1 Carl Schoedel, P.E. irector of Transportation County Engineer KANE COUNTY IVISION of TRANSPORTATION 41W011 Burlington Road St. Charles, IL Phone: (630) Fax: (630) Section RS 2018 Kane County Resurfacing Patrol March 12, 2018 Addendum - Special Provisions, sheet 7, have been revised to reflect changes to the patching requirements; CA-6 stone base shall only be used with the 12-inch patches on Fabyan Parkway (Approx. 750-ft E of Bent Tree to Approx. 1,200-ft E of Bent Tree) and on Thatcher Road. - Special Provisions, sheet 15, have been revised to reflect changes to the specified percent air voids for Leveling Binder (Machine Method); from 4.0% to 3.5%. - Special Provisions, sheet 19, have been revised to reflect changes to the specified HMA Surface Mix for Temporary Asphalt Ramps; Temporary Asphalt Ramps shall consist of HMA Surface Course, N50. - Typical Cross Section for Fabyan Parkway (Approx. 340 W of Carriage r to Kingsland r) has been revised to reflect changes to the specified HMA Surface Course Mix; Polymerized Hot Mix Asphalt Surface Course, Mix E, N70, 1 ¾ shall be used. - Typical Cross Section for Randall Road (IL Rte 38 to Oak St) has been revised to reflect changes to the specified HMA Surface Course Mix; Polymerized Hot Mix Asphalt Surface Course, Mix E, N70, 2 ¼ shall be used. - Typical Cross Section for Randall Road (US Route 20 to I-90) has been revised to reflect changes to the specified HMA Surface Course Mix; Polymerized Hot Mix Asphalt Surface Course, Mix E, N70, 1 ¾ shall be used. Note: This addendum is hereby declared a part of the proposal documents for the project designated above, and in case of conflict, the addendum shall govern. Bidders shall incorporate in their proposal this addendum in acknowledgement of receipt of the addendum and that it is reflected in the proposal submitted. Addendum March 12, 2018 Page 1 of 7

2 Kane County Section RS 2018 Resurfacing Patrol CLASS PATCHES, TYPE IV, 12 INCH escription: This work shall consist of saw cutting and removing the existing pavement and any additional material required to reach a depth of 12-inches and the replacement of 12-inches of material at locations as directed by the Engineer. This work shall be done in accordance with Section 442 of the Standard Specifications. For Route #2 (Fabyan Pkwy) and Route 3 (Thatcher Rd): This item will cover furnishing and compacting 4-inches of CA-6 stone base and 8-inches of bituminous materials, per the Mix Selections specified herein, to complete the entire area of the specified pavement patch. Bituminous materials (tack coat) shall be placed between layers of the HMA pavement structure. For all other Routes: The as-needed patching locations shall be located as directed by the Engineer and shall be in accordance with Section 442 of the Standard Specifications. Basis of Payment: This work will be paid for at the contract unit price per square yard for CLASS PATCHES, TYPE IV, 12 INCH, which price shall include all labor, material, and equipment necessary to complete the work described above. CLEAN CONSTRUCTION OR MOLITION BRIS Earthwork operations for this project shall be completed in accordance with Section 202 of the Standard Specifications and material properly disposed of in accordance with Article This special provision only applies if the Contractor chooses to dispose of material at a permitted CC or registered uncontaminated soil fill facility. The Contractor is advised to consider the cost of disposing of all surplus materials off-site and properly reflect those costs in their bids for earthwork and removal items. The Contractor must be thoroughly familiar with the provisions of the Illinois Environmental Protection Act as it relates to proper disposal of excavated material and construction n debris. Should the Contractor choose to dispose of materials at a permitted CC or registered uncontaminated soil fill facility, the Contractor shall be responsible for the lawful removal of all excavated ated soil, material and other clean construction or demolition debris in compliance with Public Act isposal of materials at a permitted CC or registered uncontaminated soil fill facility will require that Form LPC-663 be submitted to the operator of that location before any materials can be disposed of at that site. Each certification shall cover only material from the specified job site. The Contractor shall be responsible for having the required analysis of soil materials completed and the Form 663 adequately completed and signed by a Professional Engineer or Geologist licensed in the State of Illinois The Contractor is not responsible for the cost of soil remediation. In the event material is rejected it will be returned to the site where the extent of additional effort required to dispose of the material 7

4 Removal of short-term pavement marking shall be provided by the Contractor tor who installs the short-term pavement marking, prior to installation of the permanent pavement ent marking. The contractor will be notified a minimum of 48 hours prior to installation of the permanent pavement marking. U
