Contractors shall include all omissions, additions, and adjustments of all trades as may be necessary because of each change, substitution, or omission. Such omission, addition or adjustment shall conform to recognized and accepted engineering practice: General 1. The sign in sheet and the meeting minutes from the Pre-Bid meeting are attached. Specifications 1. Cover: Change the ASU Project Number to read as All pages of Project Manual: In the lower left corner, change the ASU Project Number to read as # Drawings 1. Sheet A0.0 Cover Sheet: In the upper right corner of the sheet, below the Project title change ASU # to read ASU # NOTE: THE CONTRACTOR IS RESPONSIBLE FOR IDENTIFYING ALL TRADES AFFECTED BY THIS ADDENDUM AND NOTIFYING EACH TRADE OF THESE REVISIONS TO INCORPORATE INTO THEIR WORK. 3 ALABAMA STATE UNIVERSITY Renovations to G.W. Trenholm Hall PRE-BID CONFERENCE MINUTES August 21, 2017 at 10 A.M. CST GENERAL: 1. Bid Date / Time / Location: Monday, August 31, 2017 at 2:00 pm CST Alabama State University Procurement Department Physical Plant Building, Room West Fifth Street Montgomery, Alabama Introduction of ASU and HAPC Attendees: Scott Standerfer- ASU Owner Arlene Thompson ASU Director of Procurement Anne Marie Ellis- Herrington Architects PC (HAPC) Architect Patrick Collins- Whitaker & Rawson Mechanical Engineer David Morrow Hyde Engineering Electrical Engineer 3. Eligibility: As per Contract Documents- Bidders who attend this mandatory Pre-Bid Conference and Site Walk-through. Licensed General Contractor in the State of Alabama. General Contractor s License (Put license number on outside of Bid Proposal envelope). 4. Bid Preparation / Bid Guaranty / Delivery of Bids: As per Contract Documents. Use enclosed Form of Proposal. No alterations shall be made to bid proposal unless modified by Addendum. Bid Guaranty in the form of Bid Bond or Cashier s Check; 5% of bid amount, not to exceed $10,000. Must use ABC Form C-4 for Bid Bonds. Bid Bond must be executed by a Surety Company duly authorized and qualified to make such bonds in the State of Alabama, payable to Alabama State University. Cashier s check must be drawn on an Alabama bank. Bids to be delivered to Alabama State University Procurement Department (address above) if hand delivered prior to the Bid Date and Time (Augutst 31, 2017 at 2:00 PM CST). Contractor s responsibility to make sure Bid is received. Return Bid Form and Bid Bond with bid submittal. 5. Performance and Payment Bonds: As per General Conditions, Article 38 Performance and Payment Bonds. Performance Bonds, 100% of contract amount. Payment Bonds, 100% of contract amount. 6. Addenda: Three addenda have been issued to date. Minutes of Pre-Bid and any pertinent discussed items shall be issued in an Addendum following pre-bid conference. Any further addenda necessary after Pre-Bid will be uploaded to Alabama Graphics digital planroom. 1

4 7. Pre-Bid Requests for Information or Clarification: The last day for questions is NOON on Wednesday August 23, 2017, Central Standard Time. Any questions received after that time will not be considered. All requests for information must be submitted in writing and may be ed to the Architect (attention: Anne Marie Ellis) at 8. Project NTP/Mobilization: Owner anticipates issuing a Letter of Intent to the Lowest Bidder by August 31, It is anticipated that issuance of the Notice to Proceed will be issued by September 1, Upon the effective date of the Notice to Proceed, the Contractor will have 50 calendar days complete the Work. Contractor will not have use of the construction areas until September 1, Work must be complete and ready for Owner move-in by October 20, Liquidated Damages: Liquidated Damages for which the Contractor and its Surety shall be liable and may be required to pay the Owner in accordance with the Contract Documents shall be equal to 6% interest per annum on the total Contract Sum. 10. Builder s Risk Coverage: As per the General Conditions of the Contract Contractor furnished. Contractor also responsible for deductibles. 11. Project Observation / Material Testing / Test and Balance: Testing to be performed as indicated in specifications Coordination of testing. Unless otherwise indicated, Contractor shall give testing agency a minimum of 24-hrs prior notice for activities requiring testing/observation. 12. Project Coordination / Communication: All correspondence between Owner / Contractor / Architect or Architect s consultants shall come through the Architect from Contractor to the Owner. Include Project name Alabama State University- Renovations to G.W. Trenholm and Project numbers (ASU # , HAPC #599) on all project related correspondence. 13. On-Site Personnel: Contractor will at a minimum provide the following on-site staffing for this project: A) Lead Superintendent Submit resumes within 24 hours of LOI for approval. Superintendent must be someone that is authorized to make decisions. Superintendent does not necessarily have to be a non-tool-working superintendent, but he needs to be able to monitor activities while working. 14. Safety: The Contractor shall be responsible for all project safety. Neither the Owner, Architect, nor any other Consultant employed by the afore-mentioned parties will be responsible for the Contractor s safety precautions, means, methods, techniques, sequences, or procedures. Contractor s personnel responsible for safety shall be OSHA certified. 15. Child Labor Law: All Contractors shall adhere to the Child Labor Law. (No workers on the project under the age of 18). 16. Davis-Bacon This project has Title III Funding. Therefore, the Contractor will be responsible for adhering to Davis-Bacon Act wage rates and reporting requirements. 17. Meetings: Safety Meetings - Contractor shall be responsible for coordination, content, scheduling, etc. of safety meetings with his employees. Owner / Consultant / Contractor (OAC) progress meeting to be held per project requirements- once a month at a minimum. 2

5 PROJECT SPECIFIC REQUIREMENTS: 18. Completion Time for Project: Time for Completion: The Contractor will have 50 calendar days from Date of Commencement indicated in Notice to Proceed to complete the Work. Anticipate Notice to Proceed from the Owner by 9/1/2017. A Pre-Construction Conference will be held prior to construction. Time to be determined. 19. Soil Erosion Control, Dust Control, and BMP: As per Sheets A0.0 and Project Specification Sections Summary and Temporary Facilities and Controls. Contractor is responsible for implementation of soil erosion and dust control measures in compliance with Best Management Practices as defined by the Alabama Department of Environmental Management. 20. Existing Conditions / Constraints: As per Project Specifications Section Summary and Section Execution. Contractor is responsible for verifying existing conditions prior to construction. Asbestos is present in the building. GC to have an asbestos contractor abate the spaces prior to demolition per Project Specifications Section Asbestos Abatement 21. Existing Utilities: As per A0.0 and Project Specifications- Sections Temporary Facilities and Controls and Execution. Verify location of utilities prior to construction. 22. Parking for Contractors and their Subs and workers: As per Project Specification Section Summary Parking will be allowed on site. Contractor is to obtain Parking Passes from Campus Police, located at 1452 Carter Hill Road, Montgomery, Alabama The campus is enclosed by security fencing. Primary entrance is off North University Drive. 23. Traffic Control: As per Project Specifications Sections Summary Should damage occur to University streets and/or adjacent properties as a result of construction activities, the Contractor shall be responsible for repair of all damages to the satisfaction of the University, local municipality, and/or property owner. 24. As-Built Drawings: As per Project Specifications Section Project Record Documents. Contractor required to provide As-Built drawings at the end of the project.
