Replace the As Per Plan Note: Item 619 Field Office, Type C, As per Plan on sheet 45/1771 with: Item 619 Field Office, Type C, As per Plan This item shall be in accordance with item 619 of the 2013 ODOT CM&S except as Modified by the following: 1. The proposed field office has been revised to combine the field office for both projects LUC and LUC into one field office. This field office will be for the exclusive use of the Department and their Consultants for the duration of the Contract. The payment for the use of this field office will be contained only within LUC , but the Contractor is being advised that ODOT and Consultant personnel will be utilizing the field office for both projects, including all necessary supplies for printer, utilities, lavatory and sanitary. 2. A minimum of six different options and locations (2 per location as described below) that are at least 1 mile apart from one another shall be provided to the Department for review prior to acceptance of the Department s preferred choice. The field office locations shall be located within the following footprints: A. Located in the area along I-75 between State Route 65 (Miami St.) and Buck Rd. and within a 1/2 mile east or west of this I-75 corridor. B. Located in the area along I-475 between Monroe St. and ProMedica Parkway and within 1 mile north or south of this I-475 corridor.

5 C. Located in the I-75 & I-280 interchange area located south of I-75 and West of I- 280 while within ½ mile of access to either I-75 or I-280 via Expressway Drive or Manhattan Blvd. Locations outside of these footprints and criteria will not be accepted unless approval is received by the Engineer. The field office location selected shall be reviewed and accepted by the Engineer prior to the Contractor signing any lease or procuring any facilities or services for the field office. 3. The field office requirements for moisture and density control of construction materials shall still be satisfied, however with the following modifications: The facility shall be capable of storing up to 3 nuclear density gauges and the concrete curing box shall be capable of holding at least five 3 gallon size buckets or Eighteen 4x8 concrete cylinders. 4. The field office shall provide an increased broadband internet connection capable of meeting the minimum requirements as follows: 50mbps download and 5mbps upload Network latency less than 50 milliseconds. This shall include any modifications that may be necessary to the field office building as required by the internet supplier in order to accommodate the initial service and setup. 5. A conference room shall be supplied with a minimum of 600 sq. ft. of floor space. The room shall be supplied with tables and chairs capable of seating a minimum of 30 attendees. The room shall include a phone line, data connection port and a wall mounted projection screen that is 95 diagonal or larger. 6. Overall office floor space to be provided shall be increased to a minimum of 6000 square feet including the conference room. The space shall be continuous and within the same building. 7. The requirement of one separate room shall be increased to a minimum of 10 separate enclosed rooms of 150 square foot minimum per room. Each room shall be supplied with a minimum of two electric outlets and one data connection port. 8. The all-weather parking spaces provided shall be increased to 60 and include snow removal. 9. Security shall be provided for the field office and surrounding parking facilities by illuminating all the sides of the field office and parking area. 10. The number of telephones to be supplied by the Contractor shall be increased to 5 phones. 11. The number of desks and chairs to be supplied by the Contractor shall be increased to 15 sets. 12. They also want to put the geomembrane on top of the distribution slab instead of underneath it. There is no way for the contractor to realistically estimate the cost of this work and carry it as incidental to the pertinent items. A1: See revised note on sheet 44 Q2: October 31, 2014 at 11:52:11 AM: Concerning cast stone requirements for Part 3. The plan note on sheet 215 states that Cast Stone units shall be manufactured within 500 miles of the Project Site. Both specified manufactures, ARRIS.cast (production facility: Forty Valley, GA 750 miles) and Haddonstone (production facility: Pueblo, CO 1,300 miles) do not meet this Specification. Is the contractor to use only these two manufacturers, (even though they are both outside of the 500 mile range) or will other manufacturers be allowed? A2: The distance requirement has been removed. See revised sheet 215 Q3: October 30, 2014 at 5:35:21 PM: Where can we access a copy of the Geotechnical Subsurface Investigation Retaining Structures Report referenced on page 757..? A3: The geotechnical report referenced does not exist. This note will be deleted from the plans. See revised sheet 757 Q4: Tuesday, October 14, :00 AM: Plan sheet 61/1771 shows temporary shoring. The note on the same page states that the temporary shoring is to be paid for under 503 Cofferdams and Excavation Bracing. There is no bid item for this work. Please add a bid item in the next addendum. A4: See additional pay item above Q5: October 31, 2014 at 8:31:22 AM: 1. Please provide more detail for Section A shown on sheet 748/1771. Section A indicates that the upper third of the retaining wall for Ramp NC-2 should be backfilled with low permeability embankment fill. Section A also indicates that the quantity of L.P.E.F. is included with the roadway quantities. Currently this volume of material is included with the roadway quantiies as embankment as there are no bid items setup to place low permeability embankment fill in the Roadway quantities. Please clarify whether or not the detail for the retaining wall is correct or the cross sections and bid items in the roadway items are correct. A5: The backfill detail for Wall 102 is correct. The roadway earthwork quantities adjacent to Wall 102 will be revised in the next addendum to reflect the geofoam shown in the wall plans, and the estimated volume of low permeability embankment fill will be provided. 7 Q6: Mon 10/27/2014 6:25 PM: Addendum #6 revised several concrete sealing items to be As Per Plan (Ref. 423, 480, 510, 528, 558, 593, 628, 655, 800, and 842) and added a note stating that permanent graffiti protection was to be included in each item. This is also the case for Ref. 447 mentioned in the previous question. This adds significant cost to these bid items. ODOT normally bids graffiti protection as a separate bid item. In order to avoid confusion and bidding mistakes, please add bid items for permanent graffiti protection instead of making it incidental to Epoxy-Urethane sealing. A6: This will be modified in the next addenda to an additional item 512E75500 Sealing Misc.: Permanent Graffit Protection. The total for the 12 structures is approximately SQ YDs. Q7: 10/22/2014 8:43:40 AM Please confirm the use of polymeric sand for Bid Item 1162: Misc. Recycled Brick Pavers. The details shown on sheet 188 of 234 appear inconsistent. Detail 1 shows using polymeric sand while details 2 and 3 shows using screenings. What material is required? A7: Detail #1 is correct. A8: See revised note on sheet 44 Q9: Tue 10/14/2014 2:29 PM: Part 1, reference mm surface course, calls for 3 cy of material on Detroit Ave. The Part 1 office calcs show 3 cy of 19mm PG70-22 intermediate course on Detroit Ave. Sheet 261/1771 says to see Part 3 for Detroit Ave work. Part 3, sheet 55/234 says to see Luc-Detroit Ave for the adjoining project. What are the 6 cy of material on Detroit Ave for? A9: Part 1 plan sheets 35 and 261 show the removal and replacement of a 3-foot width of existing pavement, curb & gutter and 5-foot wide sidewalk along the westerly side of Detroit Avenue to remove the existing bridge pier and construct the foundation of pier #20 for the proposed LUC L/R bridge. The total 6 CY of asphalt surface and intermediate course is for that pavement replacement.revised sheet 261 is attached Q10: Tue 10/14/2014 1:59 PM: Phillips Ave, Part 1 calls for a 448 Type 1 surface course. Phillips Ave, Part 3 calls for a mm surface course. What type of surface course will be used on Phillips Ave?
