3 UNDERWATER INSPECTION REPORT SUMMARY The substructure units inspected at Bridge No , Piers 1 and 2, were found to be in good condition with no defects of structural significance observed. The channel bottom appeared stable with no changes since the previous inspection. INSPECTION FINDINGS (A) The channel bottom material consisted of gravel and 1 foot diameter and smaller cobbles, with no appreciable probe rod penetration. (B) The concrete was in smooth and sound condition. RECOMMENDATIONS (A) Reinspect the submerged substructure units at the normal maximum recommended (NBIS) interval of sixty (60) months. The superstructure is supported by two reinforced concrete abutments and two reinforced concrete piers. The pier and abutment footings are founded on steel H-piles. The piers are labeled Piers 1 and 2 starting from the west end of the bridge. 2. 6 UNDERWATER INSPECTION INSPECTION PROCEDURES The routine underwater inspection of Bridge (CSAH NO. 11 over the Cannon River) was completed on May 26, The underwater inspection was conducted from shore. The inspection was conducted by a team consisting of a PE-Diver with a valid MnDOT Team Leader certification, a backup diver and a dive tender. Due to waterway conditions at the time of inspection, the inspection could be accomplished by wading in accordance with OSHA regulations. Profiles were taken along the upstream and downstream faces of the bridge and around the periphery of substructure units to determine the presence, location and area of scour. The bridge elements inspected consisted of Piers 1 and 2. According to the bridge inventory or design drawings, Piers 1 and 2 were founded on rectangular spread footings with steel H-piles. Inspection procedures followed FHWA guidance and the MnDOT Bridge and Structure Inspection Program Manual with channel bottom probing to search for foundations. The routine underwater inspection frequency is recommended to remain at a maximum of 60 months based on those findings and risk factors. Also, inspection procedures should continue to follow the above approach and standard guidance with 100% Level I and 10% Level II intensity efforts. Page 6 of 18

[2013] The first 3 to 5 feet on all beams were painted during reconstruction in The entire length of the remainder of the beams have faded paint with freckle rust along the edge and top of the bottom flange. [2013] All beams have fading paint with freckle rust along the edge and top of the bottom flange. Span #3: [2015] Same/add; areas of prevalent freckling rust, CS3 (100LF). North fascia bolted connection has loose bolt, inside upper flange. [2013] The first 3 to 5 feet on all beams were painted during reconstruction in The entire length of the remainder of the beams have faded paint with freckle rust along the edge and top of the bottom flange. Approximately 5 feet to 25 feet out from the abutment, the top flange has scattered areas of peeling paint with active corrosion. Southeast Wingwall: [2015]-[2013] Has a diagonal crack with a small delaminated area at the top due to impact damage. Also there is a diagonal gouge in the front face next to the parapet wall. Northwest Wingwall: [2015]-[2013] Has a horizontal crack with leaching at the top next to the railing with a small area of delaminated concrete. Also there is a diagonal gouge at the front corner. Northeast Wingwall: [2015]-[2013] The wingwall show little or no deterioration at the time of this inspection. Reinforced Concrete Pier Cap 05/26/ LF Underwater 138 LF Notes: Pier #1: [2015]-[2013] Has 4 vertical cracks with no leaching or staining. West Abutment Bearings: [2015]-[2013] Bearings 1, 2 have failed paint with active corrosion with little to no loss of section at the time of this inspection. Also all 4 bearings are set approximately 1/2 inch towards the back into expansion at 60 degrees F. Bearing 5 has moderate loss of section across the front edge of the bearing device. It is also misaligned. Measurements taken from the front edge as follows: South edge of bearing is back 1.5" and the north edge is back 1". The south edge hangs back 1/2" over the back of the masonry plate. Bearing is no longer functioning as intended. East Abutment Bearings: [2015] Same/add; Bearing 5 now has minor loss of section. [2013] All 5 bearings have failed paint with active corrosion. All 5 bearings are offset 1/4" + or - into expansion at 60 degrees F. Bearing 1 has minor loss of section at the time of inspection.
