1 THE CORPORATION OF TAY VALLEY TOWNSHIP REQUEST FOR PROPOSAL ENGINEERING SERVICES TO REHABILITATE TWO (2) BRIDGES AND RECONSTRUCT ONE (1) STRUCTURAL CULVERT CONTRACT #218-PW-7 ADDENDUM NO. 2 THIS ADDENDUM SHALL BE INCORPORATED INTO THE RFP PACKAGE AND SHALL FORM A PART OF THE CONTRACT DOCUMENTS Date Issued: June 4, 218 Issued By: Amanda Mabo, Clerk Please note the following changes, corrections, additions, deletions, information and/or instructions in connection with the RFP Package. Failure to acknowledge receipt of this Addendum as per Part A Information to Bidders may render your submission nonresponsive. This addendum is to address the following question(s) and clarification(s): Question 1: Would you confirm the location of the culvert to be replaced in 218? Answer 1: The Culvert is 8 metres east of Menzies Munro Side Road and 7 metres west of Noonan Side Road. The location noted in the RFP and in the OSIM report is incorrect. See attached map. Question 2: Could the OSIM data associated with the three sites be made available? Answer 2: See attached 216 inspections for the three sites. Question 3: Please clarify hours to be included for the Contract Administration and Site Inspection services during construction. Please confirm that the 2 hours per week per site (8 week duration for each bridge and 3 week duration for the culvert) is only for site inspection services and that additional hours are to be included to cover additional tasks listed for Construction. Question 4: Please clarify if we are to include materials testing costs during construction. Answer 4: The Consultant will be required to obtain three quotes for material testing however the Township can be directly invoiced for the testing services and therefore testing costs should not be included in the submission. Question 5: Please confirm construction staging assumptions for completion of the work. i.e. will full road closures be permitted with implementation of a detour Answer 5: It is assumed that Noonan Side Road Bridge and Upper Scotch Line Culvert may be full road closures and Glen Tay Side Road Bridge will have to be staged construction.
