All other conditions remain the same. Acknowledge receipt of this Addendum in the space provided on the Bid Form. Failure to do so may subject Bidder to disqualification. 1. Project Schedule David C. a) Project must be completed within 45 calendar days or earlier b) Every calendar day is a working day c) Tuesday, July 17 Anticipated contract award d) Monday, July 23 Anticipated contractor s first working day e) Wednesday, September 5 Anticipated contractor s last working day f) Wednesday, September 5 Anticipated on-site delivery of the prefabricated modular lab g) Thursday, September 6 Anticipated first day to attach the modular lab to the foundation 1

Project Sequence David C. a) The County intends to issue a Notice of Intent letter to the responsive, responsible apparent low bidder prior to reward of contact that will request the following: i. Submit insurance and performance bonds prior to contract award ii. Prepare submittals for consideration by the County prior to the award of contract including: - Concrete Mix Design foundations and ramps/hardscape - Concrete Reinforcing Shop Drawings - Truncated Domes - Asphalt Concrete - Canopy Steel Shop Drawings - Miscellaneous Steel Shop Drawings railing, embeds, grating, bollards, chain link fencing - Conduit and General Materials - Generator - Automatic Transfer Switch b) The contractor s submittals will be reviewed by the Engineers and the County and it is anticipated that long lead-time equipment, such as the generator and automatic transfer switch, will be ordered as one of the very first contractor activities upon the execution of the construction contract. c) If the above proposed project development strategy is followed, it is anticipated that the generator and transfer switch or any other long-lead time equipment may be delivered to the Project site on or about Friday, August 17. d) The contractor would then have approximately 2 ½ weeks to complete the remainder of the work including the installation of the generator and transfer switch. 7. Project Requirements David C. a) The project has significant funding mandates that require the timely completion of the project. 8. Building Permit David C. a) County Building Department Permit Counter Plan Review is underway b) Building Department permit review comments, if any, are to be issued via addendum. c) Contractor will secure permit(s). Anticipated permit costs are to be disclosed via addendum. 9. Special Considerations David C. a) Special testing and inspections are contained in the Bid Documents and may also be required by the Building Department; and the contractor will secure and pay for these services. For bidding purposes, the contractor shall assume these costs to be $3,000. b) The contractor may need to secure an Air Pollution Control District permit to construct for the new generator. If so, the anticipated permit costs are to be disclosed via addendum. c) The existing main lab is an access controlled space and entry into the main lab will not be allowed during the Prebid Conference. 2

4 i. Domestic water and sanitary sewer connections for the modular lab will be made by the contractor to an existing sink located on the north wall. Prospective bidders may view the north wall exterior area during the site tour. Pictures of the main lab interior in the area of the existing sink, to the greatest extent possible, will be provided via addendum. ii. The Project assumes the re-use of an assumed abandoned conduit for communications and low voltage. The conduit on the northeast exterior corner of the main lab building will be viewable during the site tour. Plans showing the type and path of travel for the existing conduit inside the main lab space will be provided via addendum. iii. Public Health Services staff is currently evaluating the main lab access controls that may be in place during the contractor s work efforts in the main lab. This information will be provided via addendum. II. ADMINISTRATIVE David C. 1. Mandatory Pre-bid Conference for Prime (General) Contractors (Sub-contractors welcome). 2. Required California Contractor licenses. a) A-General Engineering Contractor b) B-General Building Contractor c) C-10 Electrical Contractor 3. Warranties are noted in the contract documents and shall be provided. a) Bonds in full force during warranty period. 4. Prevailing Wage Job with certified payroll submission upon request. a) Contractor responsible for determining appropriate rate. b) Questions should be directed to the State of California Director of Industrial Relations. 5. Contractor should pay attention to the RFI process. 6. Contractor should pay attention to the Propose Change Order process. 7. Change Orders a) Three types- Owner s Discretion i. Lump sum negotiated ii. Unit price iii. Time and Materials 8. Mark-ups are limited, contractor to review a) If mark-ups don t cover additional overhead, need to include additional bid contingency. 9. Payments a) Monthly-Submit 25 th day b) Retention is 5% until after final acceptance and the conclusion of a 35- day Lien Period. 10. Claims and Disputes a) Review formal claims procedure 11. Contract documents are final authority and arbiter of disputes. Please review the contract documents in their entirety. 3

5 III. BIDDING 1. Viewable, downloadable, and printable Bid Documents are on the County Purchasing Project website. Bid Documents are also available at Stockton Blue/PlanWell. 2. Bid Form Fill out properly, completely, and clearly. a) Clearly fill out bid with both written and numeric cost. b) Acknowledge receipt of addenda. c) Subcontractor information. d) Contractor information. e) California License number 3. Local Hire Checklist a) Contracts estimated by the County to be less than $200,000 do not have a local hire requirement. b) Contracts estimated by the County to be $200,000 or more require the successful bidder to demonstrate their attempts to employ local hire. 4. Certification of a Drug-Free Workplace 5. Noncollusion Affidavit 6. Bid Bond, or Cashier s Check, or Certified Check IV. BID DOCUMENTS David C. 1. Available for viewing, downloading, and printing on the County Purchasing Project website. 2. Available for viewing and/or purchase at Stockton Blue/PlanWell. 3. Addenda, if any, will be posted on the Purchasing website. 4. It is the interested prospective bidder s responsibility to check the County s Purchasing website for any addenda prior to the submittal due date. 5. On Friday afternoon, June 22, the Bid Documents will be removed from the Purchasing website as an Open Bid and the Bid Documents will no longer be viewable, downloadable, or printable on the Purchasing website.
