This Addendum consists of two (2) pages with two (2) attachments. Attachments include the following: Demolition/Asbestos Containing Materials (ACM) Removal Schedule consists of two (2) pages and Responses to RFI Questions consists of four (4) pages. CLARIFICATIONS The following is issued to clarify the extent and location of the Asbestos Containing Materials (ACM) and supersedes what is shown on Drawing AD111 and previous addenda for the specific rooms listed. ACM is contained in: all floor tile and associated mastic on the first floor all mud fittings on water and steam piping the glue under the 1x1 ceiling tile Addendum No. 3 Page 1 of 2

2 A Demolition/Asbestos Containing Materials (ACM) Removal Schedule is attached listing the spaces and the action required. The Contractor shall review an existing ACM Survey Report describing the location of the ACM as it relates to the extent of work for this project within Ralston Hall and is available for review at the McEntire Building on the Clarion Campus. 4 RFI LOG Description 1 P101, P102 - Is the intent for the plumbing contractor to build chases as indicated? If so, please provide details and specifications. Response PC responsible for finished chases surrounding plumbing work. See details on P-103 rev 1. 2 P101 - Is it the intent for the plumbing contractor to relocate electrical conduit and wiring? If so, what is the extent of the work? Please provide details and specifications. PC responsible for relocation of electrical wiring and conduit in the course of performing work indicated on the Plumbing drawings and specifications. Work shall be performed in accordance with Division 26 specifications and the NEC. 3 P101 - Is it the intent for the plumbing contractor to relocate pneumatic tubing? If so, what is the extent of the work? Please provide details and specifications. 4 P101 - Are housekeeping pads required for the new air compressor and vacuum pump? If so, please provide details and specifications. 5 P103 - Is it the plumbing contractors responsibility to patch the chase wall in corridor 110A where new water and vent piping connect to existing? 6 Specification section S-1 calls for a wall mounted sink. Architectural details on A701 show a countertop sink. Please resolve. PC responsible for relocation of pneumatic tubing in the course of performing work indicated on the Plumbing drawings and specifications. Work shall be performed in accordance with Division 23 specifications. Yes. See detail on P-101 rev 1 PC responsible repair of surfaces in the course of performing work indicated on the Plumbing drawings and specifications. Work shall be performed in accordance with Division 09 specifications. Sink shall be countertop as indicated on Architectural drawings. 7 Please provide piping hook-up detail for air compressor and dryer and vacuum pump. 8 P102 - Is it the intent for the plumbing contractor to relocate data raceway and cable? If so, what is the extent of the work? Please provide details and specifications. Install per manufacturer s instructions. Termination of vacuum pump vent to outdoors not required. PC responsible for relocation of data wiring and raceway in the course of performing work indicated on the Plumbing drawings and specifications. Work shall be performed in accordance with Division 26 specifications and the NEC. 9 Specification Section A - Is a hydrant test necessary? Hydrant flow test not necessary for the performance of the contract unless otherwise required by NFPA. 10 Specification Section C - Please provide size, manufacturer and model number for hose cabinet. 5 RFI LOG 11 Will any of the equipment being proposed be tied into the bas system to ensure the maximum efficiency of the building? The controls for the equipment are currently stand alone. 12 Under the specification for the casework it looks like you are looking for Plastic Laminate Casework with Wooden Drawer Bottoms and Plastic Door and Drawer Fronts. Is this correct? 13 Can we go with an all wood cabinet with Plastic Laminate Door and Drawer Fronts and PVC Edge Band? 14 Will you be providing the most current fire alarm drawings for reference only? 15 Will the university allow a power outage during normal working hours at some point in the construction schedule?(7:00am & 3:30PM) No work is anticipated. Existing DDC system to remain operational as-is. Proposed work for controls includes, but is not limited to: connection of existing air compressor to pneumatic control air lines, replacement of existing pneumatic control air dryer, provisions for HVAC terminal units with integral controls (ie. self-contained control valves for radiation, integrated controls for window and split a/c units, stand-alone electronic controls for blower coil units) or connection to existing pneumatic controls for terminal units replaced like-kind. Yes. Alternative case construction will be considered. Detailing and colors of the PVC edgeband will need to be provided prior to any acceptance. Current fire alarm drawings are not available; bid as shown. Any power outage would require advance coordination with University to confirm the following factors, occupancy requirements, minimal-short duration of the outage, and weather (maintaining appropriate temperatures within the building). 16 P101 - Note 8 calls for the saw cutting of the floor. This floor is carpeted. Will this be removed during construction or how should this be addressed? 17 MD101 and MH101 show the hot water in the Laptop Classroom as Demo and as New is it both? 18 Please confirm if risers HWS/R 6 and HWS/R 10 are to be replaced under the alternate bid. They do not show on the Riser drawings but No. 6 is noted on MH101 to be replaced. 19 Are UH-3 and UH-4 to be replaced? They appear as being removed and replaced with new but there is no schedule. 20 Please clarify that a footer pad is required for the small condensing units shown on MH Please clarify where refrigerant tubing for ACU-A in room 137 is to be routed. The piping shows to be run on the roof but there are (2) floors above this level. Is piping to be routed up to the high roof and then back down to the lower roof and the condensing unit location? Contractor responsible for all cutting, patching and restoration of existing finishes for the performance of their contract. The intent is to replace all of the hot water piping to the fullest extent possible. This is why the risers are scheduled for replacement (as alternate) with some cost prohibitive exceptions (untouched piping in central chases). All HWS and HWR risers are to be replaced under the alternate with the exception of HWS/R 9 and HWS/R 13 as noted. Units to be replaced like-kind. Basis of design: UH-3, Trane model Force-Flo, Semirecessed, 28.4 MBH; UH-4 Trane model Force-Flo, fully-recessed, 27.9 MBH Pad location is a the base of a hill subject to high water saturation. Piers to below frostline are required. Refrigerant piping to rise from this level to roof directly above. Piping to run horizontal above first floor ceiling, combined with lines serving room 144 and rise to roof. 22 Are the original women s dormitory drawings of 1961 available for distribution as listed in the instruction to bidders? 6 RFI LOG 23 Are steel certs required for this project? If so do you intend to supply copies of the required forms? 24 Is the electrical contractor to install a temporary electric meter for construction/project power? Will the electrical contractor be required to reimburse the owner for power consumed during the project construction process? Spec Section B.1.04D indicates that the electrical contractor is responsible for reimbursement to owner for construction power consumption. I would like to suggest that the owner pays for all construction power consumption or provide an allowance dollar amount to be included in the electrical bid. 25 How is the damage to any existing surface to remain going to be tabulated per Spec Section F? Who is the responsible contractor to cover / protect these surfaces? 26 Is there a fire alarm specification for this project? Is the electrical contractor to turn over all removed fire alarm devices? 27 Is there a security / access control system specification for the required equipment and work as mentioned in the electrical summary of work? Yes The electrical contractor shall include the costs within their bid. The contractor should inlcude protection of existing services with their bid. No specifications, see the general notes on drawing FA111 No specifications, see the general notes on drawings E-001, EP111 and T Are the existing surface metal raceways to be removed in there entirety? Yes, all that are abondoned by this project or already abondoned at the start of the project. 29 Please provide the location of the communication equipment that is mentioned under general note 2 on sheet T-111. Provide a 4-11/16" square deep box with 1" conduit and grommets extended 6" into the ceiling space. See documents, door E102 is for room There is no door for room 166. AD111 calls for new door hardware on existing door. 31 On Ad 111 cannot locate keynote D6 Not applicable; deleted. 32 Sheet AR 111 details indicate 3 metal studs per item number, but say 3 Provide 3 5/8" metal studs as required. 5/8. Which is correct? 33 Sheet AR 112 same question about bulkhead framing on detail number See above FF&E calls for general contractor to supply coffee maker. No spec. 7 RFI LOG 35 The demolition drawing AD 111 seems to have a lot of discrepancies concerning asbestos and non-asbestos materials that need to be addressed. Rooms: 148A - Plaster ceiling - carpet over concrete 148B - Plaster ceiling - carpet over concrete 148C - Plaster with drop ceiling - 12" x 12" floor tile 148E - Plaster ceiling - ceramic floor 148F - Same as 148E 148G - Same as 148E 148H - Same as 148E Are denoted to have asbestos materials in them. As far as I can see no asbestos is present. If you look at each room above I have noted what is present. *There could be 9" x 9" asbestos floor tile under the 12" x 12" but not sure. Classroom 166 has no asbestos shown but it appears that there is 9" X 9" floor tile and mastic under the 12" x 12" floor tile. Rooms & have them denoted with ceiling tile which contains asbestos mastic but all of these rooms have plaster ceilings. These rooms do have carpet over 9" x 9" asbestos floor tile and mastic. Rooms Show no asbestos removal but the carpet in these rooms that is supposed to be removed and replaced is glued to asbestos 9" x 9" floor tile and mastic which will be disturbed if removed. See asbestos / demolition schedule. 36 Rooms: 137, 137A, 144, 144A, 144B & 144C These rooms are supposed to have only ceiling tile with asbestos mastic but these areas have plaster ceiling. They do have carpet which is glued to 9" x 9" floor tile and mastic. The removal of the carpet will disturb the asbestos containing floor tile. See asbestos / demolition schedule. 37 Will the University be using Hill - rom Beds in this facility? Hill-Rom beds were discussed and utilized as a Basis of Design, but final furniture has not been purchased to date. 38 Will the abatement company will do the flash patch and sand/buffing Yes. after removal of old VCT? 39 Does the project require waxing and buffing of the new Johnsonite Tiles after installation? Provide as indicated in the specification. 40 The specs call out a metal standard and supports for adjustable shelves on the interior of the cabinets. If you look at the attached file on page 12, it shows the industry standard of drillings and adjustable, anti-tipping, shelf clips. Would you please let me know if this will be acceptable? Shelf supports are acceptable; please provide metal shelf clips or equivalent durable product. 41 the specs call out for the cabinet locks are to be master keyed to the University System. If this is the case, please clarify what section is to include the locksets, or is our standard 6 pin deadbolt lock acceptable?
