1 MATTHEW G BURTON ARCHITECT LLC December 12, 2014 PC# THE COLLEGE OF WILLIAM AND MARY BRAXTON HOUSE DEMOLITION MBA No. 160 ADDENDUM #1 This addendum supplements, modifies, changes, deletes from, or adds to the original contract documents, dated 12/01/14, for the Braxton House Demolition at the College of William and Mary, and is herein made part of the contract documents. Corresponding revisions to the drawings and specifications for the items indicated in this addendum have been clouded and marked with delta 1 on the contract documents. This addendum consists of nine (9) pages, including attachments. Project Manual: Bid Form: 1. Replace the Bid Form with the attached version in its entirety. Specifications: 2. Delete Section and add Section in its entirety. Appendix A Scope of Work Documents: 3. Revise note 7 on sheet A3 to read as follows: Existing sewer connection to building to be abandoned in place. Cut existing pipe at back of sidewalk and install city cleanout with cast iron cover to match City of Williamsburg standards. Stub out pipe from cleanout and cap for future connection. 2 Responses to Questions Submitted On Or Before December 10, 2014: The following submitted questions and their corresponding answers are included in this addendum and are made part of the contract documents. 1. The mold that is present in the house- will the College have this tested and abated? Mold testing and abatement will not be performed prior to demolition activities. Bidders are advised to take appropriate worker-safety measures for any personnel that may be working inside the existing structure during the demolition process. 2. The electrical at the back of the house which is being used for another house, will the College disconnect/terminate? The temporary electric service attached to the existing house will be removed by the College prior to demolition. 3. The construction fencing around the perimeter of the house- how close can we put this fence to the property on the left? Will it be required to have fabric installed? Locate construction fencing minimum ten feet from existing structures to remain. Chain link fencing shall be in accordance with CWM Special Provisions listed in Appendix Y of the Project Manual. 4. The shrubs to the left of the house, can these be removed (and not replaced) so the machinery can use this area to set up? The shrubs to the left of the house may be removed are not required to be replaced. 5. The lead test for the house is from 2006, we were informed, however; they are to be no more than 3 years old. Will the College provide an updated Hazardous Material Report? The College will conduct a TCLP test on the debris to determine if it qualifies as hazardous waste. Bidders are instructed to provide unit pricing as a bid alternate for the quantity of debris that will be required to be disposed of as hazardous waste instead of regular waste. 3 DGS (Rev. 10/10) Standard Bid Form Format 1 BID FORM (Revised 12/12/14) DATE: PROJECT: Braxton House Demolition The College of William & Mary Williamsburg, Virginia Project Code: To: Commonwealth of Virginia The College of William & Mary Facilities Planning, Design, and Construction Grigsby Drive, Room 119 Suite C Williamsburg, VA In compliance with and subject to your Invitation for Bids and the documents therein specified, all of which are incorporated herein by reference, the undersigned bidder proposes to furnish all labor, equipment, and materials and perform all work necessary for construction of this project, in accordance with the Plans and Specifications dated December 01, 2014, and the Addenda noted below, as prepared by Matthew G Burton Architect LLC, Warwick Blvd. #212 Newport News, VA for the consideration of the following amount: BASE BID (including the following parts but excluding work in Additive Bid Items): Lump sum price for demolition of the building and site restoration complete and in accordance with the Plans and Specifications: TOTAL BASE BID AMOUNT IS: DOLLARS ($ ) ADDITIVE BID ITEM 1: The Owner will perform a TLCP test on demolition debris to determine if the debris qualifies as hazardous waste. Any debris determined to be hazardous waste shall be disposed of as such in an approved landfill. Additive Bid Item 1 shall include the difference in cost between hazardous waste disposal on a unit price basis (price per cubic yard) and standard waste disposal which is accounted for in the base bid. Final amount shall be adjusted upward or downward based on actual quantity authorized/determined to be hazardous waste. Estimated quantity of (300) $ per cy = DOLLARS ($ ) Contract award will be based on the Total Base Bid Amount shown above (including any properly submitted bid modifications) plus as many Additive Bid Items taken in sequence as the Owner in its discretion decides to award. 4 The bidder has relied upon the following public historical climatological records: NOAA, National Climate Center Report for Richmond International Airport, Richmond, VA. The undersigned understands that time is of the essence and agrees that the date for Substantial Completion of the entire project shall be on or before Februrary 13, 2015 based on a Notice authorizing Work to proceed on or before January 19, 2015, and Final Completion shall be achieved within 30 consecutive calendar days after the date of Substantial Completion as determined by the A/E. Acknowledgment is made of receipt of the following Addenda:. If notice of acceptance of this bid is given to the undersigned within 30 days after the date of opening of bids, or any time thereafter before this bid is withdrawn, the undersigned will execute and deliver a contract in the prescribed form (Commonwealth of Virginia Contract Between College and Contractor, Form CO-9) within 10 days after the contract has been presented to him for signature. The required payment and performance bonds, on the forms prescribed, shall be delivered to the College along with the signed Contract. Section Includes: seeding, mulching, and fertilizer at areas disturbed by construction. 1.2 QUALITY ASSURANCE A. Provide seed mixture in containers showing percentage of seed mix, year of production, net weight, date of packaging, and location of packaging. 1.3 DELIVERY, STORAGE AND HANDLING A. Deliver grass seed mixture in sealed containers. Seed in damaged packaging is not acceptable. B. Deliver fertilizer in waterproof bags showing weight, chemical analysis, and name of manufacturer. PART 2 PRODUCTS 2.1 MATERIALS A. Seed: mixture of 50% annual rye and 50% K31 Fescue. 2.2 ACCESSORIES A. Mulch: Dry wheat straw, free of growth or germination inhibiting ingredients. B. Fertilizer: type recommended for seed. C. Water: Clean, fresh and free of substances or matter which could inhibit vigorous growth of grass. PART 3 EXECUTION 3.1 APPLICATION A. Fertilizing: 1. Apply fertilizer in accordance with manufacturer's instructions. 2. Apply after smooth raking of topsoil and prior to roller compaction. 3. Do not apply fertilizer at same time or with same machine as will be used to apply seed. 4. Mix thoroughly into upper 2 inches of topsoil. 5. Lightly water to aid the dissipation of fertilizer. B. Seeding: 1. Apply seed with a drop spreader at a final rate of 3 to 5 pounds per 1,000 square feet. 2. Apply evenly in two intersecting directions. 3. Do not seed area in excess of that which can be mulched on same day. 4. Immediately following or concurrent with seeding, apply mulch to supply an average of 50% shade coverage.
