Addendum No.: 1 Bid#: Project Name: Lake Village Drainage Improvements/N. Pearl Dr. Mill and Overlay Bid Due Date: August 29, 2017 GENERAL INFORMATION: 1. Please replace Note 11 on Sheet G-2 with the following language: Contractor is responsible for repairs of damage to any existing improvements during construction, such as, but not limited to, drainage utilities, pavement, striping, curbs, etc. Repairs shall be equal to or better than existing conditions. Contractor shall obtain preconstruction photographs and/or video in digital format of all areas to be disturbed by construction including any adjoining property. Pre-construction photographs/video shall be submitted to the engineer a minimum of two weeks prior to commencing construction activities. 2. Contractor should phase work to limit the disruption to the residents. Sidewalk should not be removed all at once due to the schools in the area. 3. Regarding the mill and overlay, one contractor asked whether the side streets can be used to do the mill and overlay work. This is acceptable but should be included in the contractor s traffic control plan. 4. Contractors should be aware that close coordination with homeowners will be needed to minimize disturbance and maintain access to private property for the duration of the project. Traffic may be limited to a single lane with an approved traffic control plan. 6. Contractors should be prepared to notify homeowners of lane closures as needed. 7. Limits of construction for the project include the Parish right-of-way and temporary construction easements obtained by the Parish. The limits of construction will be provided to the contractor in AutoCAD format to define these limits and assist with contractor layout. 8. Existing properties and adjacent areas, particularly existing driveways, should be photographed prior to construction work beginning. 9. All connections to the existing storm drain system along N. Pearl Drive must be connected with the new drainage system. If contractor discovers an existing connection that is not already shown in the plans, the information should be brought to the attention of the Parish immediately. 6 Project Purpose Improve local drainage infrastructure to Lake Village subdivision to reduce the risk of flooding to structures. Mill & Overlay of area disturbed by construction. Multiple options considered through a comprehensive drainage analysis. Recommended option involves increasing the capacity of the area to drain toward the W 15 Canal. Project integrated with Lower W 15 Canal Improvement Project.
