1 6 IMPACT NOTIFICATION PLAN #57 The following areas will be affected the greatest: ACC WP Level 2 & 3 Emergency Department SPECIAL NOTES: - Click here to watch our progress via the webcam. SCHEDULED SHUTDOWNS: - None Scheduled SPECIAL INSPECTIONS: - Thru-penetration & partition firestopping. - In-Wall inspections prior to wall close up. KEY MILESTONES: o Mobilize work in South Corridor (ACC WP3) 3/17/18 o Level 6 Miller Bldg Drywall Completion 5/11/18 General Construction Activities EMERGENCY DEPARTMENT, WP2 & WP3 Due to a water leak that occurred on 3/3/18, there will be some restoration efforts ongoing throughout the south side of ED, (Triage, Registration, Waiting, etc) and directly above the ED on WP2 and WP3. Restoration work will entail drywall patch and paint, ceiling replacement and flooring/baseboard replacement. MILLER BUILDING CONSTRUCTION SITE Construction vehicles will be using the old ED entrance driveway and ED access will utilize the new site road. A dedicated gatekeeper will be present M-F 7 AM to 4 PM to assist vehicles, EMS staff and pedestrians at this intersection. Deliveries for the south side of the project will be utilizing the ED access road and entering into the south end of construction site. Two-way radio communication will be maintained between the gatekeepers so that access priority can be granted to ED traffic. Interiors of Miller Building will have multiple operations ongoing such as in-wall MEP rough-in, priority wall tops, firestopping, elevator installation, electrical conductors and in-wall inspections prior to drywall installation. Exterior envelope is weathertight, final finishes and cladding is being installed. Roofing installation is complete on the main building, except perimeter prep and coping installation.

2 IMPACT NOTIFICATION PLAN #57 Elevator installation is wrapping up in the south hoistways. North elevator 5&6 have arrived onsite and installation will begin. RENOVATION ACC WP: West Pavilion 2 no work scheduled West Pavilion 3 Containment will be setup in South WP ACC Corridor 3C5E for demolition and corridor improvements on 3/17/18. SEE BELOW FOR MORE DETAIL ON THE ABOVE-MENTIONED ACTIVITIES The following terms will be used in this Impact Notification to convey the level at which noise and vibrations will be occurring due to work being performed. This is an approximation and cannot predict exactly what tools, work, or unexpected conditions may create more or less disturbance than what is listed on this schedule. Slight/Minor: The use of hammers, screw guns, small power tools, non-impacting large equipment such as trucks, man lifts, etc. Moderate: Work involving hammer drills, coring drills, sawcutting, impact hammers, large equipment such as concrete pumps/trucks, excavators, bulldozers, plate compactors, etc. Heavy: Vibratory compactors & rollers, dump trucks, demolition work, pile drilling/driving, cranes, jack hammers, etc. Depending on the proximity to these activities, major vibrations and noise will be present. Skybridge: o Detailed Construction Activity Expected Aerial lifts will be in operation at the north end of the site for Skybridge east wall exterior framing and sheathing, curtainwall installation and prep for masonry. Moderate / intermittent noise can be expected when lifts are in use and framing activities are ongoing. Miller Building: o MEP Rough in, homeruns and major systems will be installed. domestic water, above ceiling HVAC piping; ductwork and medical gas work is ongoing. o Drywall installation is ongoing, South to North on each floor. o Interior stairwell close-in work will continue on South stair Level 7 working topdown; all vertical circulation should use North stair when stairwell work is ongoing. o Temporary heating & weather protection measures, including steam fired heaters, temporary exterior enclosures, etc. will be active throughout the building. o Scaffolding (swing staging) will be in used at the east elevation portion of the Miller Building to allow for metal panel operations. o Curtainwall snap caps and trim details will continue on the West elevation. o Metal panel preparation including z-girts and insulation will continue on the east elevation. This will be followed by metal panel and louver installation. o Roofing will continue Level 6 roof and Skybridge roof including roof blocking, insulation and membrane installation. o Skybridge curtainwall is ongoing. o Masonry will continue on the North elevation starting with mast climber scaffold installation, insulation and clips. o North Elevator installation will continue with building platforms and connecting motors o Patient Room Headwall med gas and power connections will continue. o Patient lift supports will continue at Level 5 and 6.

3 IMPACT NOTIFICATION PLAN #57 ACC West Pavilion Level 2 Renovations o Billing Office 2455 to be completed and turned over. ACC West Pavilion Level 3 Renovations o Demo and reconstruction of corridor 3C5E (corridor at the exterior wall of ACC WP3, from Surgery Waiting to PACU/Pre-Op) will begin 3/17/18. This phase of work will involve multiple smaller phases to renovate the new corridor that will connect the ACC building to the Miller Building. Moderate / intermittent noise can be expected during the setup of phasing and temporary provision activities. Traffic in the ACC WP3 South corridor will be redirected to account for this phasing requirement.
