Failure to do so may subject bidder to disqualification. This addendum forms a part of the bid documents as follows: 1. Changes to Special Provisions. The following Sections shall be deleted from the Special Provisions and replaced with the attached pages: Section Scope Section Measurement and Payment Section Construction. 2 SCOPE Cold milling shall be performed in accordance with Section 404 in the Standard Specifications. The existing pavement shall be cold milled as indicated on the drawings. The Work shall include removal of existing pavement to the specified thickness and creating transition wedges at the edges of overlays. Pavement adjacent to existing gutters, median curbs, cross gutters or at the ends of overlays, at equipment crossings and at bridge approaches shall be cold milled as specified herein. Limits of cold milling will be determined by the City Engineer prior to construction. The depths and dimensions of the cold milling and/or key cuts are designated on the Plans. In some cases the required depth or locations designated may include the cold milling of Portland Cement Concrete. Pavement to be cold milled may contain pavement fabric. Cold planing/key cut asphalt concrete pavement includes the removal of pavement markers, traffic stripes, and pavement markings within the area of cold planing. All loose pavement fabric left in between pavement after milling shall be removed. Prior to cold milling the roadway, any City owned affected storm drain or sewer manhole covers, survey monument covers, water meter vaults and valves covers, or other utility service structure covers shall be lowered below the proposed cold milling depth to facilitate cold milling. Refer to Section of Special Provision for the procedure to be followed by the Contractor for the adjustment of utility covers. All City owned structures from which frames and covers have been removed to facilitate milling shall be temporarily covered with a steel plate by the Contractor. A temporary wedge patch of cold mix asphalt shall be placed at all transverse keycuts or cold milled areas on the same day they are cold milled to reduce and minimize the abrupt change of pavement profile. The minimum wedge length shall be 5 feet of wedge length for each 1 inch of vertical drop-off or grade differential. On longitudinal mills where pedestrian or vehicular traffic can be expected such as driveways and handicap access ramps, a temporary wedge patch of cold mix shall also be placed on the same day as the cold milling is performed. All cold patch wedges shall be a minimum of 2 feet wide or wider, if necessary, for the public safety. The Contractor shall maintain the wedges until permanent paving is installed. All areas which are cold milled shall be finished paved within fourteen (14) Calendar Days. Asphalt paving and/or overlay procedure including asphalt concrete quantities are covered in Section 1150 of these Special Provisions.

3 The cold wedges shall be installed immediately after cold milling and cleaning of cold milling debris. Traverse cold wedges shall be reviewed daily by Contractor and maintained on daily basis. If failure to place or maintain cold wedges causes a public hazard, the City Engineer shall have the right to correct any location at any time without the Contractor s notification, consent, or agreement. Contractor shall be responsible for all costs incurred by the City to eliminate a public hazard caused by the Contractor s failure to properly maintain the specified cold wedges.

4 MEASUREMENT AND PAYMENT Cold milling will be measured by the linear foot (LF) along the edge of the adjacent curb, gutter, cross gutter, or pavement, or by the square yard (SY) as shown in the bid. Payment will be made at the unit price per Bid Sheets on the basis of actual measured quantities of cold milling completed in accordance with the Plans and these Special Provisions. Such payment shall be considered full compensation for providing all labor and equipment and performing all Work related to cold milling, including removal of pavement markers/striping/markings, dust control, lowering of manhole/vaults/valve/survey monument covers and disposal of the removed material.

5 CONSTRUCTION The Contractor shall provide a temporary false floor for manholes while the covers are being adjusted either up or down through the course of the Project. The false floor shall prevent construction debris from falling into the manhole chamber or pipeline. The Work shall conform to Section 403 of the Standard Specifications. All water valve covers, water meter boxes, survey monument covers, and sewer terminal cleanout, including the concrete box, frame and cover, and metal lid, shall be replaced in accordance with the requirements of the Plans and in conformance to the City of Oxnard's Standards at the Contractor's expense. Immediately after covering the water valve or manhole with asphalt during the paving operation, the Contractor shall locate the facility and inscribe an X into the pavement to facilitate the uncovering of the facility in an emergency. Additional survey monuments adjacent to the Streets to be overlaid shall be adjusted as directed by the City Engineer or as shown on the Plans. The concrete collar around manholes, water valves, and survey monuments shall be left 2" lower than the adjacent pavement. The surface shall be tack coated with 0.10 gal/sy and paved with Type C-2 asphalt concrete. Immediately after placement, the surface shall be sand sealed. All affected interior surfaces of sewer manholes raised to grade shall be coated with a protective coating per City Standard Plate 400, Sheet 2 of 2, Note 3. All sewer manhole frames and covers shall be adjusted within 72 hours after the placement of paving. If several lifts of pavement are to be placed, the sewer manhole frames and covers shall be raised if the paving operation ceases for more than 72 hours. All other covers, frames, grates, monuments, and so forth shall be adjusted within five (5) Working Days of final paving of each individual street segment. All asbestos cement valve risers (valve stacks) shall be removed prior to pulverization or reconstruction and replaced with 8 diameter PVC risers. Cutting, abrading or breaking ACP shall be avoided. ACP risers shall be kept wet with amended water and wrapped securely in plastic sheeting. The Contractor shall store ACP risers in an area that is secure from public access until properly disposed. ACP risers shall be disposed of in compliance with Ventura County Air Pollution Control District (APCD), State, and federal regulations.
