2 (NU) is seeking SEALED proposals for a Wood Frame, Metal Siding, Open Front Storage Building. All proposals must be mailed or delivered, at the vendor's sole risk, to the address shown in this request for proposal. NU is not obligated to accept proposals received after the date and time stated above. Exceptions, if any, will be considered in the proposal evaluation in determining the successful vendor. Vendor must provide supporting documentation with its proposals for any exceptions. NU reserves the right to reject any or all proposals, to waive any and all informalities and/or irregularities in the proposal, to negotiate and/or renegotiate with any vendor the terms of its proposal, and to accept any proposal which in its opinion may be in the best interest of NU. Proposals may be held by NU for a period of time not to exceed sixty (60) days from the due date of the proposals for the purpose of evaluating the proposals or investigating the qualifications of the vendors, prior to awarding of the order. All proposals shall remain effective for at least sixty (60) days after the due date of the proposals. OVERVIEW is a municipal electric, water and wastewater service provider providing services in Newport (Cocke County), TN and parts of surrounding counties. Newport s service area is approximately 600 square miles. Page 2 of 9

3 REQUEST FOR VENDOR QUALIFICATIONS (NU) is in the process of evaluating vendor proposals for a Storage Building per specifications in this RFP. Your firm is one of the organizations from which qualifications and proposals are being requested. Box 519 Newport, TN Project No: NU Project Title: Wood Frame, Metal Siding Open Front Storage Building Project Description seeks a General Design/Build Entity for the construction of a 75 x 24 wood frame, metal siding open front storage building upon existing concrete slab located at the Operations Center, 1419 West Highway 25-70, Newport, TN The successful Design/Build Entity shall produce professionally designed drawings as required by, Local, State and Federal building codes and execute the work based upon required drawings. Minimum Requirements Notice is hereby given to all interested parties that all teams will be required to meet all minimum qualifications to be considered for this project. To be considered as qualified, interested teams shall meet the following minimum requirements: 1. All Design Build/ Entities (General Contractor, Electrical Contractor, Plumbing Contractor and HVAC Contractor) must be licensed Contractors as required by the Contractors Licensing Act of 1976, enacted by the General Assembly of the State of Tennessee on March 18, Bidder s name, license number, date of expiration of license, license limitation and that part of license classification applying to the bid must be placed on the envelope containing the Bid, otherwise this bid cannot be opened or considered. Demonstrated specific Design/Build Contracting experience in projects of similar scope and complexity. 2. Demonstrated bonding capability up to $50,000 for an individual project coincidentally with current and anticipated workloads; provide letter from surety that affirms this capacity. 3. The prime or lead for the Design Build Entity must be a General Design/Build Entity with general construction experience. 4. The Contractor must provide insurance that would provide adequate protection for the Contractor against damage claims that may arise from operations under the project, whether such operation be by the insured and also against any of the special hazards that may be encountered in the performance of this project. 5 Teams meeting the minimum requirements may obtain the RFP documents at: 170 Cope Blvd. Newport, TN Or online at Scope of Services The successful General Design/Build Entity shall meet the following minimum requirements: GENERAL INFORMATION A. INTRODUCTION/DESCRIPTION OF BUILDING SPECIFICATIONS The construction of a 24 x 75 wood frame, metal siding, open front storage building upon an existing concrete slab that is approximately 6 thick. Roof Slope: 4/12 manufactured trusses Trusses set 16 on center 1 x 4 slating for roof attachment Open to front 8 x 8 wood posts with 25 clearance between, 3 steel beams to carry roof load 13 0 bearing height Back and side walls 6 x 6 wood post on approximately 12 centers, eave and sidewall height 12 Double 2 x 10 beam and 2 x 6 wall purlins Outside roof and wall covering 26 gauge, color to be determined by Owner Bird netting shall be installed on ceiling down to wall liner The interior walls shall be 8 light gauge metal liner panel and the bird netting will make up the difference to the ceiling. 16 overhang continuous Soffit all covered with white metal Continuous 6 gutters with 4 down spouts Page 5 of 9

6 The successful Design/Build Entity shall produce professionally designed drawings as required by, Local, State and Federal building codes and execute the work based upon approved drawings. B. DESIGN QUALIFICATIONS The successful Design/Build Entity shall meet the following minimum requirements: 1. To hire independently or have on staff the appropriate professional designers licensed in the state of Tennessee to appropriately design the work as described. 2. The ability to produce coordination documents to include but not limited to traffic control, erosion control, staging and work sequence schedules and plans in order to perform the work in conjunction with current activities. 3. Project deliverables/requirements and construction services include but are not limited to: a. 95% Design Documents for review and approval. b. Investigate to protect existing utilities. c. 100% Construction documents d. Detailed Schedule of Values e. Progress schedule f. Contract Administration g. One (1) pre-construction design meeting, and One (1) post construction closeout meeting. h. Construction services to include all labor, material and equipment to perform the work. i. 7 C. COST PROPOSALS 1. Cost Proposals shall be submitted on the attached proposal form without modification. A Cost Proposal shall be accompanied with sufficient detail to clearly identify the cost for design and management services construction and general conditions. Proposal shall include any warranties that may apply. Percentage of the cost of work is not an acceptable value. The Cost Proposal should be prepared independently in accordance with the following: 2. This Cost Proposal is a binding offer to perform the services associated with the Scope of Services described in this RFP. however, reserves the right to negotiate a cost adjustment based on scope clarification, prior to contract execution. 3. This RFP document and any written addenda issued prior to the submittal of cost proposals shall serve as the only basis for cost proposals. 4. The respondent, by submitting this proposal, does herby accept that minor changes by, which do not adversely affect the respondent, shall not be cause for withdrawal or modification of the amounts submitted herein. Upon due consideration and review of this document along with its written addenda, and written clarifications, the respondent submits a proposal for Design/Build Contracting services, consistent with the scope of Work. 9 COST PROPOSAL FORM DESIGN/BUILD SERVICES DATE: NEWPORT UTILITES METAL OPEN FRONT STORAGE BUILDING Gentlemen, The Design Build Entity is in compliance with the Request for Proposals for the design and construction of the garage addition. Therefore, having examined the site of the proposed work, and being familiar with all the conditions surrounding the purposed project, hereby proposes to furnish all design, labor, materials and supplies to construct the project in accordance with this Design/Build Services Request for the following cost: 1. D/B General Conditions (lump sum) $ 2. Add On s $ 3. Deduct s $ Total Design/Build Cost $ The cost shall include preconstruction services, design, construction costs, general conditions, profit, overhead, home office staff, home office expenses, accounting and/or legal fees, insurance and any other costs or expenses. Acknowledge receipt of Addendum Nos. No Design/Build Entity will be permitted to withdraw his bid within sixty (60) days after the above date.
