1 ADDENDUM NO. 1 DATE: December 7, 2018 PROJECT: OWNER: BID DATE: Owosso Pilot Water Grant City of Owosso Tuesday, December 18, 2018 at 3:00 pm The following shall be incorporated into the Contract Documents dated November 12, GENERAL Engineer s responses to inquiries. PROJECT MANUAL RFI - 1 What is the funding source for this project? Is this all City of Owosso funding or is the State of Michigan or Federal Government funding any portion of this project? The project will receive grant funding through the MDEQ Pilot Grant program. RFI 2 Is this a prevailing wage project? Is there a requirement for certified payroll? This is not a prevailing wage project and there is no need for certified payroll. RFI 3 Are the existing curb stop valves required to be replaced with new for Case #1 (curb to meter) replacements? Curb stop valves are not required to be replaced for Case #1 replacements. RFI 4 Are we to match existing pipe diameter sizing when replacing services? What size services will be replaced (3/4, 1, 1-1/2, 2, etc.)? If there are different service sizes can more pay units be added for the additional cost of large service diameters? Service pipe size shall be matched to existing. There will be no extra pay items added, it is anticipated the majority of services will be ¾ or 1 diameter. RFI 5 In the event that a new tap on the main is required, will the city perform the tap or the contractor? If the contractor taps the main can a pay item be added for that? The contractor will be required to perform the watermain tap if one is required. The revised bid form attached includes a pay item for a watermain tap. The Watermain Tap pay item shall include all labor, equipment and material necessary to make a new service lead connection at the existing water main. ALL BIDS MUST BE SUBMITTED USING THE ATTACHED BID FORM. Addendum #1 Page 1 of 2

2 RFI 6 If there is a meter pit and there is a Case #1 or Case #3 replacement (replacement on home side of curb stop), is the contractor to replace up to the meter pit only? Or are we to replace the piping on the downstream side of the meter and into the building? If we are to replace downstream of the meter is there a plumbing permit required? This will require that a licensed plumber does the work. The contractor shall replace the lead into the building for all cases where there is lead and galvanized pipe. All work completed inside the buildings will require a plumbing permit and inspection by the city building official will be necessary. The permit fee is $95 per building. RFI 7 What are the requirements for obtaining permission from property owners for conducting work on privately owned property? Are property owner waivers required to be signed before work is completed? What documentation is required for work on private property? Work on private property will take place only on those sites that have given consent to the work prior to construction. Paperwork will be on file with the city. RFI 8 Does the City have as-built drawings and records that can be provided pre- bid for the addresses that work will be performed on? As-built drawings are not available for the locations where work will be performed. RFI 9 Are plumbing permits required for every address? What permits will be required? Will permit fees be enforced or waived by the City? Plumbing permits/permit fees will be required for the proposed work. The city s building official will inspect all work that is completed inside buildings. The contractor shall coordinate this work with the city s building official. The permit fee is $95 per building. RFI 10 There is an address (Bob s Tire) on Corunna road (M-71). Will MDOT permits be required? The contractor will be required to obtain a r.o.w. permit from MDOT at this location. 3 BID Proposal - REVISED LD SERVICE LINE REPLACMENT BID TO: THE CITY OF OWOSSO (HEREINAFTER CALLED THE CITY ) Bidder must provide pricing for each item listed. If additional pricing elements are being offered by the bidder, they are to be listed under "other services/items offered." 4 VARIANCE FROM SPECIFICATIONS: If the bidder is unable to comply with the specifications as outlined, the bidder shall clearly note these variations from the specifications. The bidder may also propose additions to these specifications for the city to consider, but the costs associated with these additions shall be stated separately. If the work in not complete on or before the date set for completion or any extension, the Contractor shall pay the city liquidated damages of 8500 a calendar day until the work is satisfactorily completed. Liquidated damages for delay may be deducted from payments due the contractor or may be collected from the Contractor or the Contractor s surety. The undersigned agrees that if the city accepts this proposal, Contractor will, within 10 consecutive calendar days after receiving notice of this acceptance, enter into a contract to furnish all labor, equipment and tools necessary to execute the work at the unit prices named in the bid proposal. Contractor will furnish the surety for performance, for 100% of this bid, which shall be accepted and approved by the city. The undersigned agrees that if the city accepts this proposal, Contractor will start this project no sooner than January 28, 2019 and will substantially complete the entire work under this contract by May 1, This schedule may be extended for rain days or cold weather for calendar days after May 1, 2019, only as approved by the city of Owosso.
