1 FLORIDA DEPARTMENT OF TRANSPORTATION ADDENDUM NO. 4 DATE: March 30, 2016 RE: BID/RFP #: BID/RFP TITLE: ITB-DOT-15/ JR Districtwide Right of Way Asbestos Abatement Services for the Florida Department of Transportation OPENING DATE: April 4, 2016 Notice is hereby given of the following changes to, and answers to questions asked regarding the abovereferenced BID/RFP: 1. Change: Exhibit C, Bid Sheet is hereby deleted, and replaced with the attached, revised Exhibit C, Bid Sheet. Upon issuance of this addendum, Page C-1, Section 2.0 Preparation, Cleaning and Lockdown of Exhibit C, Bid Sheet will include the addition of subsection 2.5 Spray apply one coat of encapsulant lockdown. Failure to do so may subject the bidder/proposer to disqualification. 5 No set volume of work is guaranteed; the estimated quantities above are shown for purposes of determining the low bidder. If Vendor does not charge for an item, he or she shall insert a zero ("0"); if any items are left blank, the bid shall be considered non responsive. If the actual quantities during the agreement period are less than or greater than what is identified, the unit rates shall remain unchanged. Compensation to Vendor shall be at the rates set forth above as proposed by Vendor. Vendor named below agrees that, if this agreement is renewed, it shall be renewed for the same price, terms, and conditions as set forth in the original. The Project Manager and Vendor shall negotiate items of work that are not included in the unit price schedule as required by the work assignment. Each project, as defined by the Project Manager, shall be negotiated as a total sum based on bid items and prices listed above (by multiplying the actual number of units for the work description by the unit prices, as specified above). When a total sum cannot be reasonably determined, the Project Manager shall negotiate a maximum amount based on the unit prices and Vendor shall bill as allowed by the Letter of Authorization. Payment will be made upon satisfactory completion of each Letter of Authorization. Payment in full of a work authorization amount shall be made by the Department upon completion by Vendor of all work required to be performed under the terms of the agreement and acceptance of the work by the Department. MFMP Transaction Fee: All payment(s) to the vendor resulting from this competitive solicitation WILL be subject to the MFMP Transaction Fee in accordance with the attached Form PUR 1000 General Contract Condition #14. However, all vendors should be aware that effective November 1st, 2015 through July 1st, 2016, in accordance with Senate Bill 2502 A, the Transaction Fee will change from one percent (1%) to seventenths of one percent (.70%) of the payment issued. The Transaction Fees imposed shall be based upon the date of issuance of the payment.. NOTE: In submitting a response, the bidder acknowledges they have read and agree to the solicitation terms and conditions and their submission is made in conformance with those terms and conditions.
