1 COLUMBIA COUNTY PHASE 2 NEW FACILITIES ADMINISTRATION BUILDING AND HEALTH AND HUMAN SERVICES BUILDING Columbia County Portage, Wisconsin January 11, 2016 ADDENDUM 02 RFI RESPONSES, DOCUMENT REVISIONS, AND CONTRACT MANUAL REVISIONS This addendum forms a part of the contract documents and modifies the original documents dated December 18, The attached revisions are to be incorporated into the bid price. Acknowledge receipt of this addendum in the space provided on the Bid Form. Failure to do so may subject the Bidder to disqualification. The bid date shall be revised as follows: Due Date: Thursday January 28, 2016, 12:00 p.m. Location: Columbia County Clerk s Office Attn: Susan M. January 28, Refer to the Contract Manual Advertisement for Bids for submission requirements. This Addendum consists of two pages and the attached documents. RFI s will be accepted through Thursday January 21, The period for substitution requests has expired. Substitution Requests are no longer being accepted.

2 The following is included in this Addendum: RFI responses to questions received by the A/E through January 11, 2016 at 12:00 noon. Revised Bid Instructions and Bid Forms for Bid Packages 2, 9, 12, and 18. Additional Bid Form identifying Direct Owner Purchase Material and Equipment to be completed by each Bidder Record of Pre-Bid Conference held January 7, 2016 Architect / Engineer Addendum 02, with attachments. (Note that Addendum 01 was issued in December, affecting Document Package 1 - Demolition. Revisions from Addendum 01 were incorporated into Document Package 2)

Special attention was directed to the Contract Manual Exhibits including the Bid Bond Requirements, Wage Rates Schedule, Bid Delivery time and location, and Bid Opening time and location.

5 Question: The bid form requests individual pricing for each building. Asphalt paving is shown only on the Sitework drawings included in Volume 1 Administration Building. Response: Bidders shall include all costs for Sitework in the Administration Building. Question: The Administration Building and the Health and Human Services Building are served by a connecter. Should costs be split between the two buildings? Response: The connector is shown in Volume 1 Administration Building. Bidders shall include the cost for the connector in the Administration Building. Question: How long can RFI s be submitted? Answer: RFI s will be allowed through Thursday January 14, 2015 Question: Who shall be responsible for temporary climate control in the buildings prior to the operation of the permanent HVAC system? Answer: The HVAC contractor as detailed in Bid Package 17, Instructions to Bidders, item d. page 5. Question: Are structural steel fabricators and installers required to be AISC certified? Answer: Per Bid Package 3 Instructions to Bidders, Steel Section, item aa, page 7, contractors shall be AISC certified or have a minimum of 5 years experience with projects of similar size.

Please confirm if the HS100/Plan note "790.33'=100" should actually state "790.33'=91.33'". Note 2 Confirmed. Addressed in Addendum /5/2016 Plan A002 (Admin Bldg.) clearly shows Fireproofing limits although it is unclear if the 3rd floor framing should also be sprayed can you confirm? 3 1/5/2016 At HHS plan sheet HA002 does not show fireproofing. Can you confirm the limits of fireproofing on the HHS building? 4 1/5/2016 See specification , do we also need exposed fireproofing and where? Looking at detail 5/A002 it seems that part of room 026 future space maybe exposed based on the reflected ceiling plans. A002 No third floor framing fireproofing required. Extent of fireproofing as shown on A002 is correct. HA002 The HHS building is a non-separated mixed use building. No fireproofing between occupancies is required. 5/A002 Yes, some of the fireproofing will be exposed. Exposed fireproofing occurs at the exposed ceilings in the basement within the hatched areas as shown on A002. Electrical room 132 and Janitors closet 133 on the first floor will also have exposed fireproofing. 5 1/8/2016 Please advise if the 16 Projection (Spec Page 3 line 19) refers to the edge of the dock and not the bumpers themselves. Also, please advise what is intended by listing integral molded dock bumpers twice Correct. Duplication revised in Addendum /8/2016 Please advise what is intended for the restraint mounting plate due to no restraint being specified. (assumes steel frame is by BP3- Structure) 10 1/8/2016 Exhibit E Project Schedule Bid Package 13 is shown to be responsible for the furnish and install of the cement board Cement Board Siding to be furnished and installed by Bid Package 13. siding but the project schedule lists it as Rough Carp. in the Text2 column. Please clarify. 11 1/8/2016 Exhibit F Subcontract Paragraph 2.12 of the Subcontract Terms and Conditions states the Subcontractor assumes the same obligations to Findorff as Findorff has to the Owner under the Principal Contract with respect to the Subcontract Work. Please provide copy of Principal Contract terms and conditions for review. Available from the CM upon request. 12 1/8/2016 Spec Page 3 of the spec indicates that the edges of the cabinets, shelves, doors and drawers are to be selfedge Plastic Laminate. A supplier wants to make sure this is really what the owner wants. Attached is a letter that they generally send to the architect and construction companies that are spec ing self-edge. Please confirm or clarify intent. 13 1/8/2016 Sheets A604 & HA604 Details 1 & 2 among others call for spot grout at all anchors of door frames in steel stud framed walls. Please clarify. 14 1/11/2016 We received another question regarding the (NIC) Not in contract note on A100. I believe these items (#26 and #27) on the schedule on A100 are items that will be moved from an existing facility. Can you please confirm? Self-edge is correct where square edges are shown on plastic laminate counter tops in the drawings. Post-formed edges are also present in the project.
