Draft Construction Period Management Plan Project Location: 826 N Kings Road, West Hollywood, CA Date: March 10, 2015 Construction in an urban environment like the City of West Hollywood, presents challenges that requ ire a well thought-out construction period impact management plan be submitted and approved. This plan will guide the construction of the project in a manner that will not threaten the health, safety, or general welfare of the public. This document shall be submitted to the Building Official, City Engineer and Director of Community Development for approval. 2 tem B-On-site Construction Manager Project Sign Superintendent Martin Torres Coastal Pacific Construction LLC Cell Phone: A sign shall be provided on the construction site with minimum dimensions of four feet by four feet (4 '-0" x 4'-0"), with lettering no less than two inches (2") in height on a contrasting background, visible and readable from the public right-of-way stating the following : 1. Job site address 2. Name and phone number of project owner or owner's representative 3. Name and phone number of the general contractor and or on-site superintendent (both if not the same person) 4. Construction Hours: Monday through Friday 8:00 A.M. to 7:00 PM Saturday interior work only 8:00 A.M. to 7:00PM No construction activities on Sundays or holidays unless an approved " Extended Hours Construction Permit" has been obtained. 5. No equipment staging or start-up, material deliveries, or personnel arrivals before 8:00 AM Monday through Saturday. 6. To report violations to the City of West Hollywood call the Code Compliance Complaint Line at (323) tem C - Demolition Coastal Pacific Construction LLC will engage the services of AMPCO Contracting, nc., a licensed, OSHA-certified demolition company for all demolition services. The process for the project demolition includes: 1. Notify the surrounding neighbors of the impending demolition. 2. Send a salvage crew (comprised of manual labor) to sort and remove items for salvage. 3. Mobilie bob cats and a 45,000 lb. excavator to commence with the deconstruction of the existing building roof, walls, foundations, etc. 4. During the course of demolition, hand watering will be used to prevent fugitive dust from leaving the project site; 5. Once demolished, material will be segregated into solid waste and combustibles (prepared and mulched prior to haul). 6. Demolition related debris will be placed into roll-off bins provided by AMPCO Contracting, nc. (a City of West Hollywood approved hauler). Clean dirt can be removed by truck if the city has received the trucking firm's appropriate insurance documents and added the city as additional insured. Disposal site for dirt is to be noted in the Construction and Demolition Recycling Plan as an Exhibit to this report and is to be submitted for approval to the Building Official, City Engineer and the Director of Community Development prior to the issuance of the Building Permit. 7. Demolition and trucking will operate only within the allowable working hours or from 8:00 am to 3:30 pm, Monday through Friday. 8. The entire operation is expected to last four weeks. 3 tem D-Waste Hauling Coastal Pacific Construction LLC will employ the services of Crown Disposal or Waste Management; both are approved waste haulers of the City of West Hollywood. All construction re lated debris will be placed into roll-offs from the aforementioned approved companies. Additionally, the Construction Period Disposal and Recycling Plan will be submitted for approval to the Bu ilding Official, City Engineer and the Director of Community Development prior to the issuance of the Bu ilding Permit and made available as an Exhibit to this report. tem E-Crane Usage Coastal Pacific Construction LLC does not intend to erect a crane for any part of construction but rather will employ the use of boom lifts for daily hoisting of concrete forms, wood framing and other heavy materials and equipment. The lifts will be located entirely within the property lines away from pedestrians, traffic, or overhead power lines. Boom lifts are better suited for the maneuvering/lifting of materials and equipment than are cranes for the construction requirements of this project. Refer to the photographs below for examples of boom lifts that would be suited for this project: tem F - Usage of Public Right of Way Coastal Pacific Construction LLC intends to obtain an encroachment permit if any portion of any street, alley, or public right-of-way is used for any construction related activities. Coastal Pacific Construction LLC expects to obtain an encroachment permit for the placement of a field office (8'x16') in front of the subject property during the construction of the subterranean garage (6 months). Once that is complete it will be moved onto the property, within the perimeterfencing. Coastal Pacific Construction LLC will obtain a separate encroachment permit for the placement of a temporary restroom and wash station in front of 826 North Kings Road from property line to property line. 4 The waste hauler approved to perform services in the City of West Hollywood will obtain its own Encroachment permit for the parking of a dumpster on the public street fronting 826 North Kings Road for 20 days. Coastal Pacific Construction LLC will obtain a street closure encroachment permit, covering the concrete pour for the subterranean garage and foundation. We will hire a barricade company as our subcontractor to assist with the flagmen while the concrete trucks are moving through the property's frontage. The concrete subcontractors will be responsible for obtaining the proper encroachment permits for each concrete pour. The pouring of concrete is to occur over a period of 1 month, at 6-8 hours per day over this period. Coastal Pacific Construction LLC will hire a landscaping subcontractor, who will install plant material and irrigation on the subject property, adjacent to the public sidewalk. Materials will be stacked all within the jobsite and workers may be on the sidewalk during installation. The jobsite foreman will make sure that the sidewalk is kept clean and passable to pedestrians at all times. Sidewalk will be broom swept at the end of each working day. tem G - Pile-driving Operations No pile driving operations will be implemented on this project. tem H-Tiebacks No tiebacks will be constructed on the project. tem -Drainage and Storm WaterBMPs Effective erosion control measures include the use of silt fencing and sand bags around the perimeter of the project, including rumble plates at the entrance of the project for heavy trucks and equipment. Further efforts include covering exposed piles of soil, debris and construction materials with plastic sheeting in the event of inclement weather. No materials will be stored or placed in the direction of or near city storm drains. Drains shall be protected with City-approved sediment screens and maintained on a regular basis. Any nearby storm drain catch basin to the project site will be protected at catch basin inlet with filter fabric and sandbags, and maintained throughout the construction of this project. The Drainage plan and Best Management Practices plan, indicating locations of sandbags, silt fencing and rumble plates, will be submitted for approval to the Building Official, City Engineer and the Director of Community Development prior to the issuance of the Building Permit and will be an Exhibit to this report. No fueling of vehicles will be allowed at the project site, but rather at a designated area offsite. Coastal Pacific Construction LLC plans to post educational materials and conduct training to all subcontractors (upon commencement of work) about the causes of storm water pollution and steps necessary to prevent exposure from this project. The streets and sidewalks in front of and adjacent to the site shall be kept free of any trash, litter, and debris from the construction site and shall be cleaned and swept at the end of each day to ensure that no dirt, pa per, and any construction debris are allowed to enter the storm drain system. ll. l 1, s California cence #478584

5 tem J - Dewatering Based on review of the soils report, it is not anticipated that Dewatering will be necessary on this project. tem K-ProjectHauling Deliveries will be coordinated on an as-needed basis. Thus, deliveries may or may not occur on a daily basis. Coastal Pacific Construction LLC employs the use of flagmen for coordination during material delivery and unloading. Parking of trucks will be limited to that allowed by the encroachment permit, or what is available of the property frontage on North Kings Road from property line to property line. Trucks will proceed north on La Cienega Blvd., then right onto Santa Monica Blvd, and right onto N. Kings Road to the project site at 826 North Kings Road. Trucks will leave the site southbound on Kings Road, then right onto Melrose Avenue and proceed out of the City of West Hollywood. Refer to Exhibit A for Haul Route map. We will not stage any construction vehicles in the City of West Hollywood without first obtaining the required permits and approvals. tem L-HelicopterHauling No helicopter hauling will be used on this project. tem M-After-Hours Construction No extended hour work permits are planned or proposed. For any reason should there be a need for construction beyond normal working hours, we understand that an extended hours permit is required to be obtained from the City of West Hollywood prior to any extended work hours to be done. The work hoursare: Monday through Friday 8:00 A.M. to 7:00 PM Saturday interior work only 8:00 A.M. to 7:00PM No construction activities on Sundays or holidays No work will be allowed on the following City holidays: New Year's Day, Martin Luther King Day, President's Day, Memorial Day, ndependence Day, Labor Day, Veteran's Day, Thanksgiving Day and the day after Thanksgiving, and Christmas Day, without an extended hours permit. tem N - Construction Noise Reduction All employee conferences, subcontractor meetings and other group discussions will tak place within the construction trailer. Equipment must be operated with all back up alarms as per Cal/OSHA. Therefore, construction equipment will only be used between the hours of 8:00am and?pm. Coastal Pacific Construction LLC will remind workers on a regular basis of the need for conducting business without excessive noise. We will also work within working hours, in accordance with all City permits. Workers that arrive to work prior to 8:00 AM shall not congregate along North Kings Road, or have access to the job site until the workday begins. 6 until all windows and doors have been installed on the proposed project. No deliveries, unloading of materials/equipment, or the cleaning/emptying of the portable toilets will occur prior to 8:00a m without first obtaining the required extended hours construction permit. Motors and engines for construction related vehicles and equipment shall not be left idling and shall be turned off when not in use. tem 0 - Security Measures Coastal Pacific Construction LLC will use chain link construction fencing with locks, so as to prohibit entrance to the jobsite to any unauthoried personnel. None of the footings for the temporary fencing shall be placed in the public right of way. Additionally, depending on the stage of construction, we will employ additional deterrents to passersby. During excavation and shoring operations we will erect orange safety fencing or delineators with caution tape to alert people of open excavations or fall haards. We will also have guardrails erected at the perimeter of the shoring work as per Cal/ OSHA requirements. Any construction fence barricade surrounding the project will be maintained in a safe and aesthetically pleasing manner and shall be kept free of posters, graffiti, artwork, or similar decoration unless it is first approved by the Planning Division. All unauthoried postings/ graffiti shall be immediately removed or painted over. A current Letter of Agency shall be kept on file with the West Hollywood Sheriff's Department during all phases of demolition and construction. tem P-Drainage The Drainage plan is to be submitted for approval to the Building Official, City Engineer and the Director of Community Development prior to the issuance of the Building Permit and will be an Exhibit to this report. tem Q-Parking Coastal Pacific Construction LLC intends to utilie the portion of the street immediately in front of the project site for job site parking and materials delivery. Coastal Pacific Construction LLC will advise all subcontractors via the bidding instructions, subcontract agreements, and regularly on site to not park in front of neighboring properties and to follow all posted signs. Workers shall not park on any street within West Hollywood and shall utilie off-street parking secured by the contractor until the on-site garage is completed and is able to be used (6 months). The West Hollywood City Hall parking structure is the selected location for construction worker parking during this period. Refer to Exhibit B for construction parking map and worker travel path to/from parking/site. The construction parking location is to be approved by the Building Official, City Engineer and the Director of Community Development. Coastal Pacific Construction LLC advises all subcontractors via bidding instructions that on-site parking is not available. 7 n Closing We understand that the above items carry a great level of importance to the City of West Hollywood; Coastal Pacific Construction LLC will copy this document to all subcontractors and post it at the project site. Furthermore, additional reference materials necessary to ensure properly coordinated logistics will be added to the post board and weekly subcontractor meetings. As the person who prepared this construction period management plan, understand that am responsible for compliance with every item of this plan, until the plan is complete.
