1 AGENDA PRE-CONSTRUCTION MEETING Peaceable Hill Water District Water Supply & Treatment General Construction /Plumbing / Electrical Contracts Date: 10 AM Location: Town Offices, 1 Main Street, Brewster, NY Meeting Purpose: To establish lines of communications, review administrative processes and discuss prerequisites to start of construction. Contract Times will commence to run on September 11, B. Substantial completion is 12 months from date indicated in Notice to Proceed C. Final completion is 14 months from date indicated in Notice to Proceed III. Coordination A. There are three prime contracts for this project and it is anticipated that there will be several subcontractors. Cooperation and coordination is required to ensure timely completion and minimize site disruption. B. Each Prime Contractor is responsible for coordinating, administering, directing and scheduling all of its work and the work of its subcontractors. C. It is strongly recommended that all three primes work together to complete the project on-time. Work must be coordinated to the best extent practical and all activities are interrelated. D. Each Prime Contractor is to have a responsible person on-site while any of their subcontractors are working to facilitate coordination and communication. IV. Correspondence A. Direct all written correspondences and submittals to Engineer B. Where would you prefer Project-related correspondence be directed, and to whom? Page 2

3 V. Personnel/Staff On-Site A. Identification of Site Superintendent/Supervisor B. List of Personnel/Staff on-site with contact info C. Provide proof of OSHA-10 Training (contractors + subcontractors) VI. Project Schedule A. Each Contractor must submit a preliminary project schedule for review and approval. Each Contractor schedule may depend on Work completed by others. B. A revised project schedule is to be provided with each payment request. VII. Work Hours A. Normal construction hours: 7:00AM to 6:00 PM Monday through Friday B. A revised project schedule is to be provided with each payment request. VIII. Construction Layout A. General Contractor shall be responsible for laying out the Work B. Work shall be laid out by a NYS licensed land surveyor C. Owner shall provide AutoCad electronic files and appropriate field survey benchmarks to enable land surveyor to lay out the Work. IX. Subsurface Exploration/Geotech report A. Refer to Spec Section for Subsurface Exploration. B. Test borings & soil sampling for water treatment building and water storage tank C. Geotech report signed and stamped by NYS PE. X. Photographic Documentation XI. Submittals A. Document existing site conditions prior to construction B. Document construction progress A. Refer to Spec Section for submittal procedures. B. Provide a schedule of submittals per Paragraph 2.03 of the General Conditions. C. Provide submittal cover page with each submittal. Number submittals sequentially. Each submittal must have the Chazen Job Number and specification section or drawing number and detail reference. D. Each Submittal is to have a box for the Engineer s action on the cover, or if in a binder, the first page. Page 3

4 XII. Permits E. Make submittals complete prior to making the submission. This will expedite reviews. Clearly mark all sections of the submittal and provide tables of contents and divider pages for large submittals. Also identify deviations from the contract documents. F. Allow 15 days for initial review of each submittal by Engineer. Additional time may be needed to coordinate submittal reviews. G. Submit related items together to allow for a coordinated review. H. Identify spare parts within submittals. I. Identify long lead items. Submit these items early to avoid potential delay in delivery. J. Submittals should be made in advance of the work. Only approved items may be incorporated into the work. A. General Contractor is to apply for and obtain County Highway Work permit and Town Building permit. B. Contractor responsible for all application fees, bonds, security deposits C. Town will waive building permit application and inspection fees. XIII. Special Inspections A. Refer to Spec Section for Special Inspections and Procedures. B. General Contractor to engage qualified Special Inspector for this project who will provide and coordinate inspection and testing requirements per the NYS Building Code. C. Refer to the Schedule of Special Inspections and specifications for required inspections and testing (re-bar, concrete, masonry). D. Submit Special Inspector name and qualifications. XIV. Payment Application A. Refer to Spec Section for Payment Procedures. B. Prepare Application for Payment on form EJCDC C-620 or AIA 702/703 C. Submit two (2) originally signed sets and three (3) copies. D. Include with each payment request: 1. Certified prevailing wage payroll 2. updated project schedule 3. waiver s of mechanic s lien(s) E. Progress Payments will be made NOT MORE THAN once per month. For ease, only submit applications for payment once per month. F. Pencil Copies should be discussed with the Engineer of its field representative. Pencil copies should include all of the required attachments. Page 4

5 G. Pencil copies are due by for work completed last month. Final copies should be submitted by for consideration for payment by the Town. H. Incomplete requests for payment will not be reviewed until they are deemed complete. I. The items completed or percentage complete as identified within the Schedule of Values submitted and approved shall be the basis for payment. J. Retainage equivalent of 5% of the value of the Work will be held on each application for payment. XV. Change Orders A. Refer to Spec Section for Contract Modification Procedures. B. Each Contract sum includes a set allowance amount to cover construction contingencies. C. Cost based on receipt of an estimate from the Contractor for additional Work ordered by the Owner/Engineer. D. For all change orders, a price for the work must be submitted in advance of the work being completed so that it can be approved. E. Detailed breakdown of all labor and materials, including quotes from vendors and suppliers will be required. XVI. Clarifications/Requests for Info (RFI) A. To the best extent possible, answers to questions or clarifications of the plans and design will be provided in the field or verbally. We would like to work toward resolution prior to issues arising. If agreement on resolution cannot be achieved in the field or verbally, then proceed with formal submittal of RFI s in writing. B. Questions should be asked in advance of the work so as not to cause an unnecessary delay. XVII. Construction Progress Meetings XVIII. Staging Areas A. Monthly on an as-needed basis B. Representative(s) from all 3 contractors should be present A. Town will permit workers to park their vehicles along Spruce Road. B. No storage of machinery, equipment or materials will be allowed on Spruce road. C. Contractor to secure suitable on-site or off-site staging areas for storage of equipment, machinery and materials. 6 XIX. Temporary Facilities A. Sanitary Facilities provided by General Contractor B. Temporary light and power provided by Electrical Contractor XX. Disposal of Waste Materials A. Each Contractor is responsible for the disposal of waste and construction debris. B. Each Contractor is also responsible for obtaining all necessary permits for construction debris disposal. C. Hazardous or Special Disposal waste requiring Permits should be reported to the Engineer. XXI. Erosion & Sediment Control (E&SC) A. Erosion and Sediment control measures shall be placed prior to any land disturbance including removal of vegetation, stripping of top soil. B. General Contractor responsible for the protection, maintenance and repair of E&SC measures until full stabilization of disturbed surfaces. C. There are no SWPPP for this project. XXII. Safety & Security A. Each Contractor is responsible for complying with all applicable Federal and State safety requirements. B. Emergency Contact information is to be provided with phone numbers that can be reached during non-business or working hours. Include personnel that can make executive decisions on the spot. C. Each contractor must designate an on-site safety representative before starting work. D. It is the responsibility of each Contractor to provide proper protection and to ensure safety of all personnel on-site including subcontractors and suppliers, in accordance with the requirements of Industrial Code Rule 23, and OSHA regulations. E. Enforcement shall be the responsibility of each Contractor. F. Each Contractor is responsible for the safety and security of the site. G. First Aid/Emergency Plan/Directions to nearest Hospital or Clinic XXIII. Project Record Documents A. Maintain on-site 1 set of Contract Documents to record actual revisions to the Work. B. Record information/changes as Work progresses C. Submit marked-up set plus 2 copies to Engineer XXIV. Questions/Miscellaneous Items to be discussed Page 6
