2 City Of Toronto Long Term Care Homes and Services Addendum No. 3 BA and LL Reception Renovation Tender Call # December 14 th, 2017 The Addendum consists of the following changes noted below: Part A: Additional Information No additional information at this time. Part B: Revisions 1. Section 4 Summary of Work Refer to Part D: Attachments. Items 24 through to item 31 have been provided as part of this addendum. Part C: Questions 1. Question: What are the specs for: CT1 and CT2, Existing Carpet, HM Doors, Glass and Frosted Film. Refer to addendum 2 which provides Architectural specification highlighting CT1 and CT2, HM Doors, Glass and Frosted Film. Existing carpet is an old carpet which is not specified. Architectural drawings do not ask to match existing for carpet, new floor types have been specified to replace existing carpet where shown on Architectural drawings. 2. Question: Please provide mechanical specs for Bendale Acres. Mechanical specifications for Bendale Acres are provided within the mechanical drawing set for Bendale Acres. 3. Question: If there is any mechanical work in lakeshore Lodge, then please provide drawings and specs. There is no mechanical scope for Lakeshore Lodge. 4. Question: If there is any hazardous material present then please provide report. A Designated Substance Survey will be provided through cash allowance. 5. For specification on lights, exits and dimmers refer to electrical specifications for Bendale Acres and Lakeshore Lodge provided within Vendor Viewing document. Bendale Acres Specification Section Lakeshore Lodge Specification Section Question: We are unable to find specifications for all civil trades such as demolition, rough carpentry, drywall, painting etc.) please advise / issue. Refer to Addendum 2, Part B: Revisions and Part D: Attachments 7. Question: Please provide detail specifications for mechanical scope of work. We noticed very brief description of mechanical on DRG M-1.0 Refer to Reception Renovation, Bendale Acres, Mechanical Drawing Package, Page M-1.0. Mechanical Specifications for Bendale Acres has been provided in full. 8. Question: Please provide existing building controls contractor contact details including Nurse call systems & Fire Alarm. This question was answered in a previous addendum (addendum 2). The contacts provided above are for information purposes only. General Contractors bidding the work are not obliged to use base building contractors. The City does not endorse the names of the above noted contractors or recommend them for use for this Tender. All General Contractor's entering into contractual relations with the noted subcontractor s does so at their own risk. The contractors noted above are current and are subject to change any time. 9. Scope of Work Refer to Part B: Revisions, Item 1 and Part D:Attachments. Item 31 of Section 4 Summary of Work clarifies the responsibilities of each part within the contract. *Security integration and devices will be by the City s integration vendor and will be provided by cash allowance. 10. Question: Demolition drawing shows demolish millwork reception counters, whereas during site meeting this was informed to us that we have to move millwork to nearby area and this should be in use until completion of work. If this is correct then this means that all the systems such as nurse call system, parking gate barrier etc should be in working condition from the relocated reception counter. Please inform what exactly has to be done. Location of a temporary reception at Bendale Acres and Lakeshore Lodge will be completed by the Home prior to construction start. Coordination and schedule will be coordinated with the Building Service Manager for approval prior to construction start. 5 Section 4 Scope of the Work Tender Call No Contract No. N/A SCOPE OF THE WORK Provide all labour, equipment, and materials required (in phases as indicated) to complete the renovations as per the tender documents for Bendale Acres and Lakeshore Lodge Long-Term Care Homes and Services. City of Toronto reserves the right to accelerate work where advantageous to achieving an earlier completion date. This Project is subject to City of Toronto Council approvals. The milestone dates set out below for each phase may be adjusted, as necessary, based on the timing of the requisite approvals. Construction Start Date (on site) Anticipated to be on or about January 2 nd, 2018 and to be substantially completed no later than March 20 th, TOTAL COMPLETION OF THE WORK TO BE ACHIEVED NO LATER THAN March 30 th, The successful Contractor shall furnish all labour, materials, equipment and supervision to construct this project at the above site in accordance with this specification. This work shall include, but not necessarily be limited to the short summary below: 1. The successful contractor will act as the principal General Contractor and will secure all the necessary sub-trades and associated permits as required. 2. Assign one or more qualified site superintendents with more than 5 years experience typical of this type of work. The assigned site superintendent is expected to be on site, full time and for each shift and for the duration of the project while work is proceeding. 3. Thoroughly understand the base building standard and criteria, and thoroughly review and verify all data on site of the existing system and ensure buildings services are adequate for work. 4. Consult with Owner s Representative and/or Consultant to obtain/verify all necessary information for or any services required to implement the work. 5. Planning, scheduling, co-ordination and supervision of all sub-trades and work during the implementation of the scope of work. Ensure that sub-trades are co-ordinated in a manner to cause the least amount of disruption and shortest project duration. 6. Provide detailed schedule in MS Project format, indicating phases and completion within period specified. The schedule must be approved by the City prior to commencement of work. Update schedule as required or requested by the City of Toronto Project Manager to ensure accuracy of project deliverables. 7. Provide multiple crews, multiple shift work as required to achieve the designated Substantial Performance Dates. Site is available for work between hours however restrictions will be imposed for noisy and disruptive work as dictated by City of Toronto By-laws and by neighborhood agreement. 8. Cooperate with Owner and other Owner agents for general work and regular building functions for the duration of the construction period. Any anticipated interruptions shall be pre-arranged with occupants. 9. Comply with IPAC CAN/CSA-Z Infection control during construction, renovation, and maintenance of health care facilities. 10. Set up weekly construction meetings to review the progress and report back to the City of Toronto s representative. The contractor shall attend, conduct and take minutes of all meetings for PMMD V 3.1- June 18, 2010 Page of 7

The contractor shall co-ordinate with consultant to actively be present on site during any crucial service interface with the base building systems. 11. Provide market value pricing for Quotations in response to Contemplated Change Notices in a timely manner. 12. Submit monthly Progress payments with required City s documentations. 13. Protection of existing facility, and adjacent facilities, finishes, etc. during project. Any damages caused by the contractor to existing facilities will be the contractors responsibility to rectify 14. Environmental work is to be included in the General Contractor s scope of work, and in to be included in the base Contract price. 15. Demolition of existing building and new construction, systems as indicated in attached Construction Documents, including the approved MOH operational plans. (Attached) 16. Additions and revisions to all systems including Electrical Power, Systems and Lighting, HVAC, Sprinkler, Fire Alarm and Plumbing systems, Communications outlets/devices, Security systems and Exterior, work, Architectural and Structural modifications as indicated in attached Construction Documents. Include coring and Xray required for Plumbing/HVAC locate purposes if required. 17. Removal and disposal of existing finishes and remaining items in the building as indicated in attached Construction Documents. All materials/debris to be disposed of in a legal manner on a daily basis compliance required with respect to noise, dust, disruption, by-law compliance and neighbourhood aggreement. 18. Final and finishing clean, acceptable for handover to client prior to turn over. 19. Coordinate with Owner and Owner s subtrades such as locksmith, telecom contractors, security contractor, furniture and move vendors to carry out the work if required. 20. Coordinate and administer all necessary inspections for Authorities having jurisdiction including but not limited to ESA inspections, Building Department Inspections and signoff, Fire alarm Verification, and HVAC and plumbing inspections. The complete approvals of these agencies is the responsibility of Contractor, the City will provide necessary HVAC and Building Permits. 21. Provide close-out documentation (as builts ACAD2010 drawings and manuals) at end of the project. 22. Provide cost breakdowns as required to confirm backup information required by external funding sources. 23. The successful contractor will pre-order all materials and equipment for the project and will secure all required materials and equipment as per Phases inidicated above. 24. Outbreaks- Within all projects for City of Toronto Long Term Care Homes, outbreaks may occur during the course of work. In the event of an outbreak within the Home, the Contractor may be expected to suspend action onsite for a period of time. Be advised that no extras will be approved for work stoppage as a result of any outbreaks in the Home. 25. After Hours Work - All after hours work required for the completion of work as per contract documentation and as directed by the Home shall be included in the base bid price and will be carried out, as required, at no additional cost to the owner. 26. Project Area Specific Designated Substance Reports - Removal of designated substances, if required, will be carried out either in advance of the work of this contract or will be coordinated with a third party environmental contractor during the course of the work. Aside from site coordination with other parties performing the required work or the use of contract cash allowances to pay for such work, all designated substances shall be considered outside the scope of work of the General Contractor. 27. Parking - Limited parking for certain sites within the project may be permitted for Contractors and Sub-Contractors but will be provided in consultation with the Building Services Manager. It is possible that at certain times, no parking will be available. Loading and unloading will be permitted as defined below. PMMD V 3.1- June 18, 2010 Page of 7

Loading, Unloading, Storage of Materials - Selected building sites within the scope of work may have a limited amount of extra storage space within the building interior or exterior for use by contractors. This space will be allocated in consultation with the Building Services Manager for each location. Some sites will have no storage. Most building sites have loading areas and all loading and unloading of materials will be coordinated with the Home via the loading dock. Loading dock areas generally have access to the Service Elevator. The City of Toronto or individual project sites will not responsible for the safety and security of all materials delivered to and stored at the various project sites. 29. Make Good - Contractors should be aware and advised that it is expected that all surfaces affected by demolition or construction to be made good at the completion of the work. All make good work is expected match the existing condition. Make good work will affect walls, ceilings and floors at new equipment and removal of former equipment. All painting shall match existing colors. The Home is able to provide individual paint colors as all rooms are not the same. Repainting shall be completed to the nearest corner or edge (not spot painted). 30. Multiple Contractors - It is anticipated that one or more of the sites will have more than one contractor working on different parts of the site at one time. Bidders shall make allowance in their base bid for coordinating space and time separations between contractors in accordance with Ministry of Labour requirements. 31. Door Hardware and Required Installation a. The below statements are to clarify the responsibilities of each party within the Contract: i. Hardware is supply only ii. General Contractor; 1. Overall coordination, hardware installation and final system integration. iii. Electrical Subcontractor; 1. Provide and install all required conduit and wiring for specified systems. iv. Network System integration 1. Security Keypads will be by the City s integration vendor and will be provided by cash allowance. PMMD V 3.1- June 18, 2010 Page of 7
