1 ADDENDUM 1 Request for Proposals City of Minot, North Dakota Structure Demolition and Site Restoration Project, # Round 3 April 14, 2016 Proposals Due: 11:00 A.M., Tuesday, April 19, 2016, Minot City Clerk s Office, Minot City Hall, nd Avenue SW, Minot, ND This Addendum shall be part of the Contract Documents for the above referenced project. Acknowledge receipt of this Addendum by inserting its number and date in the space provided on page 2 of Section Bid Form, and also on the outside of the bid envelope submittal as described in the Bid Advertisement. Failure to do so may subject the bidder to disqualification. In reference to the above Request for Proposals, please note the following changes or clarifications: 1. The following is the discussion and questions that were asked during the mandatory Pre-bid Meeting held on April 12, Each question is provided with a response. During the meeting, it was reiterated by CDM Smith that bid documents could be picked up at CDM Smith s office at the Arrowhead Mall. It was stated that the contract award date is expected to be mid-may. The winning bidder will be expected to begin as soon as possible thereafter. The contract end date is October 15, but could be extended if warranted by the number of properties added. Twenty properties are currently ready to be assigned for demo, and as more properties are acquired they will be assigned to the demo contractor. It was explained that the Lincoln Place Apartment site has not yet been acquired; however, it is expected to be acquired in time to be included in this demo program. The Lincoln Place site includes 2 stories with basement, 15 units, garage, and parking lot. Contractors will provide one price for the demolition of the entire property on line item on the bid form. The contractor prices for line items on the bid form will be used, if applicable, at this property. It was pointed out that there is one line item, 00006B, on the bid form for RACM demolition price category. No other prices will need to be submitted in Category B. It was stated that the City deems these sites and contents to have no salvage value. It was also pointed out that the City may elect to have non-profits perform selective salvage prior to assigning the property to the demolition contractor.

2 Addendum No. 1 City of Minot Structure Demolition and Site Restoration Project Round 3 Page 2 Additionally, it was explained that all properties are required to have the sewer and water disconnected and removed. If the City requires that the disconnect(s) take place at the main, additional line items No and/or No would apply. If the City does not require that the disconnect(s) take place at the main, the disconnect(s) would take place at or near the property line and would be included in the line item price provided for the demolition (No through No ). It was pointed out that gas and electric should be disconnected prior to the property being assigned to the demo contractor; however, it is the contractor s contractual responsibility to verify prior to starting demo. A site visit to Odd s Trailer Park located at th Ave SW was conducted after the pre-bid meeting, for the Pre-Bid meeting attendees to have an opportunity to see this property which consists of approximately 36 mobile home sites. There are currently 6 mobile homes remaining on the property, which will be moved prior to demo. It was pointed out that hydro-seeding at Odd s is a requirement and should be included in the bid price for this property. It was stated that the guard rail along the road would not be removed by the demo contractor. The sign-in sheet from the Pre-Bid Meeting is attached to this addendum. Pre-Bid Meeting Questions/Answers Q1: What do we do if asbestos wrap is found on the gas line to be removed? A1: It would be handled per Section Asbestos Removal of the bid documents. Q2: Will a separate demo permit be required per demo? A2: Please refer to Section General Conditions Part 6 of the bid documents for permit requirements. Q3: Will the Right-of-Way excavation permit and the Demo permit each be priced separately? A3: Please refer to Section General Conditions Part 6 of the bid documents for permit requirements. Q4: What is the cost for disposal? A4: Please refer to Section Measurement and Payment, Part 1.02B, and Section Demolition and Debris Removal, Transportation, and Disposal, Part 1.09B of the bid documents. It is the contractor s responsibility to obtain pricing from the landfill. Q5: Will the borrow be measured compacted or loose? A5: Trucks hauling borrow will be measured to determine cubic yardage capacity and loads will be graded at the site prior to dumping. Q6: Do we take out or leave the trees?

Q7: Do the basement floors have to be removed as well. A7: Yes 2. The following are questions that were received outside the Pre-bid Meeting held on April 12, Each question is provided with a response. Post Pre-Bid Meeting Questions/Answers Q1: Will the street damages due to the main disconnect be included in the price or will road damages be repaired by the city? A1: Please refer to Section Measurement and Payment Section of the bid documents, bid items number and Q2: Will 1 ½ crusher run be sufficient for granular borrow? A2: Please refer to Section 1800 Excavation and Embankment of the bid documents. Q3: What size and type of main is the water/sewer (i.e. clay, copper, cast, pvc)? A3: It varies throughout the city; however, it shall be noted that changes in size or materials will not constitute a change in price. Q4: Do erosion blankets have to be placed on all areas before hydro seeding or only sloping/high risk erosion areas? A4: Please refer to Section 3700 Lawns and Grasses in the bid documents. Q5: Are compaction tests only performed on road beds or all back fills? A5: The city may elect to conduct compaction testing at some or all areas where compaction is required. Q6: How many main disconnects will be performed? A6: This will be determined by the city on property by property basis. Q7: Are sewer and water mains located in the center of the streets or alley ways also? A7: It varies throughout the city; however, it shall be noted that variances in the locations of the disconnects will not constitute a change in price, regardless of the repairs required. Q8: This requires a payment and performance bond. Are they each 5% I assume? For a total of 10% being bonded? A8: Please refer to Section Information to Bidders, Part 2.15 of the bid documents. Q9: Is all concrete required to go to the landfill or can it be disposed of on private property?

Q10: What are the minimum requirements for contractor for removing the asbestos that has already identified? Licensing/certification/supervision? A10: Please refer to Section Asbestos Removal of the bid documents. Q11: Do all contractors have to be licensed and hold permits prior to bidding or prior to start working? A11: Please refer to Section Asbestos Removal of the bid documents. 3. Additional Clarifications/Additions to Bid Documents: All trucks hauling debris must be fully tarped while in motion. In Section Measurement and Payment, on Page 23, add the following: BID ITEM NO Steel Plate Cover Installation The purpose of this line item is to cover an open basement staircase which has a measurement opening of 3 x 9. Contractor to install 1/4 steel plate (ASTM A588 Cor-Ten B) over the opening. Plate shall extend over the edge of the opening by 6 on 3 sides of the opening. On the fourth side measuring 9 feet, angle iron must be secured to the wall to provide a ledge for the steel plate to rest on. Secure steel plate to concrete surface with ½ x 3 zinc plated steel wedge anchors. Anchors shall be placed a minimum of 3 from edge of opening. Spacing between anchors shall be a maximum of 12. A photo of the open basement staircase is attached. A. Measurement Measurement shall be on a per each basis for the installation of a steel plate cover. B. Payment Payment shall be made at the unit price bid per each steel plate cover installed accordance with these Contract Documents contained herein. This price and payment shall be full compensation for all mobilization, material, labor, equipment, and any other related or incidental items required to complete this item of work for which separate payment is not provided for under other items on the Bid Form. In Section Measurement and Payment, under Bid Item No , on Page 21, add the following bullet to the list of incidental items: Hydro-seed entire site with proper mix as outlined in Section 3700 Lawns and Grasses.
