3 UNDERWATER INSPECTION REPORT SUMMARY The substructure units inspected at Bridge 27611, Piers 1 through 3, were in overall good condition with no defects of structural significance observed. A localized scour depression and footing exposure was observed at the upstream end of Pier 2. No vertical exposure of the footing was observed. A light accumulation of timber debris was observed along the east face of Pier 3. Minor spalls from construction were noted at the waterline of Pier 3. Aside from localized scour, the channel bottom appeared stable with no significant changes since the previous inspection. INSPECTION FINDINGS (A) The channel bottom material consisted of silty sand and gravel, allowing up to 8 inches of probe rod penetration and scattered cobbles. (B) A light accumulation of timber debris was observed along the east face of Pier 3 from channel bottom up 3 feet. The debris consisted of a 2 foot diameter log and scattered 6 to 12 inch diameter drift pieces. (C) A vertical hairline crack was observed from the top of the web wall to the waterline at Pier 2 and Pier 3. (D) A scour depression was observed around the upstream nose of Pier 2. The scour had a radius of approximately 5 feet and a maximum depth of 4 feet. The scour had partially exposed the top of the pier footing at the upstream nose with no vertical face exposure. (E) At the waterline, at midpoint and both quarter points of each face of Pier 3 there was a 2 square foot area of concrete section loss (construction related) with up to 2 inches of maximum penetration. RECOMMENDATIONS (A) Scour screening assessment (MN Scour Code: N) indicates stable rating and the extent of foundation exposure has not increased since the previous inspection, therefore, presently it is only recommended to monitor the extent of scour and footing exposure during future inspections. 6 UNDERWATER INSPECTION INSPECTION PROCEDURES The routine underwater inspection of Bridge (Plymouth Avenue over the Mississippi River) was completed on June 2, The underwater inspection was conducted from a 21 foot boat. The inspection was conducted by a team consisting of a PE-Diver with a valid MnDOT Team Leader certification, a backup diver and a dive tender. The inspection utilized commercial dive equipment and techniques (SSA and/or SCUBA) in accordance with OSHA regulations. Profiles were taken along the upstream and downstream faces of the bridge and around the periphery of substructure units to determine the presence, location and area of scour. The bridge elements inspected consisted of three reinforced concrete piers. According to the bridge inventory or design drawings, Piers 1 through 3 were supported by rectangular footings founded on steel H-piles. Inspection procedures followed FHWA guidance and the MnDOT Bridge and Structure Inspection Program Manual with channel bottom probing to search for foundations. The routine underwater inspection frequency is recommended to remain at a maximum of 60 months based on those findings and risk factors. Also, inspection procedures should continue to follow the above approach and standard guidance with 100% Level I and 10% Level II intensity efforts. Page 6 of 18
