Please complete schedules and Health and Safety Forms HS4 attached hereto and return with cover page by due date noted above. All works are to be completed as per the satisfaction of the Town of Midland. Work done in accordance with O.P.S.D., O.P.S.S. and Town of Midland standards. Earliest possible start date: April 27, 205 Work is to be completed by: June 25, 205 Contractor is responsible for applying for energy grants on behalf of the North Simcoe Sports and Recreation Centre. No deviation from these requirements will be permitted. If the Contractor wishes to change a requirement, it must be approved by the NSSRC Operations Manager. 2 204-0 TWO (2) SEASONAL CONTROLLERS.0 GENERAL. Work under this Contract covers the upgrade and/or replacement of the Refrigeration Plant Condenser System. This includes, but is not limited to, the reinstatement of all connections to the panel in accordance with the specifications. Also additional new equipment may be provided as noted in the specification. It is the responsibility of the contractor to coordinate all components of the project and defined herein..2 The specification is not intended as a detailed description of installation methods, but serves to indicate particular requirements of the completed work..3 It is the intention of the specification to provide finished work. Any items omitted there, which are clearly necessary for the completion of the work or its appurtenances shall be considered as portion of the work..4 Contractor is responsible to review the existing system and equipment within the refrigeration plant and include for all necessary devices to complete the installation. Coordinate requirements with electrical contractor if required..5 Contractor shall provide a fully functional control system to safely operate the refrigeration plant condenser system based on a floating head pressure design. 2.0 REFERENCES 2. CAN/CSA B52-99 Mechanical Refrigeration Code, latest revision. 2.2 Ontario Electrical Safety Code, latest revision. 2.3 ANSI/ASME Boiler and Pressure Vessel Code, latest revision. 3.0 MAINTENANCE 3. Provide copy of operation manual for new equipment installation. 4.0 WARRANTY 4. One year parts and labour warranty from start-up date. Including 2 hour emergency response time in the event of failure of the system.

Provide grant application services where applicable. 5.2 Contractor must have at least 0 previous installations approved by the Ontario Power Authority. 5.3 Design, provide and commission all the accessories necessary, the software, the relays, transmitters, the control panels and others for the complete operation of a DDC based dedicated floating head pressure control system. 5.4 Disconnect power to existing control panel and equipment to allow for safe upgrade of controller. 5.5 The contractor as part of the refrigeration contract is to supply and install a floating head pressure black box based automatic control system c/w 0.4 Color Touch Screen to achieve the performance specified in the following clauses. 5.6 The system shall not require any on-site programming. 5.7 The system shall be programmed to safely and efficiently operate the refrigeration plant condenser system based on a true floating head pressure system. 5.8 Coordination of controller address shall be by the owner if applicable. Each controller shall be assigned a unique BACnet address. IP addresses shall also be provided for where required. 5.9 The refrigeration contractor shall have an in-house automation group directly employ those responsible for designing and programming the floating head pressure control system. Sub-contracting of this work to another company by the refrigeration contractor is not permitted. 5.0 The refrigeration contractor shall have completed minimum of 0 floating head pressure control systems for each of the past five years (50 systems in total.) Installation addresses and site contact names must be provided with bid submission. 5. The DDC control panel shall be built by a CSA certified panel shop. 5.2 The control system shall properly control the following equipment where applicable. Control up to four (4) compressors Compressor/pump cycling Supply and install new inverter duty motors Condenser is controlled by floating head pressure control strategy

4 204-0 TWO (2) SEASONAL CONTROLLERS 6.0 PRODUCTS Provide equipment run status (alarms!) Supply and install outdoor Air Temperature % RH sensor Run status for controlled equipment Day and night set points Sensor failure protection Alarm history E-stop/high level safety status 6. Refrigeration Management System condenser controller shall be the Cimco Seasonal Controller (or equivalent). 6.2 The controller shall have the ability to be upgraded to a CIMCO 6000E refrigeration management system by the same contractor. 6.3 Controller(s) must be Delta Controls Version 3.4 hardware or newer. 6.4 All control products provided for this project shall comprise a BACnet internetwork. Where applicable, communications involving control components (ie; all types of controllers and Operator Workstations) shall confirm to ANSI/ASHRAE Standard , BACnet. 6.5 Each Building Controller shall support BACnet over Ethernet and BACnet over IP. The Building Controller shall be connected to the BACnet network using the ISO (Ethernet) Data L/Physical layer protocol where applicable. 6.6 Controller must be BTL listed. Those products not bearing this label will not be considered.

5 204-0 TWO (2) SEASONAL CONTROLLERS GENERAL CONDITIONS SUPPLEMENTARY PAYMENT The Contractor shall accept the compensation as herein provided, in full payment for furnishing all necessary equipment and other incidentals as are required to meet the specifications of this Request for Proposals. Upon receipt of an invoice and delivery and installation of the two (2) Seasonal Plus Rink Controllers, or equivalent, to the North Simcoe Sports and Recreation Centre, a cheque will be made available. HOURS OF WORK The Contractor shall be limited to working between the hours of 6:00 am and 5:00 pm, Monday through Friday. No weekend work will be permitted except in the case of an emergency or by permission of the NSSRC Operations Manager. INSURANCE/W.S.I.B. CLEARANCE CERTIFICATE The successful bidder will be required to provide a certified copy of an insurance policy covering Public Liability and Property Damage in the amount of $5 million as specified in O.P.S. General Conditions, Section GC6, showing the Town of Midland as an additional named insured. The Contractor will be required to supply a certificate from the Workers Safety and Insurance Board (WSIB) indicating that he is in good standing with the Board prior to the Town authorizing commencement of work and prior to the issuance of a Certificate of Substantial Performance. It should be noted that the Town must be able to obtain a current WSIB Certificate at all times during the contract. WORKERS SAFETY AND INSURANCE BOARD CLEARANCE CERTIFICATE The Contractor shall provide the NSSRC Operations Manager with a copy of the Workers Safety and Insurance Board Clearance Certificate indicating the Contractor s good standing with the Board: ) Immediately prior to the Owner authorizing the Contractor to commence any work. 2) Every 90 days (as applicable) during the term of the Contract, including the maintenance period. 3) Prior to the issuance of the Certificate of Substantial Performance. 4) Prior to the expiration of the Maintenance Period. 5) At any other time when requested by the NSSRC Operations Manager.

10 204-0 TWO (2) SEASONAL PLUS RINK CONTROLLERS TOWN HEALTH AND SAFETY POLICY AND PROCEDURES MANUAL The Contractor shall ensure that all employees and sub-contractors employed under this contract are familiar with the above-titled document. Copies of this document will be made available as required. In addition:. The Contractor shall at all times during the performance of the work comply with all applicable federal, provincial, or municipal occupational health and safety legislative requirements, applicable regulations and all applicable industry standard and guidelines pertaining to the work. 2. Where the Contractor employs supervisors and employees, and if the contractor subcontracts to any subcontractor, the contractor shall ensure that appropriate policies and procedures relating to health and safety aspects of the work, appropriate training, and appropriate monitoring and enforcement of safety aspects of the work are carried out respecting supervisors, employees and any subcontractors. 3. The Contractor of a tendered document shall submit a work site safety program for control and supervision of all health and safety requirements at the work site to the Town of Midland for review prior to the commencement of work. After review of the safety program by the Town of Midland, it is understood that the Town of Midland for review with applicable requirements to ensure the safety of the work site, and that the contractor shall be solely and exclusively responsible for controlling the work site and taking all necessary steps to ensure conformity by all supervisors, employees and any subcontractors with applicable requirements to ensure the safety of the work. 4. The Contractor shall be solely responsible for the means, methods, techniques, sequences, procedures or coordinating the work performed under the work being hired to perform. The contractor shall be solely responsible for ensuring that all supervisors, employees and any subcontractors are informed of all known or foreseeable health and safety hazards at the work location. 5. The Contractor shall be solely and exclusively responsible for selection of subcontractors at the work site. 6. The Town of Midland shall have access to the work site for the purpose of inspecting the work site to determine compliance with the terms of the tendered document or work being performed, at any time. It is understood that such access by the Town of Midland representatives is not for the purpose of reviewing any health and safety aspect of the work or work site.

11 7. The Contractor agrees to indemnify and save harmless the Town of Midland, its officers, directors, employee, and agents from any liability, action, claim, prosecution, suit, loss, damages, payment cost, fine, fine surcharge, recovery of expense, including assessable legal fees, arising out of the performance of its obligations under this contract, including any negligent act or omission on its behalf or anyone for whom it is in law responsible. 8. Tender bidders are required to complete the Town of Midland s Contract Pre-Job Checklist (Health and Safety Form HS-4) as part of the Tender Bid Documentation. A copy of the Contract Pre Job Checklist (Health and Safety Form HS-4) is attached to this Quotation Form Document.
