2 SCOPE OF WORK: A. SPRAY FOAM INSULATE ATTIC AREA B. FRONT ROOF REPAIR C. REAR ROOF REPAIRS D. INTERIOR WALL AND CEILING REPAIRS ALTERNATE BID ITEMS: 1. SPRAY FOAM INSULATE ADDITIONAL ATTIC RAFTER BAYS (SF) 2. ADDITIONAL SLATE ROOF REPAIRS (SF) NOTE: The contractor will block off work areas, cover and protect windows (inside and out), interior surfaces in work areas and clean and remove all debris associated with the following work. The building must remain weather tight throughout the project. A work area(s) for equipment and materials will be designated prior to the start of the project.

3 SCOPE OF WORK: A. SPRAY FOAM INSULATE ATTIC AREA Remove the existing batt insulation from the inside end wall and use some to block off open floor joist bays (2). Cover/seal end wall gable vent with ½ exterior grade plywood painted to match the exterior trim color. Use 2lb. closed cell spray foam in all exposed rafter bays to a minimum thickness of 4 and cover with an approved thermal barrier as per NFPA 275 criteria. At the interior end wall with doorway; spray foam stud bays and fill ends of open joist bays (2), cover with ½ sheetrock and fire tape. Install weather stripping around door jamb and a door sweep to the existing door. B. FRONT ROOF REPAIR Label and remove slate roofing and copper step flashing from the roof area as noted and save for reinstallation. Remove/reinstall roof boards as needed to replace or sister onto the broken rafter(s). Rafter repair to start over the end wall top plate and extend a minimum of 6 past the break area and fasten with structural screws. Remove fascia trim board and damaged soffit along front overhang as needed and rebuild/repair in kind. Prime and paint any new trim to match existing. Remove/reinstall roof boards as needed to spray foam all rafter bays. Replace any rotted or damaged roof boards with similar material. Install copper drip edge and cover the roof surface with Grace Ice and Water Barrier (or approved equal) and lap membrane up the brick side walls a minimum of 3. Reinstall existing slate and step flashings. Purchase and install replacement slate and copper step flashing for any missing or damaged pieces. Slate and copper are to match the existing materials in thickness and width and color as closely as possible. Reset the existing copper counter flashing into the mortar joints at the lower edge of the roof and check and reseal any other flashings that need repair to ensure a watertight repair.

REAR ROOF REPAIRS Starting at the Slate/copper transition edge below the three dormers in the back of the building: mark and remove all slate tiles (save for reinstallation) and flashings to a point 2 above the dormer roof lines. Remove the slate siding on the dormers and save for reinstallation. Remove roof boards as needed to determine if insulation is present and spray foam insulate roof cavity as needed. Determine if any of the roof penetrations in repair area are non-functioning (if possible) and remove. Remove and replace any rotted or damaged roof boards as needed with similar materials. Clean, prep and cover copper roofing standing seams from under the slate down to 4 past the bend in the roof panel with EternaBond CopperFlash with EternaPrime according to manufacturer s instructions. Install copper drip edge on dormers and cover all exposed roof areas and sides of dormers with Grace Ice and Water Barrier. Install copper roof panels on three dormers. Seal around window trims and reinstall slate roofing and siding. Purchase and install any replacement slate for missing or damaged areas. Remove and replace one section of standing seam copper roof panel below the chimney/valley are that was damaged by falling Ice. Install Ice and Water barrier to roof deck and seal and flash all joints and seams to match existing. Install a ¾ thick x 12 wide x 3 long impact pad below valley area to protect replacement panel. Use DUO-PAD or approved equal. D. INTERIOR WALL AND CEILING REPAIRS Protect and cover all surfaces in repair areas. Scrape, patch prime and paint various water damaged and stained ceiling and wall areas below roof leaks. Front roof leak area to include plaster ceiling and wall damage in main stairway, coat closet and bathroom. Small coat closet behind hidden panel door needs plywood patch removed, insulation installed and sheetrock installed and finished. Prime and paint to match existing wall color. Rear roof leak areas are the ceilings in the side hallway, main office area and back single office. Remove joint tape, re-tape, and finish prime and paint to match existing surfaces.

5 ALTERNATE BID ITEMS: A. SPRAY FOAM INSULATE ADDITIONAL ATTIC RAFTER BAYS (SF) Provide a per square foot price to spray foam seal and apply an ignition barrier to attic space rafter bays and end walls. B. ADDITIONAL SLATE ROOF REPAIRS (SF) Provide a square foot price to; remove damaged/leaking slate roof areas, expose and install spray foam insulation in rafter bays, install Ice and Water Barrier and reinstall slates and flashings. Notwithstanding any work outlined herein, it is the sole responsibility of the contractor to strictly adhere to any and all federal, state, and local requirements. This includes all code requirements, lead safe requirements, as well as, any asbestos requirements, etc. Contractor will purchase all materials necessary to complete the project unless otherwise noted in scope of work. Note: Cost for contingencies requiring additional work shall be determined as follows: It is understood that from time to time unforeseen circumstances, which are beyond the control of the contractor, may result in the need for additional work to be performed. However, if such situations occur, an addendum to the original proposal must be agreed upon by the contractor, The City of Rome, and the property owner. Any such agreements; and, the cost thereof; must be set forth in writing and signed by all parties hereto, in order to be valid. So, do not make changes without written approval. Contractor must keep building secure at all times; use reasonable efforts to prevent any unauthorized entry; and, to use prudent and necessary measures to ensure that there is no water infiltration or other entry of the elements due to inclement weather. In regards to the performance of the work outlined in this project, contractor shall meet or exceed all code requirements of the City of Rome. If there are any items outlined herein that the contractor feels can be better accomplished through alternate means, the Department of Community & Economic Development is open to suggestions. However, as stated above, any changes cannot be made without prior written approval. In regards to specific products designated herein, contractor shall strictly adhere to all manufacturers installation instructions. If there are products outlined herein that the contractor feels can be substituted for a more suitable or desirable product and without appreciable cost differential then The Department of Community

6 & Economic Development is open to all reasonable suggestions but as stated before must be approved in writing. I have read the above in its entirety; I agree to the specifications and guidelines set forth herein; I understand the scope of work and I am qualified to perform the work in a professional manner, within the specified time frame. SITE VISIT: Site visits are REQUIRED, and can be done by contacting MIKE or mdorsino@romecitygov.com. A sign in sheet will be in the Codes Enforcement office. Current information should also be augmented by site visits prior to bidding.
