2 Page 1 of 4 RFI QUESTIONS AND ANSWERS RFQ for D/B Contractor, MD Anderson League City Project No. FPDC RFQ No /ME Addendum 3, October 26, 2015 RFI No. 1 Question: Do team resumes count towards the overall page count of 75 pages? Answer: Yes. RFI No. 2 (3 Questions) Question 1: In Section 2.5 Project Planning Schedule it lists December 1, 2015 as Owner Approves Facility Program. This date is before the event January 27, 2106 Selected Respondent returns executed agreement to Owner. In Section 2.4 Facility Program it states that the Owner intends to have the successful respondent to be part of the final development, verification and approval of the Facility Program as a part of Pre-Construction Services. Please confirm how successful respondent would be involved in final development of Facility Program if it is being finalized prior to selection of awarding the project. Answer 1: Owner is currently working on the program verification for multiple sites. Verified programing information will be provided as it becomes available. December 1st is the target date for this information. Question 2: Attachment B Conceptual Site Plan, shows the site location and footprint of the building. Attachment A General Location Plan, shows an overall site. In the Pre-Submittal Conference it was stated that the scope of work for this project would include extending South Drive to Walker Street. Please confirm this scope as well that this would be included in the GMP.

3 Page 2 of 4 Answer 2: The extension of South Drive will be part of the scope for the GMP. Additional information will be provided for the RFP phase of the competitive bid process. Question 3: Please advise if a current Soils Report is available. Would Design/Build Team be responsible or would Owner be responsible for Soils Report if current is not available? Answer 3: The soils report is currently not available. Owner will provide prior to the RFP phase of the competitive bid process. RFI No. 3 Question: Will MD Anderson release a separate RFQ for Geotechnical Engineering services or will those services be included in the architect s team response? Answer: Owner will contract directly for Geotechnical Engineering services. RFI No. 4 Question: In section of the Draft Agreement Between the Owner and Design/Build Contractor and Exhibits (Attachment A), a list of services are provided. Does a consultant need to be identified for each listed service? Example does an Elevator/Conveying System Consultant need to be designated and included as a proposed team member? Will they also be required to be included in section of the RFQ? Answer: Yes, the defined services in will require these vendors to be listed and the requirements of will apply. RFI No Questions Question 1: Specification section item 1.5.1, please confirm the intention for the "contractor to employ and pay for services of an independent testing agency" for all materials testing. Will all material testing be supplied by the contractor? Can a listing of which independent testing agencies will be supplied by the Owner be issued?

4 Page 3 of 4 Answer 1: Owner will contract with a third party materials testing agency. Question 2: Specification section item states for the contractor to provide an individual who will act as the commissioning coordinator but cannot be the project manager or superintendent. Is MDA requiring a dedicated person for Commissioning on this project? Answer 2: Yes, a dedicated commissioning coordinator will be required for all commissioning activities. The commissioning coordinator will not be required to be committed to the project 100% of the time, but they will be required for 100% of commissioning related activities for this project. Question 3: Specification section item allows for the Contractor to subcontract its commissioning obligations. Please clarify if commissioning and its associated costs are a general condition or a cost of work expense. Answer 3: Cost of work expense. Question 4: Is there a preferred Revit Platform to be used for the Design & Construction Models? Answer 4: Version 2014 of REVIT MEP or higher. Question 5: What level of BIM Model will be required at the end of the project? Answer 5: MD Anderson is to receive an "as-built" record model containing all the information indicated in the RFQ, and here within. As various firms tend to use their own definitions of "model level", MD Anderson is not specifying a "level of model development" as defined by the AIA. The model is a part of the overall deliverable package as described in the RFQ and the Owners Design Guidelines. MD Anderson primary focus is to receive a DATA RICH model that contains all the information and model components necessary for the long term maintenance and future renovations of the facility. To this end, every technical connection, such as steel connections, gypboard layout, screw patterns, etc. do not need to be modeled for MD Andersons use. The physical properties of construction types and methods, finishes, equipment, systems and their corresponding specifications and documentation as defined in the RFQ and here within should be contained within the model(s) as well as accurate modeling of the physical components themselves.

5 Page 4 of 4 Question 6: Given that the new facility will connect to the existing building, what will UTMB s involvement be on this project? Will design approvals be required from UTMB staff? Answer 6: UTMB will be involved in the project design review process and will have a Project Manager designated as a liaison for the campus and the project team to help coordinate on site activities with the campus. Question 7: The schedule does not indicate a date after Design development to go to the UT Board for approval. Will this by required? Answer 7: Yes. Question 8: The DB agreement indicates Surveyor services as part of basic services. Please confirm if this is the intent. Answer 8: Yes. Question 9: The DB agreement call for the lighting consultant and acoustical consultant to be part of the basic services. Please confirm if this is the intent. Answer 9: Yes. Question 10: Please confirm if a traffic consultant is part of the basic services. Answer 10: Yes.
