The GENERAL BID DATE IS REVISED AS FOLLOWS: General bids will be received at the Office of the Board of Selectmen/Town Manager, no later than 11:00 AM, Wednesday, October 12, Swampscott Town Hall Office of the Board of Selectmen/Town Manager 22 Monument Avenue Swampscott, MA The bids will be opened at the Swampscott Town Hall 2. Bids that are received as of October 7, 2011 (the original bid date) will be returned unopened. 3. The Filed Sub Bid for Ceramic Tile submitted by G.V.W., Inc. in the amount of $34,000 may only be used by G.V.W., Inc; East Boston, MA. It is a restricted filed sub bid. 4. CIVIL SPECIFICATION MODIFICATIONS: The following modifications, amendments, deletions and additions shall be made to the Construction Specifications for the Swampscott Police Headquarters specifications and drawings (Contract Documents), added text is bolded (bold); omissions are italicized (italicized); words or sentences to be removed are struck out (struck out): Amend Section Clearing and Demolition as noted below: a. BASE BID VOLUMES FOR DEMOLITION: i. The Contractor shall carry the following volumes of concrete/stone foundation and footing materials to be removed as part of this section. Should additional or lesser volumes be removed, the value of such removal shall be adjusted on the unit prices defined in Section EARTHWORK, 3.17 UNIT BID PRICES, item B, 17) Unit Prices (see new section) 1. Building Footings and Foundation Walls: Base bid volume is 260 c.y. (350±lin. ft. of wall, 2.0 wide footing, 2.0 clearance, 5 deep). (140 x 75 x7)= 2722 cubic yards for building construction. 2. Slabs: Base bid volume is: 1,200 c.y. (7,800 sf x 1.0 thick slab x 3 over excavation ). Utility lines (150 x 3 trenches x 5 wide x 5 deep) = 1250 cubic yards for utility installation.

Volumes noted above shall include the removal of the footings, foundations and slabs remaining from the historic building demolition, it shall include all soils located from the bottom of the footing, plus twelve inches each side up to finished grade. The base bid provided for in Section EARTHWORK, shall include the backfilling of these areas with the specified materials. 5. The attached Section Unit Prices (3 pages) is hereby included as part of the Contract Document. END OF ADDENDUM No. 5

3 SECTION PART 1 - GENERAL UNIT PRICES 1.01 GENERAL PROVISIONS A. The predetermined Unit Prices set forth herein shall be used to determine the equitable adjustment of the Contract Price in connection with the changes or extra work performed under this Contract, as authorized in writing by the Architect. Compensation for changes in the work which do not involve Unit Price items, shall be determined by other methods of payment in accordance with the requirements of GENERAL CONDITIONS OF THE CONTRACT. B. It is mutually understood and agreed that such Unit Prices include all items of cost, equipment, taxes and insurance of every kind, overhead, and profit for the Contractor, and any Subcontractors performing work under the respective Unit Price items, and they shall be used uniformly, without modification, for additions to, and deductions from, the Contract Price. C. Sufficient prior notice shall be given to the Architect so that he may take the proper measurements of materials removed or to be replaced. All quantities used in the determination of additions to or deductions from the Contract Price due to Unit Prices shall be those which have been determined and approved by the Architect. D. Performance of work which is not required under the Contract Documents, or which is not authorized by Change Order, whether or not such item is set forth as a Unit Price item, shall not be considered cause for any extra payment. The Contractor will be held fully responsible for such unauthorized work, including the performance of all corrective measures required by the Architect. E. All Unit Prices shall be in effect from the date of commencement of work until final completion of all work under the Contract. F. All materials, methods of installation, and definition of terms set forth for the various Unit Price items, shall be as set forth in the Contract Documents. G. Unit Prices for excavation shall include the costs of sheeting and bracing, pumping and dewatering, and all other costs in connection therewith. H. The payment lines of the Contract shall be as indicated in the Contract Documents
