The Horry County Solid Waste Authority, Inc. is requesting sealed proposals for the repair of the concrete tipping floor at the Material Recycling Center (MRF) located at 511 Three R Drive in Conway, South Carolina. All interested parties can obtain the RFP at Specifications are available at the Administrative Offices of the Solid Waste Authority located at 1886 Highway 90, Conway, South Carolina, or by calling (843) Proposals will be received at the Administrative Offices until 3:00 P.M., December 6, Any proposal received later than the specified time will NOT be accepted/ considered. All proposers are responsible for monitoring the website for any changes or addendums. 2 HORRY COUNTY SOLID WASTE AUTHORITY INSTRUCTIONS TO PROPOSERS 1. This request for proposal includes the following: A. Instructions to Proposers B. Specifications C. Proposal Form (s) 2. Proposal (s) will be examined promptly after opening; immediately thereafter, all proposals will be tabulated with said tabulation being made available to all participating. It is not a practice to award any proposal until the Authority has had ample time to review each Proposal. Award will be made however, at the earliest possible date. If the mail is delayed beyond the date and hour set for the proposal opening, Proposal (s) thus delayed, will NOT be considered. Any Proposal not present at appointed time will not be opened. Facsimiles will not be accepted. The proposal number must be clearly visible on the outside envelope. 3. TAXES - The Authority pays South Carolina State Sales Tax. The Authority is exempt from Federal Excise Tax and will issue exemption certificates as requested. 4. AWARD OF PROPOSAL - Award of Proposal shall be made to the lowest responsible proposer meeting the Specifications, taking into consideration the following: A. Superior Quality B. Adequate Maintenance and Service C. Past Experience with Company's Equipment D. Company's Reputation E. Known as Technically Good and Innovative Company F. Delivery Date G. Guarantees/Warranties H. Performance of Proposer s equipment in hands of other agencies, plants, and firms. 5. Each proposer must submit a proposal (s) on the blank form attached. The Proposer shall sign his proposal form correctly and proposal (s) may be rejected if there are any omissions, alterations of form, additions not called for, conditional proposal or any irregularities of any kind. 6. DEVIATIONS - Any deviations from these Specifications MUST be noted in detail and submitted in writing with this PROPOSAL. The absence of this Specification deviation will hold the Proposer strictly accountable to the Specifications as written herein. Failure to submit this document of Specification deviation, if applicable, shall be grounds for rejection of the item (s) when offered for delivery.

CHANGES - Any changes in Specifications after the Purchase Order/Contract has been awarded, must be with the written consent of the Authority; otherwise, the responsibility for such changes shall be with the Vendor. 8. DELIVERY- Complete unit (s) shall be delivered to the Horry County Solid Waste Authority, 1886 Highway 90, Conway, South Carolina. 9. PAYMENT- The Solid Waste Authority will be invoiced after delivery of equipment or completion of project, the payment terms for this invoice shall be Net INFORMATION All questions must be submitted, in writing to Ms. Donna Crump, Deputy Director of Finance and Administration, Fax number Proper reference to this proposal is required. Deadline for questions is seven- (7) calendar days prior to proposal opening date. Questions will be answered in writing and a copy will be forwarded to all vendors who received a proposal package. All changes in specifications shall be in writing and furnished to ALL PROPOSERS. Verbal information obtained otherwise, will NOT be considered in awarding of proposal. 11. BID BOND Intentionally left blank. 12. PERFORMANCE BOND The successful proposer will be required to furnish a Performance Bond in the full amount of the contract sum, issued by a bonding company licensed to do business in South Carolina and, on the approved list of the Treasurer of the United States, or provide a cash bond in the full amount of the contract sum. 13. RETURN OF CHECKS When proposals are awarded, the Authority will return immediately all checks, except those of the successful proposer (s). 14. The Authority reserves the right to reject any or all Proposals. It further reserves the right to waive technicalities and formalities in proposals as well as to accept in whole or in part such proposals or proposals where it deems it advisable in protection of the best interests of the Authority. 15. INSURANCE: The successful proposer shall be required to maintain General Liability Insurance in the amount of One Million Dollars ($1,000,000.) and maintain Workman s Compensation Insurance. The contractor shall be required to submit a copy of proof of insurance prior to commencement of work. 16. Any material included with the proposal becomes the property of the Authority and are not returnable. 17. Any Proposer aggrieved in connection with the solicitation or award of a contract may protest to the Executive Director in writing within 14 days after such person knows or should have known of the facts giving rise to the grievance.

5 SPECIFICATIONS FOR MATERIALRECYCLING FACILITY TIPPING FLOOR REPAIR INTRODUCTION The Horry County Solid Waste Authority, Inc. (SWA) is requesting sealed proposals for the repair of a portion of the concrete tipping floor at the Material Recycling Facility (MRF) located at 511 Three R Drive in Conway, SC. BACKGROUND The SWA constructed a new Materials Recycling Facility (MRF) approximately 48,000 square feet in and began operations on March 10, In 2010 approximately 1400 square feet was repaired. The MRF processes approximately 60,000 tons of curbside recyclables annually. The facility accepts recyclables consisting of Paper, OCC, Plastic containers, Aluminum and Metal cans and Glass. The facility is open for business five days a week Monday Friday from 4:00AM 1:15AM, and Saturday 6:00AM -11:00AM to receive recyclable products. DESCRIPTION OF WORK The SWA began experiencing excessive concrete floor wear on the tipping floor in October 2008, and this floor wear has continued deteriorating. The SWA is seeking a Request for Proposal (RFP) from qualified and bonded concrete repair and/or installation companies for repair of the Materials Recycling Facility (MRF) tipping floor. The successful proposer shall meet all the requirements of all local, state, and federal laws, rules and regulations, and licensing requirements applicable to this project. All permits required for installation and construction will be the responsibility of the successful proposer. For this project, the Proposer/Contractor shall be qualified, bonded and capable to provide or obtain the following services. 1. The qualified proposer/contractor will provide a detailed proposal on their strategy on how best to repair the floor. 2. The qualified contractor will apply new material that will be consistent with an iron aggregate or equivalent type industrial high strength floor topping attaining a minimum strength of 14,000 psi at 7 days and 18,000 psi at 28 days to withstand the harsh conditions of a MRF and/or Transfer Station handling approximately 60,000 tons per year and have a life expectancy of a minimum seven years. 3. The tipping floor repair area is estimated to be approximately square feet (see attached diagram). Proposer must determine the exact square footage of deteriorated area to be repaired. 4. The contractor shall furnish all labor, materials and services necessary for and incidental to the completion of all work as indicated by the owner and specified herein. The contractor shall provide all necessary equipment for removal and installation of a high strength reinforced concrete tipping floor. All machinery and

6 equipment owned or controlled by the contractor, which he proposes to use on the work, shall be of sufficient size to meet the requirement of the work and shall be used such as to produce satisfactory work. The contractor will be responsible for the storage and security of all equipment. No SWA personnel or equipment will be utilized in the performance of the scope of the work to be completed. 5. Contractor will be responsible for all clean up and proper disposal of excess materials and waste. PLAN FOR COMPLETION Work will preferably begin on a Friday afternoon at 3:30PM and be completed and ready for full operations the following Monday morning at 4:00AM. Please provide detailed work schedule to include the number of days needed to start project after date of award (purchase order), how many days will be needed to complete project and estimated number of hours to perform daily work. EXPERIENCE/REFERENCES Successful Proposer/Contractor must have a minimum of seven (7) years experience and a minimum of five (5) references of either Material Recycling Facility and/or Transfer Station tipping floor repairs and/or new construction submitted for verification. WARRANTY The successful proposer will provide a two (2) year warranty on all materials, labor and workmanship. PROPOSAL PRICING All proposals must provide a detailed/line item square footage price list for all materials, bonding agents, curing compounds, fillers, labor, etc. Please provide the length of time the pricing will be in effect. Respondents shall submit one (1) Original and four (4) copies of written responses to this request for Request for Proposal.

Also provide a detailed/line item square footage price list for all materials, bonding agents, curing compounds, fillers, labor, etc. (Price per square foot, so HCSWA can determine how much square feet can be repaired) 2. PROPOSAL PRICING DURATION: Please provide the length of time the pricing will be in effect. 3. PLAN FOR COMPLETION: Please provide detailed work schedule to include the number of days needed to start project after date of award (purchase order), how many days will be needed to complete project and estimated number of hours to perform daily work. 4. REFERENCES: Please provide a minimum of five (5) references of either Material Recycling Facility and/or Transfer Station tipping floor repairs and/or new construction projects. 5.
