Refer to the attachment for the pre-proposal sign-in sheet. Proposal Form A. Refer to the attachment for the revised proposal form, FORM M. B. ALTERNATE #1 Deduct: eliminate installation of temporary chiller (phase 1) C. ALTERNATE #2 Deduct: All piping to be provided by contractor and installed by owner or others. D. ALTERNATE #3 Deduct/Add: fluid cooler to be provided on moveable trailer in lieu of 8 concrete pad E. ALTERNATE #4 Deduct: provide a 50 amp circuit in 1 conduit with #6 wire, in lieu of 225 amp circuit. General A. Per discussions with equipment manufacturers at the pre-proposal meeting, the phase 2 construction will include the installation of a fluid cooler system in lieu of the chiller as specified. Refer to information below for all fluid cooler operational requirements. The contractor shall include 1 domestic makeup water line from adjacent janitors closet (RPZ located in closet, pipe to be buried and turned up at the fluid cooler). Fluid Cooler A. The fluid cooler system shall be provided with integral pumps, heat exchangers, fluid cooler, etc for a complete operational package. The system shall have a single point electrical connection, with VFD driven motor(s). The system and all components are to be controlled by the existing Trane BAS. 1. 2 BAS - Trane A. The existing Trane BAS shall operate all equipment, including the new 3-way automated valves. 4 Haltom Middle School - Supplemental Chiller for Existing Geothermal Well Field RFCSP NO FORM M PROPOSAL FORM We, the undersigned, propose to enter into a Contract with the Birdville Independent School District to furnish labor, material, tools, transportation, insurance, permits, and all incidentals necessary for the completion of the subject project, in accordance with the specifications and plans included in this proposal package. Haltom Middle School The project includes installation (phase 1) and subsequent removal (Phase 2) of a supplementary chiller to an existing geothermal field including but not limited to all electrical work and all piping work. The project includes the installation of a new, permanent fluid cooler (Phase 2). We have carefully reviewed and understand the Instructions to Proposers, the specifications and have aquatinted ourselves with the existing and anticipated conditions that might affect the Work. We understand that if we are the Successful Vendor, a Contract will be prepared and if that Contract is in excess of $25,000, we will furnish satisfactory payment and performance bonds each in the full amount of the Contract covering all parts of the Work. BASE PRICE PROPOSAL AMOUNT (Dollars) BASE PRICE (including Allowances per 2.0) $ 2.0 Allowances The following allowances are to be included in the Base Prices (above). Allowances shall include Contractor s overhead and profit. To adjust allowance amounts, Contractor will be asked to prepare a proposal based on the difference between the actual allowance expenditure and the amount of the allowance. Unit prices include all necessary material, delivery costs, installation, insurance, overhead and profit. Failure to provide unit pricing for each item may result in the Proposal being deemed as non-responsive. UNIT PRICE NO.
