1 Sidewalk Assessment Restoration Process 1. Purpose 2. Scope 1.1. To assess potentially hazardous conditions for exterior sidewalk and/or pavement surfaces which require replacement and/or repair This process applies to exterior sidewalk and/or pavement surfaces on the campus of the University of Notre Dame located in Notre Dame, Indiana. This process does not apply to interior facilities and campus roads or parking lots. This process also excludes any externally leased or internationally located properties owned by the University of Notre Dame. 3. Definitions 3.1. Exterior sidewalk and pavement surfaces for which anyone within the campus limits walks, works, or gains access to various campus facilities or amenities. 4. Assessment Responsibilities 4.1. Facilities Design & Operations Planning, Design and Construction Perform yearly evaluations of all exterior sidewalk and pavement surfaces on the University of Notre Dame campus to determine hazard locations with Risk Management and Safety. Review hazardous locations throughout the year sent from the Good Catch Reporting Tool as discussed in section 6. Coordinate replacement and repair locations in full coordination with Risk Management and Safety. Determine course of action and timeframe for removing hazard with local contractor. Prioritize hazard locations identified in the yearly evaluation. Coordinate with local contractor replacement and repair plan. 1

2 4.2. Landscape Services Report potentially hazardous conditions for exterior sidewalk and/or pavement surfaces which require replacement and/or repair identified throughout the work day through the hazard reporting tool Risk Management and Safety Review and communicate hazardous conditions to the Facilities Design & Operations Planning, Design and Construction Project Manager as identified through injury reports and ND App. Verify replacement or repair to a sample of hazard locations have been completed. Conduct a yearly review of this Sidewalk Assessment and Restoration Process as outlined within this document. 5. Hazard Criteria & Assessment 5.1. Curb Ramps Settlement, depressed areas, or inadequate drainage Review bottom of ramp for signs indicating standing water or inadequate drainage. Level changes or gaps at curb ramp transitions between drive, sidewalk, and gutter or at top of curb ramp transition to sidewalk. Review all transitions for level changes and/or gaps Cracks or deteriorated sections of curb ramp 5.2. Level Changes Sidewalks/pavers Service drives or crosswalks at parking lot entrances Curb-to-sidewalk transitions Sidewalk to landscaping or lawn areas. Transition changes may occur from soil erosion, inadequate drainage, traffic ruts, deteriorated mulch beds, etc. Various material transitions Review level changes where various material changes occur such as: asphalt-to-concrete, concrete-to-pavers, stone-to-curb, etc. 2

3 5.2. Deteriorated walks, cracks, or gaps at asphalt pavement walks and concrete or paver walks Settlement or level changes at utility structures Review Appendix A for visual examples of hazardous criteria. 6. Slip, Trips, & Fall Hazard Good Catch Reporting Tool ND Mobile 6.1 The hazard Good Catch reporting tool is a tool that will soon be available on the ND Mobile application available on various mobile devices to report slip, trip, fall hazards on exterior campus sidewalks and surfaces Reports made through this tool will be ed to The Associate Director of Safety in Risk Management and Safety. The reports will then be forwarded to appropriate University organizations as needed. 3
