Timble, PE, A&Z Engineering, LLC The following BID/PROPOSAL page is to be added/changed to the contract documents: The attached BID/PROPOSAL page replaces the original page. A FULL AND COMPLETE COPY OF EACH CORRECTED PAGE IS ATTACHED TO THIS ADDENDUM. You must acknowledge receipt of this addendum by signing the first page, returning it via facsimile to (260) , and a copy of the first page must be included with your bid. This addendum consists of a total of 7 pages Also included with this Addendum are attachments, which include the following: Attachment No. 1 Revised Bid Form (2 pages + cover page)

3 ATTENTION BIDDERS All Bidders submitting a Bid on the above Contract shall carefully read this Addendum #1 and give it consideration in the preparation of their Bid. All Prime Bidders are responsible for informing their appropriate Subcontractors and Material Suppliers of these Addendum items. ADDENDUM ITEMS PROSPECTIVE BIDDER S QUESTIONS AND ANSWERS Q = Question, A = Answer Q: Is there a wage scale on this project A: There is no wage scale on this project. Q: Are commercial 53 s acceptable for this project or do they need to be State s 53s? A: 53 s shall be per current INDOT standards and specifications. Q: On TS-3, it says structure backfill for pipes under pavement shall be included in the contract unit price per linear foot of pipe. But then we have pay item #39 Structure Backfill, Type 1 for 113 CY. What exactly is this pay item for? A: Pay Item #39 stricken off from bid form. See attached Attachment No. 1 Q: Will owner s protective insurance and builder s risk/installation insurance be required for this project? See G-14 of specs. A: Yes, the successful contractor will be required to carry both owner s protective insurance and builder s risk/installation insurance. Q: Will Wells County require any permits and if so what are the cost? A: No permits and corresponding fees will be required from the contractor by Wells County on this project. Q: Will we be responsible for testing other than required test for HMA? A: Geotechnical Report Recommendations and Current INDOT standards and specifications shall be followed for testing. Q: Advertisement for Bid states bid bond is 10%. ITB page 4 states bid bond is 5%. Please clarify. A: Bid bond shall be for 5% of the bid amount. Q: Advertisement for Bid states our proposals be valid for 60 days. ITB page 1 states 90 days. Please clarify. A: Proposals shall be valid for 90 days.
