SPBSR-17 BPA PROJECT NO ADDENDUM NO. 2 SPITZER CONFERENCE CENTER BRIEFING SUITE REFRESH LORAIN COUNTY COMMUNITY COLLEGE This Addendum supplements and amends the Interior Renovation of the SPBSR-17 project dated February 1, 2018 for LCCC. This addendum consists of additional written information, modifications to the project manual, and modifications to the drawings as follows. Wherein this Addendum is in conflict with the Specifications and Drawings, the requirements of this Addendum shall govern. Acknowledge receipt of this Addendum in the space provided on the Bid Form. Failure to do so may subject the Bidder to disqualification. ITEM NO. 1 ADDITIONAL WRITTEN INFORMATION RFI QUESTIONS AND RESPONSES 1. Sheet E500 mentions a B faceplate type and to refer to sheet E503 for rough-in details, however there is no B faceplate detail on sheet E503. Is the B faceplate rough-in only, or is there a cable schedule and faceplate detail? Refer to updated detail in Addendum 1 2. LCCC normally does not use a Category 6A data/voice cabling infrastructure, and they use a yellow cable for data and voice. Is the A faceplate detail on sheet E503 correct, and if so is position D to be a shielded module? Refer to updated detail in Addendum 1 3. What is the data/voice cable schedule for the floor boxes shown in Briefing Room 106? Refer to sheet E501 in Addendum 1. Data in floorboxes shall be Type A style. 4. In the Briefing Room 106 there are (2) projector junction boxes shown on sheet E100. The projector symbol show sheet E503 does not appear on sheet E100, however a projector is shown on sheet A300. Are there data/voice requirements for a projector in Briefing Room 106? Refer to updated E100 in Addendum 1. No data/voice requirements for projector. 5. Who is installing the owner provided speakers? Is there a wiring requirement for the speakers, either to an AV/sound cabinet or a paging headend? Owner to install and wire speakers as required. 6. It is stated in the spec's regarding the OSHA Training needed for this project. Do I have to supply this with my bid packet (will be very hard to achieve on bid day). Or If I am the successful bidder can I provide then? ANSWER: Please refer to Item 9 of the Bidder s Qualifications, which reads in part: The College will accept as documentation at time of Bid Opening: copy of employees OSHA card or proof of OSHA training in process, with completion date clearly shown. ALL of the Bidder s workforce MUST have successfully completed, and have documentation for, 10 hours of OSHA safety training prior to reporting to the Project site, and construction supervisors, or those with safety-related responsibilities have to provide documentation for 30 hours of OSHA safety training prior to reporting to the Project site.

SPBSR-17 BPA PROJECT NO Due at time of bid: a copy of their card or proof of training, w/anticipated completion date is required Due at time of project start (before any employee can be on site): Proof of successful completion of training is required to be followed w/a copy of the OSHA card upon receipt. Anticipated Notice of Intent to Award is March 28 (3 business days after board of trustees meeting). For a target, of when all documentation is needed, the project start would be approximately (2-3 weeks) after that April when a fully executed contract is in place. FURTHER CLARIFICATION: General Contractor is to provide OSHA documentation for their own workforce as stated in Item 9 of the Bidder s Qualifications. OSHA documentation for sub-contractors is to be provided at the time General Contractor submits their Subcontractor and Material Supplier Declaration form (Article 4 Subcontractors of the General Conditions), which is expected to be provided to the A/E and Owner within 10 days after the Notice to Proceed is issued. The documentation (OSHA 30 for supervision and/or safety personnel, OSHA 10 for tradesmen) is required before sub-contractor can perform work on site. Drawings and general provisions of the Contract, including General and Supplementary Conditions and other Division 01 Specification Sections, apply to this Section. 1.2 DEFINITIONS A. Alternate: An amount proposed by bidders and stated on the Bid Form for certain work defined in the bidding requirements that may be added to or deducted from the base bid amount if the Owner decides to accept a corresponding change either in the amount of construction to be completed or in the products, materials, equipment, systems, or installation methods described in the Contract Documents. 1. Alternates described in this Section are part of the Work only if enumerated in the Agreement. 2. The cost or credit for each alternate is the net addition to or deduction from the Contract Sum to incorporate alternates into the Work. No other adjustments are made to the Contract Sum. 1.3 PROCEDURES A. B. C.
