1 HOUSING AUTHORITY OF THE COUNTY OF SAN JOAQUIN INVITATION FOR BID (IFB) NO : SIERRA VISTA GYMNASIUM RENOVATION DESIGN-BID Bid Addendum No. 1 November 25, 2015 The following information is being provided as a supplement to IFB package and Scope of Work: 1. Additional information to be added to the HACSJ Scope of Work document as follow: 5.1 Gymnasium Floor: Remove, dispose and replace all boards with new throughout gymnasium Sand and then apply sealant to the floor Paint striping and/or game lines Finish coats per standard practices Paint interior walls to match concept of community room attached to the west from the gymnasium. Black ceiling and light color walls. Black paint shall extend approximately two (2) feet down from ceiling to create a decorative border. 5.6 Kitchen Remodel: Note: All of the information below is based on conceptual drawings (B.5 Conceptual Kitchen Remodel) Remove and dispose of all existing plumbing fixtures and appurtenances; remove and dispose of all electrical lighting; remove and provide back to Housing Authority of the County of San Joaquin (Authority) all appliances; or dispose of if not wanted by the Authority. Demolish and dispose of wall separating kitchen and storage areas Asbestos Removal of VCT Flooring and mastic in kitchen and storage areas Concrete demolition and removal for drainage and plumbing relocation Provide and install new plumbing per conceptual drawings Provide and install new electrical required by codes Provide and install new 12 X 12 VCT Flooring w/ Base Cove Provide and install new kitchen fixtures Provide and install kitchen appliances from the provided list or equivalent: Freestanding Range [30 in. 5.0 cu. ft. Additional Information: The HACSJ Form of Bid has been revised. Please use the new form when submitting the bid (HACSJ Revised Form of Bid). 3. Additional Information: The prevailing wage lock-in date is 12/9/2015. The Davis-Bacon Wage Determination in effect on that date will be applicable to this project. As of the date of this document (11/25/2015), the current wage rate determination is: Davis-Bacon: (CA29, Building, Mod 25 dated 11/6/15) Page 2 of 5

3 4. Additional Information: A list of Contractors who attended the non-mandatory Pre-Bid Conference is attached to this Bid Addendum as document IFB Pre-Bid Conference Attendees.pdf. 5. Answers to the provided Questions: a. Question: Does the Contractor need to have a set of Plans? Answer: Yes, this is a Design-Bid project and the City of Stockton requires them before building permit(s) can be obtained. b. Question: Are asbestos and lead abatements required? Answer: Yes, please refer to the documents below: B.3 National Analytical Laboratories, Inc. (NAL) Asbestos and Lead Building Inspection/Survey B.4 San Joaquin Valley Unified Air Pollution Control District (SJVUAPCD) Asbestos Notification Form Note: Additional Kitchen and storage area testing results will be provided to the successful Contractor. c. Question: Are the windows being replaced? Answer: Yes, replace with fixed vinyl, double-pane windows. d. Question: Does the windows need to be protected from the inside? Answer: No, the Authority is not considering it at this time; however, the Authority will accept suggestions from the Contractor. e. Question: Will the security bars be removed from the windows? Answer: No, but they can be removed to install new windows and then reinstall them back. f. Question: Does the roof need to be replaced? Answer: No, but the four (4) large vents shall be removed and sealed from exterior venting. Roof shall be patched as needed. Page 3 of 5

4 g. Question: Do the lighting need to be upgraded? Answer: Yes, the new lighting need to meet City of Stockton and Title 24 requirements. The Authority wants to replace all the electrical lighting with energy efficient LED fixtures to be sized per illumination requirements. h. Question: How does the Contractor handle the conduits on the walls in the Meeting Room? Answer: The conduits will be re-routed to be above the new doors or to avoid obstructions. This shall be similarly applied to the Kitchen Remodel. i. Question: What are the sizes of the new doors for the Kitchen and Meeting Room? Answer: They will be 36 inches minimum. j. Question: Will there be on-site storage area for the Contractor? Answer: Yes, arrangement can be made with the Authority staff for site location. k. Question: How does the Contractor handle the exposed phone lines on the walls in the Gymnasium? Answer: The Contractor will run conduit to cover the phone lines.
