1 Making Modifications within your Residence If you are planning any modifications in your new residence at Galloway Ridge, please be sure to follow procedures detailed on following pages. Galloway Ridge uses outside vendors for most refurbishing needs. A list of already approved vendors is provided in this packet who have performed good work for us in the past and are familiar with our policies. You may use other vendors as long as you/they adhere to the approval process. Changes include items such as: a. Custom painting b. Custom closets or built-in s c. Installing window treatments d. Electrical modifications such as i. Adding ceiling fan ii. Adding or changing out an overhead light fixture iii. Adding electrical, cable, or phone outlets e. Carpentry modifications f. Plumbing modifications g. Flooring modifications h. Adding glass shower doors i. Countertop modifications j. Landscaping modifications k. Removing an interior door from hinges so that new residence has a more open feel (resident is required to store door so it can be re-installed when resident vacates this location) l. Any other change you may be considering Galloway Ridge approval is needed when making ANY changes or modifications. Residents who already reside at Galloway Ridge, please complete and return Modification Request form to Director of Facilities. Special Note for incoming residents: if making modification(s) prior to your settlement, please know that modification work is non-refundable. If something should happen and you do not settle and move to Galloway Ridge you may be required to return the residence to original standard. 3 Approved Outside Contractors Reminder that a Modification Request form is to be used when making any changes to your residence. All changes must be approved by the Director of Facilities before work begins, even if you are doing the work yourself. The following vendors are already approved to work at Galloway Ridge. You may use other vendors as long as you/they adhere to the approval process and supply Galloway Ridge with a certificate of insurance. Outside Contractor Insurance Requirements All outside contractors must be pre-approved by Director of Facilities Operations to be able to work for Residents of Galloway Ridge. All Contractors must: 1. Provide a Certificate of Insurance (see below for Certificate requirements) 2. Complete contact information form and sign that you have reviewed the Outside Contractor Guidelines and will comply Once we have received your Certificate of Insurance with all requested information, Galloway Ridge will contact you to confirm whether or not you have been approved to work for our Residents. All contractor work must be pre-approved before any work begins. It is the responsibility of the Contractor to make sure that the Resident has received written approval from Galloway Ridge, that proposed custom work has been approved, and that Resident has given consent that Contractor may enter the residence. Contractor Certificate of Insurance Requirements: 1. General Liability $1,000,000 minimum, with a $2,000,000 aggregate 2. Worker s Compensation (1) if you are a sole proprietor, partnership, LLC, estate or trust, and employee three (3) employees in addition to the sole proprietor, partners, formulators of the LLC, executor of the trust and bearer of the trust. It does not matter if these employees are full time, part time, regular seasonal or family members. If you are incorporated, including all forms of corporations, and those which have non-profit status, you are required by law to carry coverage once you have a total of three (3) people in the corporation. Everyone is included, including corporate officers. Worker s Compensation - (2) does not apply if contractor is a sole proprietor with fewer than three (3) employees. 3. 6 Resident, please give this form to your contractor. Outside Contractor On-Site Instructions & Guidelines 1. All contractor work must be pre-approved by Galloway Ridge before any work begins. It is the responsibility of the contractor to make sure that the Resident has received written approval from Galloway Ridge, that the proposed modification / custom work has been approved, and that the Resident has given consent that the contractor may enter their residence. 2. Each time contractor is onsite at Galloway Ridge to preform modification / custom work, they must sign in at the Front Desk in the main building. When signing in you must obtain a Galloway Ridge name badge, print your name on the name badge, and wear the name badge so that it is clearly visible at all times. You will also need to sign out at the Front Desk when leaving Galloway Ridge. 3. DO NOT prop open any exterior doors. All outside doors are kept locked at all times. 4. If you are bringing small items or equipment with, you can bring them through the Front Lobby. If you need to unload larger items then you will need to arrange for assistance and unload at the loading dock. 5. If you are conducting business in the main building for less than one hour, you may park a small company vehicle (pickup truck or mini-van) outside the Front Entrance in one the Visitors spaces. If longer than one hour, you will park on the backside of the property beyond the end of A-Wing or in overflow parking lot just beyond horseshoe parking lot by Main Lobby. You may need to sign out a Security Card if you need to enter and exit the building throughout the day. You will be required to leave your Drivers License at the Front Desk while using the Security Card. There is a $ per day fine if you do not return Security Card. 6. All modification work done inside a residence is to be cleaned up by the Contractor. There is to be no sawing inside a residence, all sawing must be done outside. Galloway Ridge will charge Contractor if construction cleaning is not done. Please remember to clean all surfaces after painting, wallpapering, closet work, electrical work, window treatment installation, new flooring installation, or any other work. In addition to a construction cleaning, if Galloway Ridge determines that the HVAC duct work must be cleaned, the contractor will be charged. This is to insure that the residence is protected from construction dust re-circulating and settling within the residence. Contact Galloway Ridge Facility Operations at (919) to arrange for all HVAC cleaning. The fee for this service when directly related to construction work is a minimum $ You must provide your own means of moving your equipment or items too heavy to carry by hand. Heavy equipment cannot enter through Lobby and will need special access. Galloway Ridge does not provide moving equipment or hand carts. 8. Public Restrooms are on the Terrace Level in D Wing, First and Second Levels of E Wing, and Levels 1, 2, and 3 in J/K Wing. 9. There are no public restaurants at Galloway Ridge, so please bring your own refreshments. 10. All contractors are to be paid directly by Resident. 11. Galloway Ridge is a smoke free campus.
