9 GENERAL NOTES DE- 1. THE EXISTING OLD CITY HALL AND SITE RETAINING WALLS SHALL BE DE-CONSTRUCTED IN ACCORDANCE WITH DRAWINGS AND IN A MANNER SO AS NOT TO DAMAGE THE ADJACENT SWIMMING POOL, STREETS, SIDEWALKS AND SPRING BOIL. ALL RESULTING MATERIAL AND DEBRIS SHALL BE MOVED TO A LEGAL DUMP SITE AT CONTROR'S EXPENSE. 2. DE-CONSTRUCT IN AN ORDERLY AND CAREFUL MANNER WITH MINIMAL VIBRATION. PROTECT EXISTING AND TREES FROM DAMAGE. TREES AND BUSHES TO BE REMOVED ARE NOTED ON CIVIL DEMOLITION DRAWING. SAWCUT CONCRETE TO BE REMOVED PRIOR TO DE-. 3. PROVIDE TEMPORARY SHORING AND BRACING AS NECESSARY SO AS TO CREATE A SAFE WORKING ENVIRONMENT. 4. CONTROR SHALL DESIGN AND INSTALL A LINE OF CANTILEVERED STEEL SHEET PILINGS ALONG THE NORTH SIDE OF THE BUILDING AS SHOWN ON BUILDING SECTIONS. THE SHEET PILING SYSTEM SHALL BE DESIGNED BY A FLORIDA REGISTERED PROFESSIONAL ENGINEER. THE SYSTEM SHALL BE REMOVABLE AND SHALL REMAIN IN PLACE TO ACCOMMODATE FUTURE. 5. REMOVE SOIL THAT OCCURS BEHIND THE EXISTING MASONRY NORTH WALL IN ACCORDANCE WITH DRAWINGS PRIOR TO REMOVING THE WALL AND ANY OF THE UPPER LEVEL FLOOR FRAMING, LOWER LEVEL WALL FRAMING AND FOUNDATION. 6. MAINTAIN THE WORK AREA AS A TIDY AND SAFE ENVIRONMENT BY DISPOSING MATERIALS ON A REGULAR BASIS. 7. ALL EQUIPMENT AND DEBRIS SHALL REMAIN IN A DESIGNATED WORK ZONE THAT IS ACCEPTABLE TO CITY. ASBESTOS ROOFING 1. FIRST PRIORITY IS TO TOTALLY REMOVE EXISTING ASBESTOS ROOFING FROM THE OLD CITY HALL IN ACCORDANCE WITH ACCEPTABLE ENVIRONMENTAL RULES AND ORDINANCES. 2. PROTECT EXISTING POOL, SPRING BOIL AND DISCHARGE STREAM FROM CONTAMINATION. 3. ASBESTOS ROOFING AND RELATED MATERIAL SHALL BE MOVED TO A LEGAL DUMP SITE AT CONTROR'S EXPENSE. SALVAGE 1. THE TWO SAFES THAT OCCUR ON THE UPPER LEVEL SHALL BE CAREFULLY REMOVED AND TRANSPORTED TO THE CITY'S DEPARTMENT OF PUBLIC WORKS. IF SAFES ARE NOT REMOVED WITH A LIFTING APPARATUS, CONTROR SHALL SHORE UPPER LEVEL FLOOR AS NESCESSARY TO ALLOW SAFE REMOVAL OF THE SAFES. 2. CONTOR SHALL DISCONNECT AND REMOVE THE FAIRBANKS-MORSE WATER PUMP FROM THE CONCRETE PEDESTAL THAT OCCURS ON THE LOWER LEVEL AND SHALL DELIVER THE PUMP TO THE CITY'S DEPARTMENT OF PUBLIC WORKS. REMOVAL OF SUB-SURFACE OBSTACLES 1. THE OVER-ALL DE- SITE SHALL BE CLEARED OF ANY SURFACE AND SUB-SURFACE OBSTACLES, INCLUDING BUT NOT LIMITED TO EXISTING FOUNDATIONS, UTILITY LINES, BASEMENTS, SLABS AND MISCELLANEOUS RUBBLE. 2. THE EXISTING LOWER LEVEL CONCRETE SLAB-ON-GRADE IS BADLY CRACKED WITH NOTICEABLE VOIDS UNDER THE EQUIPMENT ROOM. A REVIEW OF UNIVERSITY OF FLORIDA SANBORN FIRE INSURANCE PLANS DATING BACK TO 1884 SHOWS THAT TWO DIFFERENT POOL AND BUILDING CONFIGURATIONS WERE PRESENT NEAR AND UNDER THE OLD CITY HALL BUILDING. IT IS SUSPECTED THAT ONE ABANDONED POOL SLAB REMAINS, AND ITS OUTLINE IS SHOWN ON THE FOUNDATION PLAN. 3. AUGER BORINGS MADE UNDER THE OLD CITY HALL IN 1986 BY CH2M HILL ENCOUNTERED HIGHLY VARIABLE FILL CONTAINING RUBBLE ALONG WITH BURIED CONCRETE SLABS AND EXTENSIVE VOIDS. THE DEPTH OF THE UNDESIRABLE FILL AND VOIDS IS ESTIMATED TO BE EIGHT FEET AT THE ABANDONED POOL SLAB AND FIVE FEET ELSEWHERE. THE PRESENCE OF THIS FILL SUGGESTS THAT THE OLD POOLS AND SUPPORT BUILDINGS WERE PROBABLY MINIMALLY DEMOLISHED WITH MUCH OF THE OLD POOLS AND RELATED FOUNDATIONS LEFT IN THE GROUND AND COVERED WITH LOOSE FILL AND RUBBLE. 4. IT IS THE INTENT OF THIS PROJECT TO REMOVE THE ABANDONED POOL SLABS AND RELATED BUILDING FOUNDATIONS ALONG WITH ANY LOOSE FILL AND RUBBLE. FOR THIS REMOVAL, CONTROR SHALL ALLOW FOR AN EIGHT FOOT DEEP OVER-EXCAVATION FOR REMOVAL OF ABANDONED POOL SLABS IN THE AREA SHOWN ON THE BUILDING SECTION ALONG THE LENGTH SHOWN ON FOUNDATION PLAN. SIMILARLY, CONTROR SHALL ALLOW FOR A FIVE FOOT DEEP OVER-EXCAVATION FOR RUBBLE REMOVAL IN THE AREA SHOWN ON THE BUILDING SECTION ALONG THE LENGTH SHOWN ON FOUNDATION PLAN. THE VOLUME CREATED BY THIS OVER-EXCAVATION SHALL BE BACK-FILLED WITH CLEAN ENGINEERED FILL AS SPECIFIED HEREIN. 5. THE EXTENT OF ABANDONED POOL SLAB TO BE REMOVED IS SHOWN ON THE FOUNDATION PLAN. CONTROR SHALL NOTIFY ENGINEER-OF-RECORD WHEN TOP OF SLAB IS READY FOR INSPECTION. AT THAT TIME, THE SLAB WILL BE EVALUATED TO DETERMINE IF ADDITIONAL PORTIONS OF THE SLAB NEED TO BE REMOVED. 6. CONTROR SHALL NOTIFY ENGINEER-OF-RECORD OF THE EXTENT OF UNDESIRABLE FILL AND VOIDS AS EXCAVATION PROGRESSES. THE FINAL DEPTH AND EXTENT OF OVER-EXCAVATION AND BACKFILL SHALL BE DETERMINED BY THE ENGINEER-OF-RECORD. ENGINEERED FILL 1. AFTER EXCAVATION TO MINIMUM DEPTHS AND OVER-EXCAVATION AT ABANDONED POOL SLAB AND RUBBLE AREAS, THE EXISTING EXPOSED SANDY SOILS SHALL BE SCARIFIED TO A DEPTH OF 12 INCHES, MOISTURE CONDITIONED, AND COMPED TO A MINIMUM OF 95% RELATIVE COMPION USING A LIGHT-WEIGHT WALK BEHIND VIBRATOR DRUM ROLLER (1,500 TO 2,500 POUNDS) OR A PLATE TAMPER. A MINIMUM OF EIGHT COMPLETE COVERAGES, FOUR IN EACH DIRECTION, SHALL BE MADE WITH THE ROLLER OR TAMPER TO IMPROVE DENSITY AND UNIFORMITY OF UNDERLYING LOOSE SANDY SOILS. 2. ENGINEERED FILL SHALL EXTEND FULL DEPTH FROM BOTTOM OF ALL EXCAVATIONS TO THE LEVEL OF (N) FINISHED GRADE AS SHOWN ON BUILDING SECTIONS AND CIVIL DRAWINGS. 3. ENGINEERED FILL SHALL BE PLACED IN 12 INCH HORIZONTAL LOOSE LIFTS THAT ARE COMPED TO 95% MODIFIED PROCTOR MAXIMUM DRY DENSITY. 4. PRIOR TO START OF COMPION OPERATIONS, REPRESENTATIVE SAMPLES OF THE ENGINEERED FILL MATERIAL AND EXPOSED IN-PLACE FINE SANDY SOILS SHALL BE COLLECTED AND TESTED BY THE SOILS ENGINEEER TO DETERMINE THEIR COMPION AND CLASSIFICATION CHARERISTICS. TESTING BY SOILS ENGINEER WILL DETERMINE IF ENGINEERED FILL MATERIAL IS ACCEPTABLE. 5. A REPRESENTATIVE NUMBER OF FIELD IN-PLACE DENSITY TESTS SHALL BE PERFORMED BY THE SOILS ENGINEER IN THE COMPED EXISTING SOILS AND IN EACH ONE-FOOT LIFT OF ENGINEERED FILL TO ENSURE COMPLIANCE WITH SPECIFIED DEGREE OF COMPION. AT EACH ONE-FOOT LIFT, AT LEAST TWO DENSITY TESTS SHALL BE PERFORMED AT OVER-EXCAVATION FILL FOR POOL SLAB REMOVAL; AT LEAST TWO TESTS AT OVER-EXCAVATION FILL FOR RUBBLE REMOVAL; AND AT LEAST THREE TESTS AT ALL OTHER FILL AREAS. 6. ENGINEERED BACKFILL IS DEFINED AS NON-PLASTIC, INORGANIC, GRANULAR SOIL CONTAINING LESS THAN 10 PERCENT MATERIAL PASSING THE NO. 200 MESH SIEVE, THAT IS, A RELATIVELY CLEAN SAND. PLASTICITY INDEX SHALL BE LESS THAN 4. TEMPORARY GROUNDWATER CONTROL 1. ACCORDING TO RECENT SOIL BORINGS, GROUNDWATER LEVELS ON THE SITE OCCUR AT DEPTHS VARYING FROM 3.3 TO 9.0 FEET BELOW EXISTING SITE GRADES. 2. A TEMPORARY DE-WATERING SYSTEM SHALL BE INSTALLED ON BOTH SIDES OF THE EXISTING POOL ALONG ITS ENTIRE LENGTH. AS NECESSARY, THE GROUNDWATER SHALL BE CONTROLLED AT ALL TIMES TO A DEPTH OF AT LEAST NINE FEET BELOW EXISTING SITE GRADES, ONE FOOT BELOW ANY EXCAVATION LEVEL AND THREE FEET BELOW THE LEVEL OF ANY VIBRATORY COMPION OPERATIONS, WHICHEVER IS GREATER. 3. SHOULD THE NEAR-SURFACE SOILS OR ENGINEERED FILL EXPERIENCE "PUMPING" AND SUBSEQUENT LOSS OF SOIL STRENGTH DURING EARTHWORK OPERATIONS, WORK UPON THESE AREAS SHOULD BE IMMEDIATELY TERMINATED UNTIL THE EXCESS PORE PRESSURE WITHIN THE DISTURBED SOILS HAS DISSIPATED BEFORE CONTINUING COMPION OPERATIONS. SCOPE OF SITE DEMOLITION 1. COMPLETE 2. DEMOLITION OF DESIGNATED SITE STRUCTURES, RETAINING WALLS, FENCES, AND FOUNDATIONS 3. DEMOLITION AND REMOVAL OF PAVEMENTS, CURBS AND GUTTERS, DRAINAGE; 4. REMOVAL OF STRUCTURES, DRAINAGE PIPE, UTILITIES, SITE SIGNS, LANDSCAPING, AND DESIGNATED TREES 5. DISCONNECTING AND CAPPING OR REMOVAL OF IDENTIFIED UTILITIES 6. REMOVAL OF ABANDONED FOUNDATIONS AND PIPING 7. FILLING VOIDS IN SUBGRADE CREATED AS A RESULT OF REMOVALS OR DEMOLITION 8. DISPOSAL OR SALVAGE OF DEMOLISHED MATERIALS. EXISTING UTILITIES 1. PRIOR TO START OF DE-, THE LOCATIONS OF ANY UNDERGROUND UTILITY LINES WITHIN THE WORK SITE SHALL BE ESTABLISHED AND INTERFERING LINES SHALL BE REMOVED OR RELOCATED IN ACCORDANCE WITH DRAWINGS. 2. ALL EXCAVATIONS FOR THE REMOVAL OR RELOCATION OF UTILITIES SHALL BE BACKFILLED WITH ENGINEERED FILL TO SPECIFIED FINISH GRADES. REGULATORY REQUIREMENTS 1. CONFORM TO APPLICABLE LOCAL CODE FOR DEMOLITION OF STRUCTURES, SAFETY OF ADJACENT STREETS, BUILDINGS AND STRUCTURES; DUST CONTROL AND RUNOFF CONTROL. 2. OBTAIN REQUIRED PERMITS AND LICENSES FROM AUTHORITIES HAVING JURISDICTION. PAY ASSOCIATED FEES INCLUDING DISPOSAL CHARGES. THE CITY WILL PAY THE COST OF THE DEMOLITION PERMIT DIRECTLY; CONTROR REMAINS RESPONSIBLE FOR OBTAINING THIS PERMIT. 3. NOTIFY AFFECTED UTILITY COMPANIES BEFORE STARTING WORK AND COMPLY WITH UTILITY COMPANY REQUIREMENTS. 4. DO NOT CLOSE OR OBSTRUCT ROADWAYS, SIDEWALKS OR FIRE HYDRANTS WITHOUT PERMITS. 5. BARRICADE AND MARK HAZARDS AS NECESSARY. 6. CONFORM TO APPLICABLE REGULATORY PROCEDURES WHEN DISCOVERING HAZARDOUS OR CONTAMINATED MATERIALS. NOTIFY CITY IMMEDIATELY UPON DISCOVERY OF HAZARDOUS OR CONTAMINATED MATERIALS. DO NOT COMMENCE REMOVALS, REMEDIATION, OR ABATEMENT WITHOUT AUTHORIZATION FROM CITY. EXISTING CONDITIONS 1. STRUCTURES INDICATED FOR DE- DEMOLITION WILL BE DISCONTINUED IN USE AND VACATED PRIOR TO START OF WORK. 2. CITY ASSUMES NO RESPONSIBILITY FOR CONDITION OF STRUCTURES TO BE DE-CONSTRUCTED. 3. CONDITIONS EXISTING AT TIME OF INSPECTION FOR BID PURPOSES WILL BE MAINTAINED BY CITY. 4. UNLESS OTHERWISE INDICATED IN THE CONTR DRAWINGS OR SPECIFIED BY CITY, REMOVE ITEMS OF SALVAGEABLE VALUE TO CONTROR FROM PROJECT SITE. STORAGE OR SALE OF REMOVED ITEMS ON PROJECT SITE NOT PERMITTED. 5. BURNING OR FIRES OF ANY NATURE NOT PERMITTED. SITE EXAMINATION 1. VERIFY EXISTING CONDITIONS BEFORE STARTING WORK. 2. VERIFY THAT FIELD MEASUREMENTS, SURFACES, SUBSTRATES, AND CONDITIONS ARE AS REQUIRED, AND READY TO RECEIVE WORK. 3. LOCATE AND MARK EXISTING UNDERGROUND UTILITIES. 4. REPORT IN WRITING TO CITY PREVAILING CONDITIONS THAT WILL ADVERSELY AFFECT SATISFORY EXECUTION OF WORK. DO NOT PROCEED WITH WORK UNTIL UNSATISFORY CONDITIONS HAVE BEEN CORRECTED. 5. BY BEGINNING WORK, CONTROR ACCEPTS CONDITIONS AND ASSUMES RESPONSIBILITY FOR CORRECTING UNSUITABLE CONDITIONS ENCOUNTERED AT NO ADDITIONAL COST TO CITY. SITE PREPARATION 1. PROVIDE, ERECT, AND MAINTAIN EROSION CONTROL DEVICES, DUST CONTROL MEASURES, TEMPORARY BARRIERS, AND SECURITY DEVICES AT LOCATIONS NOTED ON DRAWINGS. 2. PROTECT APPURTENANCES, TREES AND STRUCTURES WHICH ARE NOT INDICATED TO BE REMOVED OR DEMOLISHED. REPAIR DAMAGE CAUSED BY DEMOLITION OPERATIONS AT NO ADDITIONAL COST TO CITY. 3. PREVENT MOVEMENT OR SETTLEMENT OF ADJACENT STRUCTURES TO INCLUDE BUT NOT LIMITED TO BUILDINGS, WALNUT STREET, SIDEWALKS, EXISTING POOL AND SPRING BOIL. PROVIDE BRACING AND SHORING AS REQUIRED. 4. MARK LOCATION OF UTILITIES. PROTECT AND MAINTAIN, IN SAFE AND OPERABLE CONDITION, UTILITIES TO REMAIN. PROVIDE TEMPORARY SERVICES DURING INTERRUPTIONS TO EXISTING UTILITIES ACCEPTABLE TO CITY. 5. PROVIDE A FLOATING TURBIDITY WALL COMPLETELY ACROSS THE DISCHARGE STREAM AT THE BY-PASS OUTFALL BOX AS SHOWN ON CIVIL DRAWINGS. 6. INSTALL A SILT FENCE ON BOTH SIDES OF THE DISCHARGE STREAM EXTENDING FROM THE POOL DISCHARGE WEIR TO AT LEAST FIFTY FEET DOWN THE STREAM AS SHOWN ON CIVIL DRAWINGS. DE- REQUIREMENTS 1. CONDUCT DE- TO MINIMIZE INTERFERENCE WITH ADJACENT STRUCTURES AND PAVEMENTS. 2. STOP OPERATIONS AND NOTIFY CITY IMMEDIATELY IF ADJACENT STRUCTURES APPEAR TO BE IN DANGER. VERIFY STRUCTURES ARE UNOCCUPIED, THEN DE-CONSTRUCT STRUCTURE COMPLETELY AND REMOVE MATERIALS FROM SITE USING METHODS AS REQUIRED TO COMPLETE WORK WITHIN LIMITATIONS OF GOVERNING REGULATIONS. 3. PROCEED WITH DE- IN SYSTEMATIC MANNER, FROM TOP OF STRUCTURE TO GROUND. 4. REMOVE ASBESTOS ROOFING AS DEFINED HEREIN. 5. SALVAGE TWO SAFES AND ONE PUMP AS OUTLINED HEREIN. 6. REMOVE UPPER STORY FRAMING AS OUTLINED HEREIN. 7. REMOVE SOIL THAT IS RETAINED BEHIND THE BUILDING'S NORTH WALL AS SHOWN ON DRAWINGS PRIOR TO REMOVING ANY OF THE LOWER STORY ELEMENTS. 8. REMOVE LOWER STORY ELEMENTS AS SUMMARIZED HEREIN. 9. LOCATE DE- EQUIPMENT AND DUMPSTERS AND REMOVE MATERIALS USING PROCEDURES SO AS TO PREVENT EXCESSIVE LOADING TO ADJACENT STREETS,SUPPORTING WALLS, FLOORS, OR FRAMING. 10. INSTALL DE-WATERING SYSTEM AS DEFINED HEREIN. 11. DE-CONSTRUCT AND REMOVE ALL EXISTING FOUNDATIONS AND SLABS-ON-GRADE. 12. IN ADDITION TO OVER-EXCAVATION FOR SUB-SURFACE OBSTACLES AS DEFINED HEREIN, OVER-EXCAVATE UNDER THE ENTIRE EXISTING SLAB-ON-GRADE TO A MINIMUM DEPTH OF TWO FEET BELOW TOP OF SLAB. 13. REMOVE RUBBLE LOCATED WITHIN A DEPTH OF APPROXIMATELY FIVE FEET BELOW PROPOSED SUBGRADE. 14. BREAK-UP BURIED ABANDONED CONCRETE POOL SLAB THAT IS LOCATED APPROXIMATELY EIGHT FEET BELOW PROPOSED SUBGRADE. REMOVE CONCRETE AND MASONRY IN SMALL SECTIONS. 15. THE EXISTING POOL SHALL BE EMPTIED BY THE CITY TO ITS LOWEST WATER LEVEL, WITHOUT SUPPLEMENTAL PUMPING, PRIOR TO START OF OVER-EXCAVATION FOR REMOVAL OF ABANDONED POOL SLAB. CONTROR SHALL COORDINATE THIS IVITY WITH CITY. POOL SHALL BE EMPTIED WHEN EXCAVATION FOR REMOVAL OF POOL SLAB BEGINS, AND SHALL BE RE-FILLED WHEN ENGINEERED FILL HAS BEEN COMPLETED IN THE OVER-EXCAVATION. CONTROR SHALL MINIMIZE THE TIME, TWO WEEKS MAXIMUM, WHEN POOL IS EMPTY. 16. PLACE COMPED ENGINEERED FILL IN ACCORDANCE WITH DRAWINGS. 17. CONTROR SHALL SUBMIT A DE- SCHEDULE TO CITY FOR ACCEPTANCE PRIOR TO START OF DE-. SCHEDULE SHALL INCLUDE BI-WEEKLY MEETINGS TO BE ATTENDED BY CONTROR, CITY AND ENGINEERING REPRESENTATIVES. 18.
