but the total amount of paved/impervious surfacing in the front yard may not exceed 40%. Inspections of driveways and aprons are performed by the Engineering Department prior to pouring/installing the concrete, asphalt or brick pavers once the forms are in place and the base is installed. For an inspection, call (847) twenty-four (24) hours prior to pouring concrete, asphalt or brick pavers. After the job is complete a final inspection will be conducted by the Building Department. Parking If you need to park on the street overnight when new driveway/apron is being installed please notify the Police Department at (847) Failure to do so can result in the issuance of a citation. Call J.U.L.I.E. (Joint Utility Locating Information for Excavators) forty-eight (48) hours before you dig. This is to verify location of underground utilities in the area. The service is provided to you at no charge and should be done by the person performing the work. When calling J.U.L.I.E. at , the following information about the proposed work location will be requested: county, township, section number, and quarter section. This information can be obtained from the legal description on your plat of survey. APRONS CONSTRUCTED IN BRICK PAVER Pursuant to the Buffalo Grove Municipal Code, Maintenance - In the event that a concrete brick paver apron, or portion thereof, is removed by the Village in the course of repairs or maintenance to the public street or utilities, the owner of the property shall replace said paver apron within ten days of notification by the Village that the street or utility work is complete. If said paver replacement is not performed by the owner of the property within said ten-day period, the Village may exercise its right to install a concrete apron, full or partial, pursuant to the standards of the Village Development Ordinance.
